At Centacare we’re all about providing quality and caring support, that lets a person live their life the way they want to. With choice, flexibility and a dedicated team, Centacare helps people to maintain their independence and continue to be active members of the community.

We’ve been making a difference in the lives of seniors for over 30 years and as a not-for-profit, we’re here for our clients.

Our Home Care Packages offer our clients who want to stay at home, access to a range of ongoing personal services, support services and clinical care that help them with their day-to-day activities. Our packages offer flexibility, choice and control to ‘help our clients make the most of everyday’.

Through our team of dedicated, caring and highly trained staff, Centacare provides quality support that is tailored to individual needs. The services we offer include:

Help at Home – a range of options that assist people to remain in their home, while making the most out of life.

Health and Wellbeing – the importance of feeling healthy and happy can’t be underestimated and that’s why we offer a range of options in this area, including allied health services, like occupational therapy, podiatry, physiotherapy, counselling, dietitians, therapy services and assistive technology, such as personal medical alarms.

Carer Support – as carers, family members and friends provide amazing support, but with not enough hours in the day it can be hard to juggle all of their responsibilities. We provide a number of carer support options including, day and overnight respite, dementia specific care and other activities focused on the carer, such as group support sessions and education programs.

Transport – sometimes it’s difficult to maintain independence without adequate transport and that’s why Centacare offers a diverse range of support in this area from cars to buses, so that people can get out and about. This could include, making it to appointments, shopping, attending social outings or visiting Centacare’s respite centres.

HELP AT HOME

Centacare’s help at home services give back choice, control and most importantly confidence in knowing that they can get on with life.

This is done by not only matching services with individual needs, but also giving the client the flexibility in determining how and when their services are delivered.

Centacare staff are highly trained, however it’s more than that, it’s the compassion, understanding and commitment they show their clients that helps set them apart.

Personal CarePersonal care includes general grooming, dressing, bathing, medication monitoring, getting in and out of bed, moving about the house, using the bathroom or the fitting of sensory aids.

Domestic HelpTrained, experienced and caring support workers provide practical assistance around the home, as well as support to make sure the household continues to run smoothly. Activities include, cleaning, washing dishes, laundry, ironing and shopping.

MealsWe provide support with preparing and storing of meals. There is also the opportunity to enjoy a home cooked meal and the company of others at one of Centacare’s day centres, which can accommodate dietary needs.

Home SafetyFeeling safe and secure in your own home is vital not only for peace of mind, but also for maintaining independence. That’s why Centacare offers:

Home Safety AssesmentsThe first option is a visit by a trained field officer to assess the home for key safety issues including; fire safety, electrical safety and falls prevention.

Home MaintenanceThe second option is the home maintenance service that covers garden maintenance to minor repairs, such as replacing light bulbs and changing tap washers.

Home ModificationThe third option involves the home modification service for items such as grab rails and hand rails, ramps, emergency alarms and safety aids.

Specialist Cleaning Service This Centacare program provides a respectful, supportive and non-judgemental service for our clients, who are at the point where they need assistance in working towards living in a safe and habitable home.

HEALTH AND WELLBEING

Feeling healthy and happy are fundamental to enjoying life. This could be as simple as spending time with other people, to having someone around for a chat.

There are also times when professionals can be a great help. This could include allied health, exercise and therapy sessions to counselling and support.

Centacare places great importance on the health and wellbeing of clients and with a diverse range of quality services can offer a holistic approach to a person’s care.

Social Support Having fun and socialising is a great way to maintain health and a sense of wellbeing. With this in mind, Centacare offers a number of social support activities to suit everyone. For some, group activities may be the preferred option, including fun and educational programs at Centacare’s day centres or alternatively guided excursions to various locations. While for others, a home visit, assistance with shopping or help to access social groups and clubs may be the way to go.

Dementia Support Our dementia programs provide support to both clients and carers of people living with dementia. We work with our clients to jointly agree a plan of care, that has the aim of providing them with a high level of holistic care, that is unique and tailored in its approach.

