PROGRAM PLAN FOR THE CENTER FOR ORAL HEALTH

I. Preface and Summary 1. Executive Summary A new Center for Oral Health facility housing the University of Colorado School of Dentistry (UCSD) at the University of Colorado Health Sciences Center (UCHSC) Fitzsimons campus will considerably enhance the provision of clinical education for providers of oral health care to the benefit of Colorado citizens. The mission of the School of Dentistry is to provide programs of excellence in teaching, research, patient care, and community/professional service for Colorado and the nation. The ability to serve the State of Colorado pursuant to this mission has been greatly enhanced by the opportunity to be a part of the 21st century academic health center being built on the Fitzsimons Campus. At the core of the UCHSC Institutional Master Plan, approved by the University of Colorado Board of Regents in October 1998, is the relocation of all of the educational programs of the five UCHSC schools (Dentistry, Graduate, Medicine, Nursing, and Pharmacy) to new state-of-the-art facilities. The Center for Oral Health program plan fulfills this goal for the School of Dentistry. The UCSD is the only dental school in Colorado and in the entire Rocky Mountain Region. The school has a DDS program, a dental hygiene program, a general practice residency program, and a pediatric dentistry residency program in collaboration with the Children’s Hospital. An orthodontic and dentofacial orthopedics program is being developed. The past three decades have witnessed significant development and sophistication in all three areas of the school’s programmatic mission: teaching, scholarship, and service (patient care and community activities).

The School of Dentistry’s Strategic Plan 2000 describes a strategic direction that will build on the school’s historical strengths and provide opportunities to better serve Colorado. School of Dentistry faculty will collaborate with other members of the Health Sciences Center to provide interprofessional educational experiences to students, while improving the dental and general health of the patients served. By linking the education of all health care professionals, future practitioners work together much as they will in future practice, to provide the highest quality patient care. Whether considering the birth weight of babies born to mothers with periodontal disease, the relationship between periodontitis and cardiac disease, or the lost productivity of American families because of children’s dental diseases, it is clear that oral disease has consequences to the health of the general population. The planning of this clinical education facility provides an opportunity to address an unmet educational need for pre-doctoral and post-doctoral dental education programs in Colorado. Because of the rigor and multiple community-based practice experiences in the dental curriculum, the CU School of Dentistry has enjoyed a long history of producing excellent general dentists. The DDS degree is a terminal degree, and the overwhelming majority of its 649 DDS graduates, as well as its 428 dental hygiene graduates, have remained in Colorado. A dental manpower shortage of general dentists, specialists, and dental hygienists is projected for Colorado in the next decade. The Colorado Report: A Study of the Relationship Between the University of Colorado School of Dentistry and the Supply of Dentists in Colorado – May 2001 summarizes Colorado’s maldistribution of dentists and projected future shortages. The construction of the Center for Oral Health will consist of 87,560 gross square feet to be built in the educational zone of the Fitzsimons campus. The structure will be sited adjacent the Educational Facility 1B and diagonally across from Education Facility II, the two facilities housing interdisciplinary campus classrooms and lecture halls. The building will be designed to accommodate and support future structural expansion, while the finished areas will be designed as flexible space to accommodate repurposing to meet anticipated and unanticipated future program needs. The Center for Oral Health primarily will contain clinical, clinical support, dental laboratories, and dental patient simulation spaces. The curriculum requirements of the national accreditation organization – the American Dental Association, include expectations for gaining clinical competence by performing preclinical laboratory exercises and treating patients in a clinical setting. In the physically constrained existing facility, students and faculty provided oral health care for approximately 33,000 patient dental visits to Colorado residents in the past year. The Center for Oral Health will provide the clinical education space to satisfy the bulk of the ADA requirements, while community based public health centers and rural practice sites provide the rest.

The following will be housed in the Center for Oral Health: The pre-doctoral dental program Pre-clinical education laboratories used to instruct students in the fabrication of

crowns, bridges, etc. A patient simulation clinic The baccalaureate dental hygiene clinical program Discrete dental specialty clinics for providing advanced patient care and specialty

programs. The current facility at the 9th and Colorado Boulevard campus was built in the 1970s and the spaces were planned to meet the needs of a dental class size of 25 students. The school’s dental class size has expanded to 40 and is projected to expand to 50. The Center for Oral Health will accommodate this growth and provide for more appropriate discrete settings for dental specialty clinic operatories. The program plan for this building was developed with considerable input from a faculty oversight committee of the School of Dentistry and representatives of other UCHSC programs. The spaces for this building were identified and planned based on the programmatic requirements of the School’s 21st century curricula and on the projected future dental manpower needs of the state of Colorado. Additionally, flexibility has been incorporated in this plan to address future changes. An important challenge to meet the necessary clinical and support spaces for high quality student learning experiences was coupled with the need to plan spaces equally supportive of the provision of high quality patient care. The 87,560 gross square foot facility is estimated to cost $26.5 million. Design for the project is planned to begin in a fast track mode following anticipated receipt of approvals. Building Support (Other) – 6,175 asf

The purpose of this facility program plan is to seek appropriate approvals of the University of Colorado Board of Regents and the Colorado Commission on Higher Education (CCHE) to: Permit the University of Colorado Health Sciences Center to proceed with the planning, design, and construction of a 87,560 gross square foot facility at the Fitzsimons site to house the clinical program of the School of Dentistry at a project cost of $26,500,000. 2. Process The Institutional Master Plan that details the development of the University of Colorado Health Sciences Center at Fitzsimons describes the need for a specifically designated clinical dental education facility. This facility is unique because, while its primary purpose is to provide access to clinical education for students pursuing professional education in dentistry, it also provides access to reduced-fee dental care to patients in the community. During subsequent updates to the Master Plan, the dental clinical education facility continued to be an integral program element. The name of the facility changed to the Center for Oral Health and the square footage was modified as the vision for the center became more refined. A program plan for the clinical education facility was drafted in the spring of 2000, primarily by the Oversight Committee comprised of School of Dentistry faculty and staff. The Oversight Committee was commissioned by the Chancellor of the UCHSC to identify specific programmatic needs for the clinical facility intended to support the clinical curriculum of the School of Dentistry’s programs. The steps taken in developing this plan included: Discussing the future strategic direction for the School of Dentistry, and the appropriate

facilities needed at Fitzsimons; Identifying enrollment goals to address projected shortages of general dentists, dental

specialists, and dental hygienists; Agreeing on goals and assumptions for the facility and the facility’s major functions of

providing clinical education simultaneously with patient care; Identifying specific types of spaces to meet program needs; and Identifying the appropriate location for the facility.

The Oversight Committee became less active while funding opportunities continued to be pursued. In 2002, the Oversight Committee was reconvened with the anticipation of project funding. During meetings in the summer and fall of 2002, the committee reviewed and planned the following topics as components of the program plan for the Center for Oral Health: Conducted a comparative site analysis of six alternatives on the Fitzsimons campus and

recommended the site proposed in the program plan;

Reviewed future strategic directions for the School of Dentistry including visions for curricula and patient care;

Reviewed enrollment goals to address projected shortages of general dentists, dental specialists, and dental hygienists;

Reviewed goals and assumptions for the programs and for the facility; Deliberated program adjacencies; Developed and reviewed program plan assumption for each room and space; Sent committee representatives on a site visit to a new school of dentistry at Marquette

University and reviewed lessons learned; Reviewed and massaged square footages by program function to arrive at an acceptable total

project square footage; Reviewed equipment items; Reviewed budget and schedule.

