Overview:

The Location Disaster Recovery Plan (LDRP) consists of the following four elements in order:

1. Ensure Safety

2. Establish Communications

3. Assess Damage

4. Resume Operations

The LDRP establishes an Emergency Management Team (EMT) which becomes the HUB for communication

within CFL LTL, with our Customers, with our Suppliers, and with repair contractors as necessary.

The EMT will set up operations in a Command Center (maybe virtual) at CFL if available; otherwise operations

will be established at a Temporary Command Center.

The LDRP also establishes responsible team leaders for resumption of operations in affected areas and address

Supply Chains which might be a disruption to our Operations.

If the facility is not functional, the LDRP establishes alternate operating sites available short notice for

emergency operations. Employees may be notified to work remotely, operations will be recovered as quickly

to ensure a transparent and seamless operations to our customers and employees.

Ensure Safety:

If the building is occupied, safety is ensured by evacuating to the designated evacuation area and conducting an

HR roll call based on the day’s attendance records. If any injuries have occurred, first aid is to be administered

by a qualified first aid responder. A call for an ambulance is to be made for any medical assistance required. In

no event should an employee be transported to a medical facility with a private or company vehicle.

Establish Communications:

The first step in establishing communications is to notify a member of the Emergency Management Team. The

members of the EMT and their cell phone numbers are:

The four questions to be answered in the initial notification are:

1. What is the nature and extent of disaster event? (initial judgment, a detailed assessment is to be

performed as next step)

2. Are there any known injuries?

3. Can the building be occupied?

4. Do the telephone, fax and email systems work?

The notified EMT member shall notify senior management and the other EMT members.

If the building can be occupied and communication systems are functioning, EMT operations are to be

established in a Command Center. Otherwise, EMT operations will be established in the Command Center.

Assess Damage:

After safety has been ensured and an EMT member has been notified, a thorough assessment of the damage,

accessibility of the facility and operational capabilities will be made for the impacted areas. This assessment

will be led by either the area Team Leader, or a member of the EMT. Using the Damage and Operational

Assessment Checklist as a guide, document the damage and operational capability of the facility. The

underlying question to answer is: Can CFL operate in this location or do we have to move elsewhere?

The damage assessment shall be given to the Emergency Management Team.

Resume Operations:

The EMT will notify the affected LDRP Team Leaders that the Facility Disaster Recovery Plan is in effect.

Designated Team Leaders are responsible for managing the resumption of operations in their respective

locations. The one operational area and Team Leader is:

a guide for resuming operations.

Team Leader Responsibilities / Checklist:

General:

The Primary responsibility of the Team Leader is to provide leadership of the recovery team and coordinate

support for the recovery effort. Other responsibilities include:

1. Participate in Resumption meetings with the Emergency Management Team.

2. Direct the Business Continuity efforts of your team.

3. Oversee communications activities of the team.

4. Coordinate with the Emergency Management Team regarding all administrative issues.

Normal Business Hours Response:

During an emergency that happens during normal business hours, follow the corporate emergency procedures to

ensure the life and safety of all employees.

If the building is not accessible, the team personnel should assemble at:

- Primary site: (Depending on building)

- Alternate site: TBD

Immediate actions to be taken by the department leader or assigned alternate:

1. Take a head count to make sure all team members are safe and available. Notify the

Emergency Management Team immediately if anyone is missing.

2. Look and call for a member of the Emergency Management Team to get instructions.

3. Record all the information and instructions given by the Emergency Management Team. Use the

Notification Checklist located in this section as a guideline and work paper.

4. Before contacting anyone else review the Notification Procedure located in this section.

5. Notify department personnel not already notified. Use the Notification Call List located in this section; it

contains a list of who to call and what information to pass on.

6. If instructed by the Emergency Management Team, activate the Recovery procedures for this area.

After Normal Business Hours Response:

When notified by the Emergency Management Team that the Business Resumption Plan has been activated, the

team leader will:

1. Record all the information and instructions given by the Emergency Management Team. Use the

Notification Checklist located in this section as a guideline and work paper.

2. Before contacting anyone else review the Notification Procedure located in this section

3. You may be instructed to only notify your alternate team leader, your entire team or as many department

personnel as possible. Record the status of all notifications and give the completed call list to the team

leader.

4. If instructed by the Emergency Management Team, report to the Emergency Operations Center.

5. If instructed by the Emergency Management Team to activate your Recovery Team, procedures are located in this

section.

Team Recovery:

Business Resumption Plan Copies

The team leader should ensure that sufficient copies of the Disaster Recovery Plan are available.

Cellular Phone (TBD)

The team leader has a cellular phone for team use. The Emergency Management Team should be notified

immediately of the cellular phone number.

EMT Work Area

The Emergency Management Team will provide the team with a work area for their use. Use the Business

Recovery Work Area Checklist in the appendix to ensure that the area is setup to match the requirements that

the Recovery Team will need to support the recovery operation and resume essential business functions.

