This is a practical assessment.

The student must submit a video recording of the practical assessment.

There is a single task to perform.

The video must be submitted with this completed form.

Practical Assessment:

1. Submit a video record of yourself AND a user getting on and off the Core Isolators

using the “three points of contact”.

2. The video must include:

a. Introduction. State your name, date, and the following copy: This is the

practical video of Certified Core Isolators Instructor course.

b. Verbal instruction to a user getting on and off using “three points of contact”

method.

c. Demonstrate YOURSELF stepping ON the Core Isolators “three points of

contact” method.

d. Demonstrate YOURSELF stepping OFF the Core Isolators using the “three

points of contact” method.

e. Instructing a USER stepping ON the Core Isolators using the “three points of

contact” method.

f. Instructing a USER stepping OFF the Core Isolators using the “three points

of contact” method.

g. Submit the video and this scanned form via upload FTP server accessed

You can attempt the assessment questions as many times as you like. There is no limit to the

number of times that you can attempt the assessment.
