based certified nursing program to practice

and master skills necessary for safe patient

REQUIREMENTS bull Physical Exam

bull Proof of Hep B bull Flu shot is highly recommended not required

bull Proof of TB ndash QuantiFeron-TB Gold Test or Mantoux (Skin Test)

bull CPR Card Required On-line CPR from any approved provider is ok for renewal and initial applications Card must say ldquoHealthcare

ProviderBLSrdquo (Please see preregistration packet for more details)

Packets will be accepted while space is available Once the class is full no more packets will be accepted

Once all requirements are completed and approved you will

receive your Request for Live Scan form Class Schedule and Initial Application

October 15th for the SPRING semester and

May 15th for the FALL semester

SELECTION PROCESS

The first 30 students who complete the
