Program Details Group size refers to the number of employees enrolled with BCBSAZ.

Group must have at least five enrolled employees to be eligible for a bonus payment.

If you sell BCBSAZ dental plans in conjunction with their medical plans, you qualify for the dental incentive.

Operational efficiency incentive eligibility requires submission of employee application data using BCBSAZ’s Enrollment Census template available on azblue.com.

Bonus payments are for BCBSAZ administered plans only. Balanced Funding and Association Health Plans (AHPs) are excluded. (See details on BCBSAZs Balanced Funded Broker Bonus program)

Bonus payments will be issued in March of 2020. Bonus payments will be rescinded for any groups that do not stay in effect for six months from their effective date.

The Balance Funded bonus program is for new groups with October 1, 2019, through January 15, 2020 effective dates.

Program Details Group must have at least 15 or more enrolled employees.

Sold group counts are based off of the writing agent, not the broker agency.

Existing groups moving to Balanced Funding from another BCBSAZ plan DO NOT count for this bonus.

If the group terms within six months of their effective date, BCBSAZ will recoup the bonus payment. (In other words, the groups have to stay in effect for at least six months.)

Broker must submit employee applications using BCBSAZ’s excel Balanced Funding Enrollment Census template located on azblue.com.

Bonus payment will be issued in March of 2020.

Broker Bonus Program details

To qualify for the Bright Health broker bonus program, you must be a contracted and appointed agent, as applicable. All bonus payments will be calculated at the individual agent level and will be calculated by the total number of Qualified Health Plan

new members (on and off exchange) based on application date and current status as of 12/2/2019. Payments are made once per member and are not on-going. This applies to new business only with effective dates of 1/1/2020, written between 11/15/2019 and 12/1/2019. Bonuses are payable to a maximum of three dependents per policy. The cut-off date for considering retroactive enrollments will be 12/10/2019. Bonus payments will be paid in December 2019. This program is intended for individual Broker participation only, all others are subject to forfeiture of bonuses paid. How to Qualify

Small Business Opportunity: The Agent of Record must have at least 26 small Group Medical Cases in-force

Specialty Lines of Coverage for which you are Agent of Record and receiving base commissions, with two or more enrolled employees at the end of each semi-annual Bonus Period will be eligible for this bonus. How to Qualify As the Agent of Record, place eight new Lines of Coverage during the bonus period and finish with 20 in-force Lines of Coverage and 350 enrolled employees, OR place two new Lines of Coverage during the bonus period and finish with 10 in-force Lines of Coverage and $260,000 in Aggregate Premium*. Initial Payment Range 0.5-5% of Compensable Premium**. Modifying Factors Earn up to 20% more for net growth in specialty enrolled employees. Net losses reduce or eliminate bonus payments. Earn a bonus when you sell 100+ size Dental & Vision Groups in 2019: Agents with eligible fully-insured Humana Dental and/or vision cases with 100-5,000 enrolled subscrib-

ers with effective dates January 1 through December 31, 2019. Agents will earn a bonus equal to 3% of annualized premium for groups with 100 to 999 enrolled sub-

scribers. Agents will earn a bonus equal to 2% of annualized premium for groups with 1,000 to 5,000 enrolled

subscribers. The bonus will be earned for dental and/or vision fully-insured sales for these case sizes. This includes

new dental and/or vision lines sold to existing cases. Humana also has a special earning opportunity for January 1, 2019 only. They have a bundling program

for 100+ cases and now this bonus program too. Agents can earn both for January 1, 2019 as long as the case meets the provisions of both programs. In this instance, “seeing double” is great!

1. To be eligible for the 2% STM commission increase, the supplement premium amount must be equal to or more than $18 per month, and the STM must be 12 months or greater.

2. Bonus applies to new Short Term Medical plans and eligible combined sales packages submitted from August 1, 2019, to January 1, 2020. 3. Each Short Term Medical plan and eligible combined sales package must be in force for a minimum of 90 days to qualify for the applicable bonus. Charge-

Reportable monetary compensation includes, but is not limited to, contingent commissions and finders' fees. Classification as a group “with 51 to 100 eligible employees” is determined by UHC considering a number of factors, including the enrollment at some

point in time. UHC reserves the right to classify groups according to their rules, regardless of a group’s actual enrollment at any time. 5. All bonus payments will be made to the agent or agency to which the commissions are paid. The bonus will be paid after the bonus period is over and

when all information required for verification of enrollment data and calculation of the bonus is available. 6. For dual or multiple broker arrangements, the bonus will be allocated in the same proportion as the commissions are split on the case. 7. General Agents are not eligible for the bonus. 8. UnitedHealthcare is offering a bonus to agents permanently located in AZ & NM who sell new Specialty Bene-

fits lines of coverage to groups with 51 to 100 eligible employees with at least 5 enrolled employees, with

effective dates from Dec. 1, 2019 through Jan. 31, 2020.

Eligible groups are new employer-sponsored or voluntary, fully insured dental, vision, basic life, supplemental

life, short-term disability, long-term disability, critical illness, accident and hospital indemnity lines of coverag-

es sold to groups with 51 to 100 eligible employees with 5 or more enrolled employees per line of coverage

that have effective dates from December 1, 2019 through January 31, 2020. The Specialty Benefits groups

may be sold with medical or on a stand-alone basis.

