Solution Overview

Ø  CareFeed is an online communication platform that enables

providers to •  communicate with their patient populations •  via a real-time information feed that is •  available on the web, Windows 8 application and on iOS mobile

devices.

Ø  Essentially, the system is designed to allow providers to

increase engagement with patients and improve chronic

care outcomes

Problem

Ø  Chronic diseases are a significant burden on the healthcare system

Ø  Chronic disease experts have called for a “paradigm shift” in

provider-patient relations from directive to more and enhanced

patient-provider communication

Ø  However providers face a significant challenge in maintaining

regular communication with patients •  Lack of time •  Lack of appropriate tools besides manual phone calls or sending postcards •  Lack of specificity and exclusivity of social media tools such as twitter

Technical Summary

Ø  The CareFeed App is integrated with the AllScripts EHR using the API and

can run across any browser

Ø  Available as Windows 8 application and also as iPhone application

Ø  The app uses several API calls to both read and push data to the EHR

Ø  App works for a specific patient and also performs cross patient queries

Ø  Integrated with a cloud-service to make automated calls

Ø  Built as a Django/Python application that works across browsers

Key Innovations of CareFeed Ø Gives providers the complete control of the communication

Ø  Integrated with the EHR, hence allowing providers to use existing information and one which is pertinent to the

patient

Ø  Designed to facilitate communication between real-world

physician-patient relationship, and not a generic website or tool that may connect a patient to another doctor who may

not be their patient

Ø Allows messages to be tailored based on the patient record

– providers can create groups of patients to target the message based on the real patient record