As a result of the activities of the Oversight Committee, this program plan was developed for review and approval by the Board of Regents of the University of Colorado and by the Colorado Commission on Higher Education.

II. Program Information Center for Oral Health 1. Program Description The UCHSC Center for Oral Health will increase access to educational programs and state-of-the-art oral health care for Colorado citizens. Consistent with the educational vision of the new campus at Fitzsimons, the Center for Oral Health will be a collaborative educational and clinical patient treatment facility where students, residents, and faculty from the UCHSC professional schools can learn and provide oral health care together. Sited in the educational zone and immediately accessible to the clinical zone on the Fitzsimons Campus, the Center for Oral Health will provide an environment for high tech clinical education programs. The site will facilitate existing and future collaboration with programs located in the adjacent facilities including The Children’s Hospital and the Nighthorse Campbell Native Health Building. Specialized educational and patient treatment spaces will support the core clinical curricula of the University of Colorado School of Dentistry’s Doctor of Dental Surgery (DDS) program, the Bachelor of Science in Dental Hygiene (DH) program, and advanced education programs in the dental specialties. The School of Dentistry The University of Colorado School of Dentistry (UCSD) is the only dental school in the state of Colorado and the entire Rocky Mountain Region. A relatively young dental school by national standards, its first dental class graduated just 25 years ago. Its mission, as an integral part of the University of Colorado Health Sciences Center and University of Colorado, is to provide programs of excellence in education, research, patient care, and community service. There is a continuous focus on assessment of program outcomes to ensure that the School of Dentistry is satisfying its mission. Today, the University of Colorado School of Dentistry is nationally recognized as an excellent school of dentistry. The School of Dentistry has a DDS program, a dental hygiene program, a general practice residency, and a pediatric dental specialty program which is a collaborative program with its affiliate, The Children’s Hospital. The UCSD is in the process of developing another post-doctoral certificate-granting programs in the dental specialty of orthodontics and dentofacial orthopedics. The dental program is one of the most selective of the 55 US dental schools with a program acceptance rate of 5%, and a ratio of applicants to first year enrollments of 20.9. The dental class entering the school each year consistently ranks in the top three of US dental schools for entering average GPA, with a 3.6. Like the dental program, the dental hygiene program is also nationally recognized. The graduating class of 1999 ranked #1 of 215 US dental hygiene programs for student performance on the dental hygiene national board examination while the graduating class of 2001 ranked #4 of 230 programs.

Following a recent American Dental Association (ADA) Accreditation Site Visit, the visiting committee made no recommendations for program improvement and offered 21 commendations. The commendations recognized the quality of UCSD students and faculty, highlighted the strength of the dental and dental hygiene curricula, and acknowledged the recent growth and breadth of the research program. This puts the University of Colorado School of Dentistry into an elite group of dental schools that have recently had a comprehensive program review by a team of dentist peers (from other US schools of dentistry and the ADA staff), and received zero recommendations and numerous commendations). To date, the school has had 649 DDS graduates and 428 DH graduates. The overwhelming majority of UCSD graduates have remained in Colorado to practice. The current dental and dental hygiene class sizes include 40 dental students and 20 dental hygiene students, respectively. To meet expanding population needs and projected dental workforce shortages, the 20-year student headcount enrollment projections anticipate increases in dental class size to 50 students and increases in the dental hygiene class to 30 students. In accordance with American Dental Association requirements for the accreditation of dental programs, the Center for Oral Health will house the clinical facilities required for completion of required patient care experiences, clinical support facilities, laboratories where students learn to use dental equipment and prepare patient care materials, and a dental simulation clinic. Dental of Dental Surgery Curriculum The Doctor of Dental Surgery Curriculum The dental curriculum is a four-year, year-round program designed to integrate the basic, clinical and behavioral sciences. The first year (DS 1 year) of the program combines didactic courses (an intensive basic sciences core) with high technology simulated patient care experiences designed to teach the psychomotor skills needed to provide dental treatment. Dental patient simulation allows students to perform patient care on dental manikins in a learning environment that closely simulates a dental office, with the intent of minimizing risk to live patients and improving the confidence and skill of dental student care providers. With the opening of the Patient Simulation Clinic in 2001, the UCSD became one of only twelve US dental schools that have a patient simulation clinic. In anticipation of the move to Fitzsimons, this high technology facility was built with portability in mind. It has individual student workstations (complete with monitors and laptop computer connections) that mimic dental patient operatories found in dental offices and a multimedia instructor pod with the capacity to teach students through real-time video camera demonstrations, projected slides, and computer generated images. When students are ready to progress from providing simulated patient care to actual patient care, they are assigned to clinical Comprehensive Care teams, set up to function like large group dental practices. This occurs in the DS 2 year when students begin to perform basic diagnostic, periodontal, operative, and prosthetic procedures in the clinic. As DS 3 students, they work primarily in the main comprehensive care clinic, learn more advanced dental procedures, and do

three to six-week rotations in specialty clinics (surgery, pediatrics, geriatrics, care for developmentally challenged, and emergency care). DS 4 students also rotate through the school’s specialty clinics when they are not treating patients in community-based clinics outside of the School of Dentistry. The dental curriculum prepares dental students for community-based care to diverse populations. The community-based clinical care program is called the Advanced Clinical Training Services (ACTS) Program. The ACTS Program is a collaborative effort between the School of Dentistry, urban and rural community-based preceptors, and underserved Colorado residents. Students provide care in a variety of settings throughout the state, increasing access to oral health care for underserved individuals. Some community health clinic sites contain medical clinics, and dental students treat patients together with medical, pharmacy, and nursing students. While providing extensive clinical training opportunities for dental students, the ACTS Program provides a significant amount of dental care to several disadvantaged populations. Annually, the estimated market value of dental care services provided to the citizens of Colorado by dental students in the ACTS program is just over three million dollars. The ACTS Program can appropriately be labeled a win-win experience for students and patients. An analysis of program outcomes to date suggests that in addition to enhancing clinical skills, the ACTS program enhances students' social experience base and increases their cultural sensitivity to a variety of populations. In summary, the dental curriculum includes: Core basic science courses and electives Didactic (lecture-based) preclinical courses Preclinical laboratory courses Transitional clinical experiences Clinical patient care experiences within the School of Dentistry Didactic courses supporting clinical patient care Clinical and other electives Clinical patient care experiences in the community

The didactic courses of the dental curriculum will be ultimately taught in the Education Facility 1B and Education Facility II; the patient simulation clinic, main clinic, and laboratory will be in the Center for Oral Health. The table below illustrates the types of courses dental students take in each of the four years, and where the courses will be located at Fitzsimons.