Notifications

Provide notification of the problem to vendors. The information provided will be reviewed with the

Emergency Management Team prior to sending update. Notification may be email, or a phone call.

The following recovery actions are to be used as a guide. During a disaster circumstances may dictate

that some or all of the steps documented may have to be altered. The team leader should use his/her

judgment while managing the recovery operation.

1. The team leader should contact the Emergency Management Team to find out:

• When voice communications will be available at the work area.

• When servers will be operational and how current the master files will be.

2. Departmental Meeting:

Key area personnel should meet to determine actions to be taken and establish the priority of

restoring business functions based on the work area and resources available. The area Team Leader

should explain the goals and objectives identified by the Emergency Management Team.

a. Review tasks to be performed and assign personnel.

b. Personnel should be assigned to contact vendors and advise them about the situation and when

they can expect service to be restored. Use the Vendor Notification in the appendix for contact

information.

c. Determine if some personnel will have to travel to the business recovery site.

d. Distribute copies of any forms that will be needed during the recovery operation.

e. Distribute copies of the news media statement that has been prepared. Copies can be obtained

from the Emergency Management Team. Instruct everyone not to make statements to the news

media.

f. Personnel should be assigned to provide recovery support needed by other teams, as needed.

g. Identify the category in which personnel should be alerted. Consider:

- Personnel that might be needed to give aid to other teams / departments.

- Personnel that will be needed at the work area to resume normal business functions.

- Personnel who should stay home and remain on standby (they will be needed when the initial

group needs rest).

3. Contact personnel that will be needed to report to the assigned work area.

4. Designate space for personnel reporting to the work area.

5. Implement procedures to resume time dependent functions based on the priority established.

6. Instruct all department personnel to carry photo identification with them at all times and be prepared

to show it to security or local authorities.

7. As progress continues during the recovery operation, the team should be prepared to move back to

the affected facility and resume normal business operations.

Notification Procedure:

The team leader, alternate or assigned individual upon activation of the Business Resumption Plan will contact

team personnel using the following procedure:

During notifications of an alert or declared disaster, use this procedure to alert all personnel. Read the procedures

thoroughly prior to making a call. By using the following instructions, you will not unnecessarily alarm family

members of an employee who was working at the affected site at the time of the disaster.

Place phone call and say, “May I speak with (individual)?”

1. If available, provide the information you called to convey.

- Remind the person to make no public statements about the situation.

- Remind the person not to call co-workers (unless instructed to) and to advise their family not to call other

employees.

- Record the information in the contact status column.

2. If not available, say, “Where may I reach (individual)?”

- If at any location other than the data center, get the phone number. Call the other location and providing the

information you wanted to convey.

- If the individual was working at the affected site, indicate that you will reach the individual there.

DO NOT discuss the disaster with the person answering the phone.

- Immediately notify the Emergency Operations Center.

- Record the information in the contact status column.

3. If contact is made with an answering machine: Make no statement regarding the situation.

Provide the phone number to call at Emergency Operations Center; ask that the employee make contact at that

number as soon as possible.

- Record the information in the contact status column.

4. If no answer:

- Record the information in the contact status column.

5. If no answer and the individual have a beeper:

- Place a call to the beeper number.

- Enter the number of the Emergency Operations Center for the individual to call.

- Record the information in the contact status column.

Notification Call List:

Using the team member contact list in the front of the plan, the team leader, alternate or assigned individual

should convey the following information when contacting the team personnel:

• Brief description of the problem.

• Location of the Emergency Operations Center and / or the Business Recovery Site/Command Center

• Phone number of the Emergency Operations Center.

• Immediate actions to be taken.

• Whether or not the facility can be entered.

• Location and time the team should meet.

• All team members should carry photo identification with them at all times and be prepared to show it to

security or local authorities.

• Instruct everyone notified not to make any statements to the media.

All callers should record status of everyone they call, noting the time the call was placed and whether the person

was contacted. Make a reasonable number of attempts if the phone was busy or there was no answer. Forward

the completed list to the EOC (Emergency Operations Center or Command Center) and the staff will continue

to attempt to contact team members.

Evacuation Plan – Each Location/Terminal

PURPOSE: The purpose of this plan is to provide guidance for CFL employees in case of emergency or disaster. A

disaster may be to a physical location or a public event.

LOCATION: See Location List/ Terminal Manager/ Locations DEFINITION: All CFL locations

An emergency / disaster is a situation that has occurred which requires immediate attention and

assistance. Situations can include injury, fire, public health event or any force of nature.

I NOTIFICATION IN AN EMERGENCY:

• It is the responsibility of each employee to report an emergency situation to his / her supervisor

immediately or Contact HR Dept. and EMT if the emergency is a public health event.