Agents who meet the qualifying criteria of selling eligible Specialty Benefits lines of coverage to each customer will receive a bonus accord-

ing to the following table:

The bonus amounts in the bonus payment table are not cumulative. Only the highest amount indicated in the row in the table where each of the total number of eligible lines of coverage sold to each customer are met will be paid to the agent or agency that qualifies for the bonus. Program Details

Only Agents of Record permanently located in AZ and NM are eligible for this program.

Eligible groups are new groups with 51 to 100 eligible employees with 5 or more enrolled employees per line of coverage that have effective dates from December 1, 2019 through January 31, 2020. Eligible Specialty benefits lines of coverage are employer-sponsored or voluntary, fully insured dental, vision basic life, supplemental life, short-term disability, long-term disability, critical illness, accident and hospital indemnity groups. The eligible lines of coverage can be sold with medical coverage or on a stand-along basis.

Agents must sell the eligible line of coverage combinations to a customer to qualify for the bonus. The lines of coverage will be derived from the speci-fied group as of January 31, 2020. The enrolled employee counts will be derived from the specified lines of coverage, and will be based on the number of enrolled employees for that line of coverage as of the group’s effective date. UnitedHealthcare’s determination of line of coverage and enrolled em-ployee is final.

All sold business must be active and the selling agent must remain the Agent of Record on January 31, 2020 to be included in the bonus calculations. 5. All bonus payments will be made to the agent or agency to which the commissions are paid. The bonus will be paid after the bonus period is over and

when all information required for verification of lines of coverage sold and enrollment data and calculation for bonus calculations. 6. For dual or multiple broker arrangements, line of coverage credit and enrolled employee credit for determining eligibility for the bonus will be allocated

in the same proportion as the commissions are split on the case. Fractional credits will be used in the calculation, and credits will not be rounded to the nearest integer.

7. General Agents are not eligible for the bonus. 8. Groups transferring from a UnitedHealth Group subsidiary, business segment or case size segment will not be considered new business for this bonus

program. Agent of Record changes on existing UnitedHealthcare groups will not be credited as new business for this bonus program. 9. Special rules apply to payment of bonuses for non-commissionable customers and customers referred to as “Governmental Entities” in the UnitedH-

ealthcare Agent/Agency Agreement. UHC requires written customer acknowledgement and approval before paying bonuses on non-commissionable customers, and on commissionable governmental entity customers. Any limits on compensation in the RFP, RFI, bid specifications or other written instructions for governmental entities cannot be exceeded. Please refer to the Producer Compensation Policies and Practices in the Producer Perfor-mance Guide for more information.

All terms and conditions of the UnitedHealthcare Agent/Agency Agreement and the Producer Performance Guide apply to all compensation programs. CONTEST RULES:

1. To be initially eligible for this incentive, you must be registered for the “Q4 Growth” incentive. Registration prior to

Dec. 1, 2019 will have all online applications received Nov. 1, 2019, and after, count toward the incentive. On Dec.

1, 2019, and after, only online applications received on or after date of registration will be applied toward the incen-

tive.

2. Eligible product applications are those submitted online from November 1, 2019 through January 31, 2020 and are

for one of the following eligible products: TriTerm Medical Plans, Health ProtectorGuard plans, Golden Rule Insur-

ance Company Short Term Medial plans, Accident plans (Pro series and SafeGuard plans). Critical Illness plans,

3. Registered brokers must meet a minimum 20 select product applications baseline before the incentive may be earned on all eligible placed business

submitted during the contest period. Applications that meet the following criteria will apply toward the baseline:

TriTerm Medical, Short Term Medical and/or Health ProtectorGuard applications only (any combination of these products).

Submitted between Nov. 1, 2019-Jan. 31, 2020, and issued by February 15, 2020.

With a “paid to” date past the effective date, and

An effective date no later than April 1, 2020.

4. Once the baseline is met, all eligible plans must be in-force (active, paid coverage) for a minimum of three months (90 days) after the effective date to

count as “placed”. In the case of Short Term Medical consecutive plans (e.g. 2x8 plan) only the first plan will count toward the incentive, provided it

meets all other eligibility requirements. If coverage is terminated for any reason prior to three months, the plan no longer becomes eligible for incentive

payment.

5. Bonus follows the Assignment of Commission Form you have on file at the time the application is received, for each qualifying application.

6. These bonuses will be subject to a managing broker commission split, if applicable.

7. Bonuses are paid per broker, based on his or her personal production.

8. Incentive will be paid over a series of rolling months and will not exceed five payouts. This schedule is designed to eliminate chargebacks and ensure

proper payment on qualified plans. The Q4 Growth Incentive payout will begin monthly in March 2020 through July 2020, with the intent to pay no later

than the second commission pay period of each month. Each month will account for eligible business through the third week of the prior month.

9. Qualification is based on production as broker of record.

10. Applications cannot be split between brokers.

11. Any separate applications of family members for the same product type count as only one application for this contest.

12. Broker must have an active contract with Golden Rule Insurance Company and be in good standing at the time of contest payout in order to receive

earned incentive payment.

13. Bonus is paid as described above.

14. Golden Rule Insurance Company reserves the right to make final judgement on contest qualifiers or end this contest at any time without prior notifica-

tion. Coverage that is withdrawn or does not meet the terms listed in these rules will not count toward contest bonus.

15. Any required disclosures to brokers’ clients are brokers’ sole responsibility.

16. Select Key and FMO contract agencies may not be eligible.

17. 1099s apply.