Dental Hygiene Curriculum The dental hygiene (DH) curriculum is a two-year program culminating in the awarding of the Bachelor of Science in Dental Hygiene. Students enter into the program after completing two years of coursework at a four-year college or community college. DH 1 and DH 2 students take didactic courses in lecture halls, use the simulation clinic for learning technical procedures, and provide patient treatment in the various student clinics. History, Role and Mission, Unique Programs History: While providing access to high quality dental education programs, the provision of direct patient care to citizens of Colorado, including disadvantaged, underserved, and medically compromised patients has been a hallmark of the School’s mission since its inception. During core clinical activities in school-based clinics as well as in community-based health centers, students provide care to geriatric, pediatric, mentally/physically challenged, and HIV-infected individuals. The new Center for Oral Health will allow the School of Dentistry to educate 21st Century oral health providers and to meet a greater number and scope of patient care needs than the current facility allows. Role and Mission: Dental schools play not only a key role in educating future practitioners, but they are also a key component in the community safety net that provides access to care for needy citizens in their communities. In the first-ever US Surgeon General Report on Oral Health, the US Surgeon General reports that 108 million Americans do not have private dental insurance. For every adult 19 year or older without medical insurance, there are three without dental insurance. Further, employed adults lose more than 164 million hours of work each year due to dental disease or dental visits. Dental schools are helping this segment of the population access oral health care

services. More than two-thirds of the patients visiting dental school clinics are members of families whose annual incomes are $15, 000 or less. About one-half of these patients are on Medicare or Medicaid, while more than a third have no insurance coverage or government assistance program to help pay for their dental care. The mission of the University of Colorado School of Dentistry, as an integral part of the University of Colorado Health Sciences Center, is to provide programs of excellence in teaching, research, patient care, and community/professional service for Colorado and the nation. The ability to serve the State of Colorado pursuant to this mission has been greatly enhanced by the opportunity to be a part of the 21st century academic health center being built on the Fitzsimons Campus. The School of Dentistry’s educational programs have as their primary purpose the education of graduates who possess the knowledge, skills, and values to begin the practice of dentistry, the dental specialties, and dental hygiene as ethical practitioners committed to lifelong learning and dedicated to fulfilling the public’s trust. The school’s patient care programs provide comprehensive care with a focus on prevention and wellness, while serving as a clinical education resource for dental, dental hygiene, and post-doctoral dental students.

New strategic initiatives of the School specific to its educational mission include: Strengthening educational programs to develop critical thinkers and lifelong learners through

the increased use of technology and innovative teaching methodologies to include examples such as a state-of-the-art patient simulation clinic and the addition of online and distance education courses.

Increasing class sizes for the dental and dental hygiene educational programs to respond to growing manpower shortages in several Colorado counties.

Creating advanced education programs in the dental specialties to address dental workforce shortages projected in Colorado.

In addition to a planning focus on education, the School of Dentistry’s Strategic Plan 2000 also details a commitment to increasing research and community service activities. Today, dental faculty members participate in collaborative research activities at the UCHSC and within the University of Colorado system; all research faculty have joint appointments in the School of Dentistry as well as in either the School of Medicine or within the University of Colorado Boulder School of Engineering. The historical strengths of the School’s current community outreach and service learning programs will be built upon to further increase access to care for the disadvantaged within clinics of the Center for Oral Health, as well as, in community-based sites around the state. Unique Programs: Since 1986, the Advanced Clinical Training and Service (ACTS) Program has afforded dental students an opportunity to participate in extensive community-based clinical training experiences while providing needed dental services to under represented minority and low-income communities in Colorado. This program is the national model for dental school service to its surrounding community. Appropriately called a “transition to practice,” the current ACTS Program consists of a broad range of community-based clinical experiences throughout the dental curriculum. The most significant of these experiences is in the senior year. The quantity

and breadth of such experiences also allow students to sample practice opportunities in rural Colorado and in urban public health settings, while beginning the important transition from dental school to dental practice. The University of Colorado School of Dentistry currently has two major programs on the Fitzsimons campus and looks forward to the relocation of the remaining educational and research programs in the next few years. The University of Colorado School of Dentistry Biomaterials Research Center and the School of Dentistry’s Healthy Smiles Clinic (treating the oral health needs of disadvantaged pediatric dental patients) are presently housed on the Fitzsimons campus. 3. Program Needs and Trends Nationwide studies reveal that dental treatment has resulted in healthier adults and children, and that the percentage of the population visiting a dentist has actually increased during the last three decades. According to the American Dental Education Association (formally the American Association of Dental Schools), the percentage of children without tooth decay increased to 55% in FY 1994, as compared to 50% in FY 1987 and 37% in FY 1980. In adults, the number of teeth present has increased, and the percentage of adults needing dentures has declined. Much of this improved dental health is a result of better technology and materials, along with an increased visitation rate among patients. New procedures along with changes in the types of dental procedures performed have resulted in more adults retaining more teeth longer. Approximately 60% of the population went to the dentist in FY 1993, as compared with 50% in the 1970s. The trends toward increasing dental visits are expected to continue. However, significant disparities exist in the oral health of the nation’s citizens. Oral disease disproportionately affects the poor of all ages, particularly children and the elderly. Today, while demand for dental services has been increasing, the supply of dentists is decreasing. The primary reason for this is that retiring dentists (both general dentists and specialists) are outpacing graduating dentists and specialty practitioners. Nationwide, there is a limited capability within the current dental education infrastructure to accommodate much of an enrollment increase in DDS programs. For these reasons, the state of Maryland has approved $114 million in funding to allow the University of Maryland to begin building a more modern expanded dental school, and the state of Nevada has just opened a dental school in Las Vegas. Approximately five years ago, a new dental school opened in south Florida. Along with the need for additional general dentistry spaces in dental schools comes the need for additional post-doctoral program spaces. Within the last several years, nearly 49 percent of dental school seniors applied to one or more programs for advanced postdoctoral dental education (either residencies in advanced general dentistry or advanced education programs in the dental specialties). One obvious reason for this is that the demand for dental specialists is also increasing. A dental manpower shortage to include both general dentists and specialists is projected to exist in Colorado in the next decade. A recent report, The Colorado Report: A Study of the

Relationship Between The University of Colorado School of Dentistry and the Supply of Dentists in Colorado - May 2001, written by Dr. Eric S. Solomon, Executive Director of the Texas A&M University System Health Science Center Institutional Research Office of Dallas, Texas, summarizes Colorado’s problems of maldistribution of dentists and of projected future shortages. Dental manpower trends change very slowly. A lengthy educational process is required to educate a dentist, and additional years are required for the dentist to establish a practice. This heightens the need for sound planning and early intervention when addressing dental manpower needs. Enrollment changes at the University of Colorado School of Dentistry are a solution for the projected dental manpower decrease in Colorado. The new Center for Oral Health will allow the school to increase the class sizes in the dental and dental hygiene programs and to expand dental specialty programs to provide a predictable source of general dentists and dental specialists for the state in order to offset the anticipated decrease in the supply of dentists. There is a definite correlation between the supply of dentists and the level of service to the community. A relative fall in the supply of dentists (signaled by falling dentist-to-population ratios) will generally result in reduced access to dental services. This reduced access can be seen in longer waiting times for appointments, higher costs of services, and a geographic shift by dental practitioners to more “desirable” areas (a shift from rural and central city areas to the suburbs). The net result of these shifts is an increased proportion of the population that is underserved in their dental needs. As mentioned above, today significant disparities already exist in the oral health of Colorado citizens. While the reasons for such disparities are often complex, socioeconomic factors such as the lack of resources to pay for dental services and the difficulty of individuals getting time off from work for dental visits, are often cited as barriers to care. In summary, the demand for dental services has been steadily increasing while the supply of dentists has been decreasing. Enrollment projections for the School of Dentistry at the University of Colorado Health Sciences Center on the Fitzsimons campus reflect this increased demand and need for more dentists. The current facility was built for class sizes of 25 dental students and 15 hygiene students. Frequently the opportunity for students to learn is hindered by the inadequate facilities in the building. At Fitzsimons, there will be sufficient space to accommodate increase education program growth and to meet the anticipated increases in patient care demands.

