    A. Biographic Data

    The clients name is V.A., a 46-year old female and a house keeper who is currently

    single and unattached. She originated in Aklan but is now currently residing in 1020 Poblacion,

    Bustos, Bulacan because of her work. She was born on the 21st

    of March, 1963 in Aklan. She

    converses fluently using Visayan Dialect. She can also speak and understand Tagalog.

    Moreover; she is a Filipino citizen and is a Roman Catholic. She was admitted last July 18, 2009

    in Sto. Nino Hospital at Bustos Bulacan. Her attending physician is Dr. Yumul.

    B. Past Health History

    When the client was at the age of 7, she suffered from Chicken pox and mumps

    and she stated that she didnt do anything about it. She has scars on her left foot because of a

    bicycle accident during her high school years. The client is fully immunized with 2 doses of BCG

    (the first one was upon discharge from the hospital, neonate; the second dose was when she

    was 7 years old), DPT (7 months old), MMR (9 months old), Anti-Polio (3 months old) and Anti-

    Hepa vaccines. Her immunizations were administered in the hospital. She never had any foreign

    travels yet but she already had visited some local spots like Las Pinas and Boracay.

    C. Family History

    The client mentioned that as of now, no other member of the family have

    diabetes mellitus. (The health care providers failed to get the history of the family or the

    background of the family regarding the illness of the patient).

    D. Health Perception and Health Management Pattern

    The client perceives health as being strong and no diseases/ illness felt. Malakas,walang

    masyadong nararamdaman, malinis ang katawan at mabagsik, as verbalized by the client when

    asked about her perception of a healthy person. She rank her health 6 (1 as the lowest,10 as

    the highest). Lampa kasi ako kumilos at dahil na rin sa condition ko ngayon, she said. For her,

    to become healthy, a person must eat a healthy and balanced diet. She also mentioned that she

    tried to make her diet balanced in order to avoid complications in her present problem (DM),

    but she failed to do it.

    According to the client, she determined her problem in Diabetes Miletus 10

    years ago but she didnt do anything about this matter. Nagpachek-up ako dati kasi ang tagal

    na ng sugat ko, di pa rin gumagaling tapos sabi sa akin diabetic daw ako. She also stated that

    she experienced some minor illnesses and sicknesses in the past but she did not go to hospital

    or any health care provider for check-up. Ako kasi kapag kaya ko ang sakit, di ako agad

    pumupunta sa Ospital, minsan umiinom na lang ako ng mga paracetamol (or other common

    medicines as self prescribed), the client verbalized.

    E. Nutritional metabolic pattern

    Pagkain ng tamang pagkain gaya ng prutas at gulay para sakin ang nutrition

    client verbalized. Mahilig kami sa soft drinks lalo na yung Coke, nakakaapat nga kami nun sa

    isang araw eh. Halos hindi na nga maalis samen yang Coke eh client said. Almost 4L of Coke a
