Marine Orders Part 41 (Issue 9) Provision 7
I, the undersigned, Alexander G.T SCHULTZ-ALTMANN, hereby accept the dangerous goodstraining course available from the training provider identified below, for the purpose of complyingwith the requirements of:
The requirements of which give effect to the mandatory training requirements for shore basedpersonnel as prescribed in:
Chapter 1.3 of Volume 1 of theInternational Maritime Dangerous Goods (IMDG) Code (2008 edition)
This approval relates to the "AMSA Accepted" training that is to be provided to shore basedpersonnel who undertake any of the following functions:
I. Packing of dangerous goods, [including those that supervise the packing of Cargo Transport Units];
II. Marking, labelling or placarding of dangerous goods; and
111. Preparing and signing transport documentation [shipper and container packing declarations].

CONDITIONS:
1 This acceptance is issued in respect of the 2008 edition of the IMDG code and a verificationaudit will be required when the training package is updated to the next edition of the IMDGcode. This is to occur before the 2008 edition of the code is finally superseded by MarineOrders Part 41 ;
2 The certificate number of this acceptance certificate is to be included on all training recordsissued in relation to it;
3 A copy of this approval is to be provided to users of the training course upon request; and
4. A short term certificate has been issued to allow time for AMSA to conduct verification audit onthis training package.
Subject to the conditions this acceptance certificate shall remain valid for Haylock Maritime untilthe 30th of December 2009, unless it is withdrawn or altered for any reason before that date.