The constraints of the current clinical facility not only impact the ability of the school to meet the manpower needs of the state, but they have also limited the ability of the dental students to provide patient care. The school operates with available students rotating into the clinic, due to lack of dental operatories. Clinic sessions consistently have more students scheduled than available dental operatories, counting on a percentage of late patients, cancellations, and no shows. The limitation is further illustrated by reviewing the number of recorded patient visits. Between 1988 – 1992, the range of annual recorded patient visits at the school hovered between 15,273 and 17,500. With a change in clinical operational structure in 1994, patient visits steadily increased to an annual level of approximately 22,734 by 1999. The number of recorded patient visits jumped to 33,750 in 2000, but much of the increase was due to a change in the clinic information management system that accounted for visits that did not have fees associated with treatment. The number of visits has remained stable since that time, with the most recent fiscal year 2001-02 at 32,847. The insufficient number of dental operatories also has challenged the student providers by not allowing them to operate out of multiple operatories at the latter stages of their clinical development as is typical of private practitioners in the community. In addition to the enrolled students, the School of Dentistry includes a faculty consisting of 55 Clinic Research Faculty, 13 Fellows and Professional Research Assistants, and 235 Part Time Faculty. 4. Relation to Academic or Institutional Strategic Plans School of Dentistry The University of Colorado School of Dentistry “2000 Strategic Plan” is considered a dynamic document and will be revised as needed to insure that the mission and goals it details, will remain consistent with both the UCHSC and the University of Colorado.

Mission of the UCSD

The mission of the University of Colorado School of Dentistry, as an integral part of the University of Colorado Health Sciences Center and University, is to provide programs of excellence in teaching, research, patient care, and community/professional service for Colorado and the nation.

Goals that Support the UCSD Mission 1. The School will provide contemporary educational programs, the primary purpose of which

will be to prepare dental and dental hygiene graduates who possess the knowledge, skills, and values to begin the practice of general dentistry and dental hygiene, respectively.

2. The School will increase research activities to contribute to the discovery and dissemination of new knowledge for society’s benefit and to be consistent with the mission of the Health Sciences Center and University.

3. The School’s patient care activities will provide high quality comprehensive patient care with an emphasis on prevention and wellness, while serving as a clinical education resource for students.

4. The School’s community and professional service programs will serve Colorado and the nation by providing primary care to underserved populations, and professional service programs to enhance the external dental community and profession.

5. The School will respect and preserve its human resources by recruiting and retaining high quality faculty, staff and students, by insuring that alumni remain a School resource, and by creating a humanistic atmosphere where diversity and multiculturalism are valued.

Master Plan The Institutional Master Plan that details the development of the University of Colorado Health Sciences Center at Fitzsimons describes the need for a specifically designated clinical dental education facility. As mentioned earlier, this facility is unique because while its primary purpose is to provide access to clinical education for students pursuing professional education in dentistry, it also provides patients and communities throughout the State of Colorado with access to reduced-fee dental care. The history, relationships, and consistency to the UCHSC Master Plan is addressed in further detail in section V. Relation to Master Plan/Other Projects. Information Technology Strategic Plan The University of Colorado Health Sciences Center Information Technology Council developed a campus-wide Strategic Plan for information technology with goals and objectives in 1998. The School of Dentistry has implemented numerous technologies consistent with the plan, including development of on-line courses, a simulation clinic with highly advanced instructional AV technology, renovated student computer lab, and wireless network access for student laptop computers in instructional areas. The Center for Oral Health will provide the facilities and infrastructure to support the goals of the Strategic Plan. The Center’s new educational spaces, simulation clinic, community spaces,

and clinics will have state-of-the-art technology infrastructure to enhance the students’ educational experience. This infrastructure will allow for access to educational materials through student laptop computers anywhere in and around the facility, as well as from remote access. The technology will support different learning modalities such as expanded on-line course instruction, distance learning for students, and community professionals. Community partnerships will be enhanced through the development of collaborative core curriculum with other schools in the University and across the nation and further development of the School’s Virtual Oral Health Center for patients and professionals. The infrastructure will facilitate the School’s implementation of an electronic patient dental record. The electronic record will be invaluable in supporting student clinical education, quality of patient care, multi-institutional health care collaborations, clinical research, and business operations of the School. Opportunities will exist for research and development of advanced technology in dental care including the development of computerized robotics and laser technology as alternatives to traditional dental handpieces and the use of intraoral microchips to monitor bacteria and prevent oral disease. 5. Relation to Other Programs or Agencies Because of the considerable amount of uncompensated care that the current ACTS program provides to the communities in the state of Colorado, the UCSD enjoys the strong political support of the local dental community, state public health agencies, and other community partners. The UCSD has received recognition for its community service from many foundations, and has received financial support from several to include the Rose Community Foundation and the Caring for Colorado Foundation. Additionally, UCSD has developed relationships with key state policymakers and professional organizations such as the Governor’s Office, the Colorado Dental Association, the Society of American Indian Dentists, the Colorado Minority Health Forum, and the Oyate UCHSC minority student organization. Seventeen years ago, UCSD had a vision to better serve under represented minorities and low-income populations by taking dental care providers to the communities in need. Today, dental students in the ACTS Program currently make up nearly 27% of Colorado’s public health workforce dedicated to treating oral health needs of disadvantaged citizens. Using formulae developed by the Colorado Community Health Network (CCHN), ACTS students serving on during their senior year currently provide services equivalent to 13 full-time community health dentists. Federal grants - The past four years have witnessed significant development and sophistication in not only the education programs of the UCSD, but also in the quality and breadth of its research programs and in the level of extramural research support – both Federal and other. Of great relevance to this grant application is the expansion of the basic science research efforts of the School of Dentistry. As part of the Strategic Plan 2000, a major objective of the school has been to establish a critical mass of sponsored research activity. Over the past three years, the school has recruited many teams of well-respected basic science faculty members to add to their core research faculty pursuing studies in craniofacial development, oral pathology, pain mechanisms and biomaterials. These School of Dentistry researchers have joint appointments in the UCHSC

School of Medicine, UCHSC Graduate School, or the School of Engineering on the Boulder campus, and participate in collaborative research projects that enhance the productivity of the individuals, the School of Dentistry, and the entire UCHSC research enterprise. In just three years, sponsored research at the UCSD has increased from less than $1 million to more than $4 million. It is expected that in the next academic year, sponsored research will exceed $5 million. The research program at UCSD is consistent with the mission of the National Institute of Dental and Craniofacial Research (NIDCR) “to promote health, prevent diseases and conditions, and to develop new diagnostic and therapeutics.” A recent conference, cosponsored by the NIDCR and the American Dental Association (ADA), was devoted to developing “a long-term initiative to build on the strategic position of schools of dentistry as national resources to conduct cutting edge science to translate science into practice.” Dental schools represent a nexus of teaching, research, and patient care focusing on dental and craniofacial issues. 6. Existing Programmatic/Operational Deficiencies The Center for Oral Health will increase the capacity of the School of Dentistry to satisfy its mission – to provide programs of excellence in education, research, and service. Today, the 9th Avenue campus facility can no longer provide adequate facilities to meet the demands of Colorado citizens for educational programs and direct patient care services in dentistry for the following reasons: Number of clinical operatories is inadequate because the current facility was built to

accommodate a dental class size of 25 and a dental hygiene class of 15 students. While there have been some remodeling projects over the years on the Ninth Avenue

campus, the sizes of didactic and preclinical educational spaces are inadequate to teach current curricula.

