WHAT IS THE COMMUNITY RESOURCE COORDINATION GROUP (CRCG)
We are a community group that meets with individuals struggling to meet their needs to create an individualized plan to support their recovery.
We are a partnership between the community and behavioral health service providers to support the success of adults with complex needs.
We recognize the essential value of friends, family and supports in any individual’s recovery.
We affirm that each person knows what they need and can achieve best.

Empower you in your recovery. Recovery is a journey of healing and transformation enabling a person with a mental health problem to live a meaningful life in a community of his or her choice while striving to achieve his or her full potential.
1You, your CRCG partner and others will put the plan into action
2The CRCG coordinator will update CRCG members on how the plan is going
3You will be asked to take a survey about the job the CRCG did helping you
4You, and/or the CRCG partner can ask for another CRCG session by contacting the coordinator
WHAT HAPPENS BEFORE THIS MEETING?
WHAT HAPPENS AT A CRCG MEETING? NEXT STEPS WITH CRCG
You attend along with any desired support person(s) and meet with the group of about 10 agency partners
Each person meets with the group for 35 minutes
You and/or your referral agency person tell a little about your strengths, current challenges, successes, and needs
You and the agency partners will work together to create a CRCG plan to suit your individual needs
Your plan will include tasks, resources and say who is responsible for each task
A partner agency is picked to work with you and help with your plan
After the meeting, you will get a copy of your plan to look over and approve
You will work with a person from a partner agency that you are already involved with to complete a referral packet together.
You need to be aware that if
any abuse or neglect of child, a
person over 65, or a person with a
disability is suspected or disclosed
at the meeting it will be reported
to the Department of Family and
Protective Services.
