Starting December 1, 2016, you will no longer get a monthly paper drug eligibility card with your statement ifyou have an Ontario health card.

Instead, you and each of your familymembers, will use your Ontario healthcards to access drug coverage.

To ensure that you can access drugcoverage, you should get an Ontariohealth card, if you do not have one.

If you are not eligible for an Ontariohealth card, you will continue toreceive a paper drug eligibility carduntil you receive one.

If you, (or your family member), are not eligible for an Ontario health card, contact your ODSP/ACSD or Ontario Works