The 9th Avenue campus facility was not designed to house advance dental education

specialty clinic areas. The technological infrastructure is not present to support increases in distance education. All support areas such as sterile supply, waiting areas, records storage, radiology, offices, and

student areas are grossly inadequate for both the current state-of-the-art practices and for the sizes of the student body, faculty, and administration.

7. Program Alternatives Alternatives to the construction of a new Center for Oral Health include, (1) Operational changes such as limiting enrollment, (2) Curricula changes such as increasing the proportion of time spent in off campus clinical settings , and (3) Minor facility modifications.

Operational changes such as limiting enrollment The alternative of limiting enrollment is at cross purposes to the objective of expanding the number of trained dentists and dental hygienists necessary to care for Colorado’s expanding population at a time when the number of retiring dentists is exceeding the number of trained graduates. Curricula changes such as increasing the proportion of time spent in off campus clinical settings The current program already offers a considerable exposure to community based clinical experiences. Additional off-campus time has been discussed and is currently felt to compromise the amount of consistent supervisory time found in on-campus teaching. Minor facility modifications Any area that is expanded through the modification and renovation of other space in the current School of Dentistry would necessarily decrease the amount of space for those other areas. The 26 year old facility is outgrown by the current dental programs in all areas. No space can be freed up to address any overcrowded and stressed areas without further compromising the ability of such an area that is already deficient of sufficient space to function satisfactorily. None of these alternatives allows the Center for Oral Health to meet its needs and objectives. Moreover, the Center for Oral Health is an integral element of the new Fitzsimons campus of the University of Colorado Health Sciences Center and implementing any program compromising modifications is unnecessary when the UCSD will be moving to Fitzsimons anyway.

III. Facilities Needs 1. Total Space Requirements The program requirements of the Center for Oral Health in its proposed new facility translate into 87,560 gross square feet. The major program elements and their assignable and gross square footages are listed below:

The new facility will include a 43% increase in GSF. This is partly attributable to enrollment growth and partly attributable to the correction of many existing spatial inadequacies from operating a program with 2002 needs in a facility that was constructed in 1976. The 1976 building was designed to accommodate a DDS class size of 25 compared to the current class size of 40 and projected class size of 50. The original DH class size was 15 compared to the projected class size of 30. Moreover, graduate classes were originally non existent, and future class size is projected to grow to 22. Current space inadequacy is particularly pronounced in areas of surgery, radiology, sterile supply, waiting, and simulation clinic, and is further exacerbated in all clinic programs due to the sharing of the same main clinic operatories by all of the specialty programs. In the new facility, the specialty clinics of geriatrics and special care, emergency, and orthodontics will have their own dedicated operatories.

A. General Planning Assumptions Planning assumptions used to determine the functional and space requirements for the proposed Center for Oral Health include the following:

The Center for Oral Health will be located at the Fitzsimons campus and will be consistent with the Institutional Master Plan. The Center for Oral Health will be located within the educational zone.

The Center for Oral Health will contain preclinical learning laboratory space, general dental

clinics, specialized dental clinics, business areas to support the dental clinics, appropriate storage areas, sterilization and dispensary facilities, and offices.

Bathrooms, janitor’s closets, telecommunications closets, and trash rooms are not included in

the space program, but are included in the assignable to gross square footage conversion factor. These rooms need to be included in the design process.

Parking for patients will be convenient.

The design of the facility should encourage chance encounters among the student and faculty

of various departments.

The building configuration will facilitate patient follow-up scheduling and payment.

The design and construction of the building will consider future expansion.

The facility will be outfitted with the newest technologies to accommodate the latest changes in the dental sciences curriculum.

The utilities and infrastructure will be based upon the campus wide utilities and infrastructure

requirements and guidelines of the Master Plan. A central vacuum and compressor capabilities are required for dental operatories.

A 54 station simulation laboratory is required for teaching basic clinical skills utilizing

manikins linked to computers.

The three primary components of the preclinic laboratory are: a large benched room with 54 stations, with each station tied into a natural gas line; a production laboratory for professional technicians, and a wet clinic support area for use of dental laboratory equipment.

The geriatric and special care clinic areas may share operatories on a scheduled basis.

Radiology will be a central area to support the other general and specialty clinics.

Main Comprehensive Clinic support spaces must be adjacent to the operatory areas.

Use of natural light is integral to the clinical operation in order for dentists to provide appropriate esthetic dentistry.

Clinic Seminar rooms for problem- and cased-based learning must be designed for maximum

flexibility and with the latest technology to maximize teaching capabilities of faculty clinicians.

The aesthetic appearance of the complex will comply with UCHSC Design Guidelines.

Building design will consider energy life cycle costs.

The facility must anticipate a large daily population utilization. For example, there will be

50 students in each Dental School class (totaling 200 students) and 30 students in the two Dental Hygiene classes (60 students). In addition, there will be at least 35 faculty and 80 clinical, professional, and administrative staff at any one-time attending to the clinics during the facility’s daily operation and as many as 150 patients in operatory chairs or waiting in reception. Therefore, it is quite likely that as many as 450 people will be in the building at the same time.

Interdepartmental Adjacencies/Access The interdepartmental functional relationships are illustrated in the following narrative:

The preclinic technique and clinical support and production laboratories must be adjacent and connected.

The central sterilization and dispensary areas must be contiguous.

The main clinic must be designed to include groups of four to six operatories.

The core clinical support areas must be adjacent to the main comprehensive care clinics. The

core consists of a dispensary, contaminated storage and waste area, equipment storage, small clinic support laboratory, seminar or problem-based learning room, dental assistant workstations, clinic coordinator office, gurney storage, and disposal supply alcoves.

Patient consultation and preventive education rooms must be adjacent to the main clinics.

Urgent care should be provided in a specialty care area.

The lounges and locker rooms will be near each other.

The central record area will be convenient to the front office area.

Facility Layout/Design Considerations: The main reception area must seat 60 visitors. Secondary reception areas must seat 12

visitors.

All operatories in the main comprehensive care clinic will be designed to balance patient privacy with supervisory concerns.

Clinical Area Design Considerations: The circulation paths for the dental, dental hygiene students, faculty, and patients will

accommodate high traffic volume. Hallways associated with surgical and urgent care areas will accommodate gurneys.

The main comprehensive care clinic will be a generally open room with operatories divided

by modular partitions, approximately five feet high. The operatories associated with the main comprehensive care clinic will be organized to

optimize curricular efficiency and maintain the integrity of a simulated private practice situation.

