Supported Living Services
About CCS Disability Action
CCS Disability Action works to
support:
disabled people
family / whānau of disabled
people
services that support disabled
people.
We are well known as an advocate for disabled people.
An advocate is someone who can
support people to have their say.

CCS Disability Action works to
make the lives of disabled people
better.
We also work with other people to
make this happen like:
Māori
communities
health and social services
government
employers.
Employers are the people who pay
people for their work.

CCS Disability Action has offices
all over New Zealand.
We support about 5 thousand
disabled people and their families.
We support people of all:
ages
cultures.
We work to make life better for
disabled people.

About Supported Living
CCS Disability Action has a service
called Supported Living.
Supported Living can assist you to:
find your own home to live in
plan for the things you need
when you move into your new
home
find people to work with you:
o on your day to day activities in
your home
o in your community.
Our Supported Living service can
also support you with things like:

education or training
getting ready to find work
doing more of the things you like
to do.

How Supported Living works
We have people called Supported Independent Living Coordinators.
A Coordinator is someone who will
work with you and your family to
find out:
what things are important to you
the best ways for us to support
you
what information you need about
Supported Living.

You will talk with your Coordinator about lots of things like:
where you want to live
who you want to live with
what support you need to live in
your own home
what things you might need to
learn so you can live in your
own home.
Your Supported Independent Living
Coordinator can also put you in
touch with other services that may
be able to support you.
It will not cost you any money to
use our Supported Living service.

How to get Supported Living
To get Supported Living you need
to:
be more than 17 years old
fit the Ministry of Health -
Disability Support Services eligibility criteria.
Eligibility criteria says who can
get this service.
You might fit the eligibility criteria if
your disability:
will go for 6 months or longer
makes it hard for you to do things
by yourself.

To find out if you fit the eligibility
criteria you will
need to work
with a Needs Assessment and Service Coordination agency.
The Needs Assessment and
Service Coordination agencies are
also called NASCs.
A NASC agency will:
talk with you to see what support
you need
tell CCS Disability Action what
support you need.