Indigenous and Multi Cultural Support Centacare is at the heart of the community, with a range of services that are culturally enriched and diverse in nature including indigenous programs, as well as catering to people from many different backgrounds that now call Australia home.

Counselling and SupportLife can have its ups and downs and when challenges do present themselves and for whatever reason family and friends are not the answer, it’s reassuring to know that Centacare’s dedicated counselling service is here to help.

Centacare’s professional counsellors offer confidential support in a respectful and sensitive way. They can provide guidance with a particular problem, help with a relationship issue or provide ways to deal with a personal loss.

Allied Health and Therapy ServicesStaying active is an important aspect of remaining fit and healthy. To help achieve this, Centacare has access to a number of allied health professionals, including dietitians, physiotherapists, podiatrists, exercise physiologists and occupational therapists.

Assistive TechnologyTechnology can make life a little easier and safer. That’s why Centacare is able to provide electronic alert devices, such as personal medical alarms that let people know when something unexpected happens.

CARER SUPPORT

Looking after a someone can be a very rewarding experience, but with the pressures of daily life, taking on this responsibility can take its toll. Centacare understands this and so can provide carers with a break, while most importantly giving peace of mind that the person they are caring for is receiving high quality care and support.

Often referred to as respite care, this assistance can be for a few hours, days or for longer periods, depending on need. The location is also flexible, as it’s available at home, in an overnight respite cottage or at a day centre.

Day Centre RespiteCentacare’s day centres offer an opportunity to socialise through a range of fun and educational activities. Our qualified staff and trained volunteers take pride in getting to know their clients and by doing this they are able to offer activities tailored to individual needs. Transport can be provided to and from these centres. Our kitchens are all HACCP certified and provide a range of nutritious meals, catering to all dietary needs.

Cottage RespiteCentacare’s respite cottages are a home away from home, but with the added convenience and security of qualified and caring staff (24 hours a day) and freshly prepared meals. Guests are able to stay overnight, multiple nights or on weekends. Cottage respite also provides an excellent opportunity to socialise with other guests.

In-home RespiteAt times carers just need a short break, such as to attend an appointment or to socialise with friends. Centacare gives carers the chance to make these commitments, through its flexible in-home respite service and the peace of mind in knowing that their loved ones are in the best of care.

Dementia CareLiving with dementia can create many challenges for both the person and their carer. Centacare understands this and the emotional toll on carers. That’s why Centacare offers a dedicated dementia respite service with qualified and caring staff who work with clients to support their strengths and interests. We also understand that dementia can affect younger people and we have programs and activities to support people across a range of ages.

This service focuses on supporting working carers by providing extended hours of support (early morning, late evening and night) at home or at a day centre.

Looking After CarersGiving carers a break is really important, but caring can also be an isolating experience and so it’s important for carers to realise that they’re not alone. To achieve this Centacare’s carer support activities give carers an opportunity to access helpful information as well share their experiences with other like-minded people.

Without adequate transport it becomes much more difficult to remain independent and to be an active member of the community. Centacare’s community transport services are a great way to get out and about, with the knowledge that clients are in safe hands with a driver who is qualified and respectful of individual needs.

With Centacare’s fleet of modern wheelchair accessible buses and cars, our clients are able to:

• attend medical and other appointments

• go to hospital for treatment

• visit a shopping centre

• go on social outings

• see the latest movie

• visit a club

• spend time at a respite centre

• attend a church service.

Getting startedThere are government eligibility criteria for aged care support and this may include a free face-to-face assessment with My Aged Care, an Australian Government service designed to assess your need for support.

To ensure that Centacare is able to offer aged care support, it’s important to mention during the assessment process that Centacare is your preferred provider. You can show the assessor one of our Intake team postcards to ensure they know exactly how to make the referral.

Next StepsOnce aged care support has been approved, Centacare will be in touch to organise a time to meet, so that the approved services can be tailored to a person’s individual needs.

Finally, and most importantly the person will join the Centacare family and receive quality support in a caring and compassionate way.