B. Planning Assumptions Relative to Specific Selected Program Areas The Comprehensive Care Clinic The Main Comprehensive Care Clinic is the clinic in which students learn to care for patients. The curriculum is designed after a general practice model. The philosophy of the model is to provide quality comprehensive general dentistry without fragmentation of care. Care groups of six students work with a group of full-time and part-time faculty specialists. Students learn diagnostic skills, develop treatment plans, and learn periodontics, operative dentistry, prosthodontics, and occlusion. This clinic will contain four major group clinics, with each clinic comprised of subgroupings of operatories in groups of six operatories each. Students are primarily taught in groups of six to maximize faculty/student interaction. Ideally, there is one operatory for each DDS student in two program years and one for every two DH students. However, other assignments and scheduling opportunities allow for less than one operatory per student. Thus, 84 stations will be adequate for the 160 students in these two classes. The clinic should be designed for expansion opportunities should the class size increase further. Each operatory should be fully functional and designed for four-handed dentistry techniques required by accreditation standards. Therefore, professional dental assistants along with DS 1 and DS 2 students, will be assisting their DS 3 and DS 4 colleagues.

Clinic Support Lab The Clinical Support Lab will be a small laboratory in which students are able to perform laboratory activities and fabricate dental appliances in support of their treatment of dental patients. The room will contain plaster bins, large sinks, model trimmers, lathes, sandblasters, vacuum investors, polishers, electric lab handpieces, and other related dental lab equipment. Dispensary The dispensary will be an area from which dental instruments, equipment, supplies, and materials are transferred to the dental operatories. The dispensaries will be 140 asf. Clinic Coordinator Offices Each clinic has a coordinator that monitors and manages each student’s practice of patients. They monitor timing of treatment, quality of care indicators, patient financial account status, students’ clinical experiences, and act as a liaison between the student, faculty, and patient. These staff are essential to the clinic’s operation and are located adjacent to the main clinics. Their offices will be 120 asf each, and may be combined into a shared office suite if centrally located in the main clinic. Front Desk and Cashier Stations Registration and cashiering functions will be conveniently located for service upon arrival and departure from the facility. Each station will be 50 asf and include a front unit that accommodates a computer with consideration for patient privacy and a back storage unit for supplies and reference material. Reception Waiting Area At the main entrance, a waiting area will seat 60 patients comfortably. This area will be approximately 400 asf. Patient Education Resource Room An area located adjacent to the patient reception area designed to serve as a place for patients to educate themselves on dental health. It will also serve the dual purpose of an area where they can collaborate with dental students, staff, and faculty in the education process. These rooms will be equipped with computers and written educational materials and seat up to eight people. The room will be approximately 160 asf. Patient Consultation Rooms In addition to patient education rooms, areas are needed where dental students can present their patients with their diagnosis and treatment alternatives. Privacy is often a concern with patients

as their medical background and care is discussed. These small rooms will allow students and their patients to conduct confidential discussions. These rooms are also available to students after hours as study areas. The rooms will be approximately 80 asf each. Clinic Seminar Room The Clinic Seminar Rooms will be located within, or immediately adjacent to, the main clinic. These rooms will accommodate the students, staff, and faculty that make up one group practice. Groups will hold group meetings and discussions before, during, and after patient care sessions. They will also be frequently utilized for problem- and case-based learning seminars that build upon current and past clinical cases. Large rooms will be provided to accommodate a group of students and appropriate video technology for problem-based, or case-based learning. These rooms will be 600 asf apiece. Community Spaces The Community Spaces will serve as the hub of students’ education. Initially, these spaces will serve as the home for learning of small groups of dental and dental hygiene students, with a long range plan to house interdisciplinary student groups as other health professional programs transition to Fitzsimons. These spaces will be near large and small group learning areas. They will be designed to facilitate learning between students, with and without faculty participation. Faculty Offices The School of Dentistry faculty teach clinical education and need to be easily accessible to their students during clinic hours. It is essential that many of these faculty be located in the facility and near the main comprehensive care clinic. The faculty spend most days working directly with their students in the comprehensive clinics and need to have office space to meet with the students privately to discuss their progress and assignments. Each office will be approximately 100 asf. Radiology All students need adequate training in radiology, and a centralized radiology area will reduce costs and facilitate equipment maintenance. While radiological services currently utilize film, thus necessitating a dark room, in the near future, digital techniques will be utilized. X-ray chairs are needed for this area, which will be equipped with standard and panoramic radiographic equipment. Areas for intraoral films, panoramic cephalometric imaging, complex imaging, CAT, and digitizing areas will be included. Orthodontics Clinic There is a high demand for orthodontic specialists. The program plan includes 48 chairs needed in this area which has specialized materials and equipment requirements.

Geriatric and Special Care Clinics The UCHSC traditionally has provided care to underserved populations. Within this group of patients are the elderly with their own unique needs and individuals who need special care because of either physical and mental disabilities. These populations will be better served in an area separate from the main clinic. The two clinics can share the same area on a scheduled basis. This area includes 6 operatories and support areas. C. Planning Assumptions By Systems

Operational Support System Assumptions The facility contains various functions, all of which support health sciences education. Specific functions within the building have operational requirements outlined in the following narrative. Access and Security

There will be different levels of access to the Center for Oral Health:

Open access: Waiting areas, student areas. Controlled access: Clinic operatories for main comprehensive care clinics and

specialty clinics, including surgery, geriatric and special care, pediatric, and orthodontic areas.

Secured: Storage and dispensary areas.

A controlled key/card system will be provided at all building entry points.

The mini-dispensary areas, including cabinets and storage lockers, will be locked in the areas adjacent to the clinics.

The Center for Oral Health will be open 7 days a week on a 24-hour-a-day basis to

students and faculty members through controlled key/card access with particular ease of access to the simulation clinic and the patient education areas as dual use student study rooms. The main entrance will be open during daily business hours for patients. Certain areas will require extra security, including dispensary, storage, information technology, and faculty offices.

Alarm systems will be installed to protect areas from unauthorized users and for the

safekeeping of expensive equipment.

Environmental Control Systems Sterilization areas must be equipped with appropriate autoclaves and adequate

ventilation. Central computing areas have specialized environmental control needs to maintain

system integrity.

Information Technology

This complex will have access to a centralized computer network/fiber optics system.

All offices must be equipped with wiring for computer and telephone usage.

All learning spaces will be equipped to utilize a variety of educational technologies. These “smart” learning areas will be capable of using audio/video and computer graphics.

Electrical Systems and Lighting

Flexible electrical and communication systems will be necessary in the patient care areas and in the lecture hall to support a range of functional requirements.

Service/Trash/Materials Management

Trash and hazardous waste will be held in a designated trash room and removed by environmental services personnel or health and safety personnel.

Contaminated waste and storage rooms will be designed for each clinic area. A

segregated waste management system is needed for the disposal of hazardous waste.

An elevator will be required for vertical movement of materials. Mechanical Systems

The building’s mechanical systems (compressor and vacuum) must meet optimum clinical operation efficiency and building standards.

Materials Management

A loading dock must be constructed adjacent to the rear of the building with adequate entrance to the first floor and a special materials elevator with controlled access service to the various areas of the building.

The loading dock must be sufficient for the delivery of supplies and equipment and the

removal of waste.