• HR. Dept will contact EMT of the event type. Confidentially will be maintained at all time as

Centers for Disease Control and Prevention (CDC), federal and state laws will be adhered to while

handling the event.

• If an injury occurs and requires medical attention, the injured should be transported to the

emergency room or established medical clinic if available by way of a medical responder.

Emergency – Dial 911

• The Safety Manager and General Manager must be contacted.

• A representative of management should meet the injured associate at the hospital to check on the

condition of the associate and confer with doctors if necessary. A family member of the

associate should also be contacted.

• If an associate has suffered a minor injury, supervision will be notified and a first aid, CPR

responder should check the associate. Associate could be asked to report to the company

physician and someone from management would follow the associate to the doctor and confer

with his staff.

Note: An investigation of all injuries should be performed and reported to management. Extent of

injuries must be reported to the EMT, Safety Manager and Human Resources Manager.

II FIRE EMERENCY:

– Fires are extremely dangerous and may cause large amounts of damage to the location and even

loss of life.

– Professional fire fighters should be called if there is a major fire.

– If a major fire occurs Dial 911 and give the dispatcher, the facility address.

– When fire fighters arrive direct them to the location of the fire.

– If a fire occurs, the alarm system will sound.

– Employees are to immediately exit the building through the nearest exit. Exit lights above the

exit doors are always illuminated and in the event of a power failure. Floodlights located on

each side of the exit lights will come on.

– There will be designated assembly points outside the building indicating where employees are to

report.

– The Safety Coordinator is assigned to each location and he / she will be responsible to account

for each associate and any visitors that may be on site.

a) Accidents/ Personal Injury: If an injury to a person occurs at a location/terminal:

1. Immediately attempt to locate the on-site person qualified in CPR or a manager; follow the

instructions of the manager and/or CPR qualified person.

2. A manager will call the appropriate health organization.

3. If a CPR qualified person or manager is unable to be found, the senior employee, i.e. team

leader should call 911, if necessary.

4. Do not move or touch the injured person to avoid further injury.

5. Input information in Safety Box when feasible after the event.

b.) Fire: In case of fire in the facility, the employee should immediately notify a manager. The manager

will:

1. Immediately call the local fire department and inform all other managers and team leaders.

2. Implement the emergency evacuation plan as prescribed in the EDP.

3. If fire is small enough to extinguish immediately, employ the nearest fire extinguisher.

4. Other considerations:

• For customer products: determine the quantity and hazards collect product labels and

safety data sheets, as appropriate.

• Inform local medical facility to ensure a physician is standing by in case needed.

• Begin evacuation of personnel downwind; isolate the affected area, ensuring the area is

patrolled to keep out spectators.

• Avoid products, smoke, mist and run-off; in case of contact follow first aid procedures.

– All attendance sheets will be turned into the safety coordinator, or shift supervisor who will

check the names against the staff roster and visitor log to ensure accountability.

– An all clear signal will be given after the fire has been put out, smoked cleared, and it has been

determined safe to re-enter the building.

III ACTS OF NATURE:

– Acts of nature can pose a serious threat to site personnel, equipment and operations. In the event

of an earthquake, tornado or hurricane, all associates and visitors are responsible to take cover in

designated areas. Please refer to your Location/Terminal Plan.

– The on-duty Shift Supervisor will put out a message to inform associates of the act of nature and

the need to take shelter.

– All associates and visitors are to go to the Designated Area for your particular location as listed

in the location plan.

– The Location Safety Coordinator will account for the employees and visitors who might be

located on site.

– The Terminal Manager and Safety Coordinator will be responsible for monitoring the act of

nature, making a decision on when to go back to work, and for assigning clean up assignments as

needed.

IV PUBLIC HEALTH EVENTS:

– Public Health events may be reported by an employee disclosure or via customer or vendor

communication. If an employee reports the event to a co-worker, supervisor or management –

HR Dept must be contacted immediately. HR Dept will contact the EMT.

– Employee should be isolated from others and have the employee talk with the HR Dept.

– Communications will be developed and disseminate to CFL employees, customers and vendors

if the change has an impact on delivery options. CFL intent is to meet all delivery obligation

within the defined time frame.

– The employees will be notified of CDC recommendations which may require testing. If the event

is under CDC directives, CFL will be under their directives.

– EMT will follow CDC notification protocols for co-workers, customers and vendors.

– Work options may be provided depending on the CDC directives. Ex: working remotely, shift

changes, assigned to a different work location.

– Technology solutions will be an integral solution to continue work productivity which will

empower our team to keep up with rapidly changing priorities to meet our continued success.

V TRAINING REQUIREMENTS:

– There should be two associates for each shift that are trained in first aid and CPR.

– Supervisors and designated backups should be trained in First aid, CPR and proper use of fire

extinguishers.

VI EQUIPMENT:

– Basic First aid kit

– Bloodborne Pathogens Disposal Kit

– Flashlights, batteries, Gloves

– Weather Band Radio