Unique or Special Features The Center for Oral Health is unique among many education facilities because much of its education is provided in a clinical setting in environments with both simulated patients and with real patients. Almost all of the facility’s program represents clinical space, simulation space, and support functions necessary for the provision of clinic services to the patients receiving dental care in this facility. The Simulation Clinic is a unique and highly technical program element that includes 54 operatories with dental manikins. This is where dental students have their first clinical

encounters and practice a variety of dental procedures and techniques on individual manikins. They are close supervised by faculty and receive both group and individual instruction. Students also often return to the Simulation Clinic after hours to practice their skills. The clinics in the Center for Oral Health include the Main Comprehensive Care Clinic for the teaching and provision of general dentistry and dental hygiene as well as specialized clinics for Orthodontics, Emergency Dental Care, Pediatric Dentistry, Geriatric Dentistry, Special Care (for physically handicapped patients), and Surgery. These clinics include open operatories where a number of operatories are located in the same room separated by partial partitions, closed operatories in private rooms, and operatories with radiography equipment. The various support facilities unique to this facility and not normally found in education facilities include administrative areas (waiting, reception, business office, and records areas) and clinic support areas (sterile supply, clinic support laboratories, contaminated waste storage, supply dispensary, and radiology dark rooms and viewing rooms). 3. Health, Life Safety, and Code Issues Not only does the current School of Dentistry at UCHSC’s 9th Avenue campus have many programmatic shortcomings that have evolved over its 26 year history, but the physical facility also has a number of deficiencies. Most noteworthy of these are projects for Elevator Upgrade ($105,200), Replace AHU Fan/Motor Drive ($74,500), and Upgrade Temperature Control ($288,000). Furthermore, the UCHSC Facility Audit Program assessed the building’s major components to have the following deficiency ratings and costs:

4. Site Requirements Site The new Center for Oral Health will be constructed on the Fitzsimons Campus on a site within the education zone. At the south end of the Education Commons, this facility is proximate to the center of the campus. In this location, it is convenient to other educational facilities, to the library, to the clinical zone, to the research zone, and to parking and access by the public. The proposed structure will be adjacent to Education Facility 1B where basic science courses will be taught and diagonally across the commons from Education Facility II where other didactic facilities will be available to the dental students. The Center’s location not only accommodates the building footprint, but also allows for minor expansions of the facility to be accommodated in the future. Parking near the education zone will be located east of the site. The proposed site is relatively flat causing minimal grading issues and allowing for easy accessibility by pedestrians and vehicles. Utilities An extensive evaluation regarding the development of utilities and infrastructure on the Fitzsimons campus has been completed. The evaluation is detailed in the Health Sciences Center Physical Master Plan Update(s). This Center will be served by the existing utility and infrastructure systems as well as be supported by the proposed infrastructure developments planned for the site. A summary of on-site infrastructure and utilities serving the Fitzsimons campus site is provided below:

Potable Water The City of Aurora is the current water supplier for Fitzsimons. Static water pressure at the city water connection is 75 p.s.i. Sanitary Sewer Effluent generated on the Fitzsimons site is conveyed to a City of Aurora sanitary sewer interceptor to the north of the Fitzsimons development. This interceptor line feeds into the Metro Wastewater Reclamation District Treatment Plant. Existing lines vary in size from 4” to 18;” much of the existing system will require replacement since the majority of the system is at least 50 years old. Stormwater Sewer On-site storm drainage is collected in an underground system and is to be discharged into Toll Gate Creek and Sand Creek.

Natural Gas Natural gas is available at the center of the Fitzsimons development but can be extended to serve any campus facility at minimal cost. Electrical The existing system includes both above and below grade lines in combination with pad-mounted transformers. The system is fed from the UCHSC switch located just south of the Central Utility Plant (CUP). Telecommunications As part of the Administrative Building Fitzsimons renovation project, a telecommunications control and switch room facility has been constructed. The communication system will be extended from Building 500 to serve the new facility. Steam and Chilled Water Steam and chilled water are available from the CUP at minimal cost.

Landscape Development There are many large, mature trees in the existing landscape at the Fitzsimons site. The landscape is closely tied with Fitzsimons’ historic heritage. The site was originally developed as a nursery prior to the establishment of the Fitzsimons Army Medical Center in the early 1900’s. Consequently, some of the existing mature trees may be traced to the 1920’s, when more than 200 trees were transplanted from Pike National Forest to the Fitzsimons site as a part of the development of the Fitzsimons property as a treatment center for tuberculosis. Many trees will be protected under the landscape plan which covers the entire campus. 5. Equipment Requirements Equipment required for the Center for Oral Health includes clinical equipment for operatories including dental chairs, dental lights, instrument delivery, stools, curing lights, triturators, and view boxes. Office and meeting room furniture includes chairs, desks, tables, and computers. The Simulation Clinic requires 54 stations with dental manikins. Audiovisual needs are included within the budget for information technology. The Sterile Supply area includes washers and sterilizers which will be purchased and installed through the construction contract. As many pieces of furniture and equipment as possible will be reused from the current facility. None of the moveable equipment items has a cost in excess of $50,000. Categories of needed equipment as well as budgeted costs are presented in the Project Cost Estimate section of this Program Plan.

6. Acquisition of Real Property The University of Colorado’s Health Science Center campus (known as the Fitzsimons campus) is a 217 acre property located in Aurora, Colorado. It is bounded by Colfax Avenue, Peoria Street, Montview Boulevard, and Potomac Street. The UCHSC campus is part of a larger campus including the Fitzsimons Biotech Research Park. The subject of numerous conveyances from the Department of the Army, ownership of the vast majority of the UCHSC campus has been transferred to the University of Colorado. The education zone in which the Center for Oral Health will be sited is among the conveyed parcels now owned by the University of Colorado at the Fitzsimons campus.

IV. Project Description 1. Facility Improvements and Scope of Work To fulfill the vision for the Fitzsimons campus, to accomplish the School of Dentistry’s programmatic missions, and to provide facilities in which to teach the dental, dental hygiene, and advanced education curricula, the Center for Oral Health will contain on-campus clinics and learning laboratories. The clinics in the facility described here include: Main Comprehensive Clinic Oral Surgery Areas Pediatric Dentistry Area Orthodontic Area Endodontic and Prosthodontic Area Emergency Dental Care Area Geriatric Dental Care Area Special Care Area (for developmentally challenged patients) Radiology Areas

As the facility serves essentially as a student dental practice, a number of the support areas for the clinics are required, including reception and waiting areas, equipment sterilization and dispensaries, patient consultation rooms, scheduling areas, seminar rooms, clinical laboratories to make dental materials, storage areas for supplies and contaminated waste, and offices for faculty, clinical coordinators, and assistants. In addition, learning laboratories will be utilized to teach pre-clinical courses in which students learn to manipulate dental materials, make dental appliances and prosthetics, learn about the dental anatomy, and work on patient simulators. The space program for the Center for Oral Health addresses these highlighted needs for each programmed space in the square footage table found below:

Proposed Center for Oral Health

The Center for Oral Health will be a both a programmatic and an esthetic addition to the Education Commons. Located adjacent to both the Nighthorse Campbell Native American building and the Education Facility 1B, it will be a significant anchor to the southwest corner of the commons. The rendering above demonstrates the attractiveness of this clinical education facility, and the site plan below depicts the locus of the building site.

3. Project Cost Estimate(s) Construction The capital construction budget for the construction of Center for Oral Health is $26,500,000. The sources of these cost estimates include professional estimating resources applied to gross square footage projections as well as recent experiences with actual construction costs of the University of Colorado Health Sciences Center with projects under construction or recently constructed on the Fitzsimons campus. This facility includes a considerable amount of clinic space in its program. Utility and plumbing costs associated with clinic space will cause this facility to be more costly than a typical education facility on a campus of higher education. Total $1,290,375 Technology Budget Visual presentation technology, network connectivity, and digital control systems will be integrated into the teaching and conference rooms of the Center for Oral Health. Each meeting room will be equipped with video, audio, fiber, and data network interfaces. Total 278,500 Cost Effects of Project Delay Construction and equipment purchasing costs can increase at rates from 0% to 10% per annum. Until this past year, costs increases have often been in the range of 3% to 5%. A delay in commencing this project will likely result in increased costs of this magnitude. Operating Costs Annual facility operating costs for the Center for Oral Health with a July 2005 occupancy are projected at $640,663 including $289,706 in utility costs and $350,958 in other operating and maintenance costs. Operating costs for the existing School of Dentistry facility will be substantially diminished until an alternate use or owner/occupant is identified at which time those operating costs will disappear. 4. Life Cycle Cost Analysis Life Cycle Cost analysis tables for this project are found in the Appendix. In a 30 year study, they indicate that the current project expenditure of $26,500,000 plus the present value of future operating costs and capital replacements of $20,593,754 compare favorably to a future facility value of $86,125,000. Due to the commitment to develop Fitzsimons as the replacement campus of the 9th Avenue campus by the University of Colorado Health Sciences Center, comparative life cycle cost analyses for alternative off campus sites not owned by the University of Colorado were not considered.

6. Project Schedule Relation to Other Projects and Programs The program of the Center for Oral Health is proceeding in a faster track than was described in the last Master Plan Update due to capital donations. Thus, the Center will be operational on the Fitzsimons campus ahead of several other facilities to which it relates. While the program plan accommodates this scheduling change, there will be a greater desire for interrelated facilities such as the Library and Education Facility II to be constructed at Fitzsimons as soon as possible. As more of these programs are able to relocate to the Fitzsimons campus, they will be better able to share and integrate facilities and to avoid the time and expenses associated with the operation of dual campuses. Urgency The Center for Oral Health is fortunate to have capital donations enabling the construction of this facility to proceed expeditiously. The immediate availability of these funds allows the project to proceed and further enhance the abilities of the University of Colorado to train more dentists to

Proceeding immediately with this project also allows it to proceed on a funded cash basis without additional state funds. Furthermore, the principal urgency is to not endanger any potential loss of particularly generous donations by creating schedule or implementation delays. V. Relation to Master Plan/Other Projects The Master Plan for the future of the University of Colorado Health Sciences Center was developed during an 18-month period in 1997 and 1998. The education vision within the plan was developed by a campus-wide education committee and was designed to accommodate current and future generations of Health Sciences Center students and faculty. The vision emphasizes a strong, integrated campus community that utilizes emerging technologies, faculty, and interprofessional learning opportunities efficiently and effectively. The 1998 Master Plan referenced and described a facility known as the Student Dental Clinic. Subsequent Master Plan Updates referred to the same facility as the Clinical Education Facility. For the past 2 years, it has been known as the Center for Oral Health. During the updates, the size and schedule for the Center have varied, but the need for the clinical education facility has remained and grown in importance. It has always figured prominently among the education facilities in the Transition Schedule found in the Master Plan and its updates. In comparison to the last Master Plan Update, the program plan for the Center for Oral Health includes more square footage and proposes an earlier scheduled implementation. The greater square footage is due to more detailed planning which resulted in this program plan. The schedule acceleration is due to the availability of substantial donations which make this project possible at this time. It should be emphasized that this program plan supports the achievement of the same policy objectives as were described in the Master Plan and its subsequent Updates. As stated above, the programmatic uses of the Center for Oral Health relate to other education facilities in the Master Plan. On the interprofessional Fitzsimons campus, classrooms, lecture halls, and other learning spaces will be utilized by students and faculty from all schools. In addition to the dental programs described in this program plan, the School of Dentistry operates two clinics associated with the University of Colorado Hospital. These are known as the Sands Clinic and the Faculty Clinic. These clinics will move to Fitzsimons when the Hospital moves its inpatient services to Fitzsimons. The relocation of the School of Dentistry from the UCHSC 9th Avenue campus to the UCHSC replacement campus at Fitzsimons along with the relocation of these hospital associated clinics will allow the University of Colorado to vacate the current School of Dentistry facility in accord with its plans to vacate the 9th Avenue campus.

VI. Facilities Alternatives The Facilities Alternatives listed below were considered but were rejected as unacceptable since they would not allow the School of Dentistry or the UCHSC to accomplish its objective of providing sufficient space for its programs, providing appropriate space and location for its programs, and allowing it to ultimately vacate the UCHSC 9th Avenue campus. Alternatives to the construction of a new Center for Oral Health as proposed in this program plan include (1) Expand the School of Dentistry’s current building at the 9th Avenue campus, (2) Lease an off-campus facility near Fitzsimons, and (3) Construct a new facility on a different site within the Fitzsimons campus. None of these alternatives allow the Center for Oral Health to meet its needs and objectives effectively. Expand the School of Dentistry’s current building at the 9th Avenue campus While the current crowded facility was constructed to allow for upward expansion and for conversion of administrative space to clinical space, this alternative is problematic since (1) it would not only necessitate expansion, but also costly and disruptive renovation of the entire facility and (2) the school of Dentistry would not be relocated to the Fitzsimons campus along with the other schools of the Health Sciences Center in accordance with the UCHSC Master Plan. Lease an off-campus facility near Fitzsimons While this alternative may provide sufficient space to meet the needs of the program in a larger facility if an appropriate facility were identified and available for lease, an off campus location would make the Center’s programs inaccessible to many of its constituents and create unnecessary ongoing leasing payments when the University of Colorado already owns an appropriate site at Fitzsimons. Construct a new facility on a different site within the Fitzsimons campus Six sites were evaluated by the Oral Health Oversight Committee. The other 5 sites were deemed less favorable due to substantially higher infrastructure costs, delayed schedule considerations, distance to important amenities such as parking, and concerns over the permanency of location due to adjacent programs which may have significant future expansion requirements.

Other Approvals This Project Request is in conformance with the most recently approved facility Master Plan for the University of Colorado Health Sciences Center entitled Institutional Master Plan – University of Colorado Health Sciences Center and the University of Colorado Hospital Authority and approved in October 1988 by the Board of Regents of the University of Colorado. Subsequent Institutional Master Plan Updates of 1999, 2000, and 2001, respectively, were approved by the Board of Regents of the University of Colorado and submitted to CCHE.

This specific program plan is being submitted to the Board of Regents and is anticipated to be approved on November 14, 2002. Room Utilization Addendum The principal program element in the School of Dentistry for which utilization concerns are relevant is the dental operatories. As indicated earlier, the growth of clinic visits is restricted and capped by the current number of operatories. Moreover, there are insufficient operatories for the number of students to be scheduled for them. Essentially, the operatories function at 100% capacity except for a small number patient appointments that become cancellations and no shows.
