CAPITAL CODERS DIGEST DECEMBER 2007 Happy Holidays to you all! PRESIDENT’S CORNER December is now upon us and as I reflect back on 2007 I am so pleased about what a wonderful year it has been. As a group we have done a great deal to promote our organization and help it to grow, and it has been an honor serving as your chapter president. I want to thank each of you who were willing to serve as an officer this year. Amy Brazell not only was most helpful as president elect by doing registration for the Spring Seminar and also providing us with our Coder’s Day Luncheon, but was always willing to step in to be a speaker for the group and be a regular contributor to the newsletter. Janet Dunkerley kept accurate minutes and has continued to do an outstanding job providing us with a monthly newsletter. Belinda Inabinet has faithfully kept the books and made sure all the information required by national was turned in on time. Debra Jones has not only continued to promote membership by collecting goodies for our monthly meeting raffles, but also has provided space and been a proctor for all the exams last year and this year. She has provided information to each of the examinees, and we have seen many of them join our organization because of it. Lastly, Lisa Duval has made sure that our monthly speakers are outstanding and that we have all the paper work completed on time, so we receive our CEU’s. I also want to thank those who have accepted these positions for next year we are all looking forward to an outstanding year to come. PRESIDENT’S CORNER

December is now upon us and as I reflect back on 2007 I am so pleased about what a wonderful year it has been. As a group we have done a great deal to promote our organization and help it to grow, and it has been an honor serving as your chapter president.

I want to thank each of you who were willing to serve as an officer this year. Amy Brazell not only was most helpful as president elect by doing registration for the Spring Seminar and also providing us with our Coder’s Day Luncheon, but was always willing to step in to be a speaker for the group and be a regular contributor to the newsletter. Janet Dunkerley kept accurate minutes and has continued to do an outstanding job providing us with a monthly newsletter. Many of you were instrumental in making our Spring Seminar a huge success. Those projects do not happen without many hands being willing to assist with registration, volunteering to be speakers and doing all the other little jobs that need to be done to make a meeting successful. Because of you we have provided outstanding educational opportunities for our members and the community. You have provided us with outstanding meeting facilities and when problems arose scurried to find alternate facilities. Because each of you has contributed to our organization we have been able to pay for National Conference registration for our officers as well several of our chapter members. We also have money left over in our account to defray the cost of chapter shirts for each of our members as well as pay for our Christmas dinner this year.

Each of you has been a part of making this an outstanding year, we accomplished all we did because of your efforts. As I pass the leadership to next years Board let me again thank you for all your help and support. I also want to wish each of you Merry Christmas and a wish for a blessed and prosperous new year.

Christmas Dessert CodingBy Amy Brazell, RHIT, CCS, CPC-H

There are so many things I could write about this month. For example, there are the CPT code updates. There is also the Hospital Outpatient PPS “Final Rule” which can be found at http://www.cms.hhs.gov/HospitalOutpatientPPS. Hmmm, what to choose... Well I don’t know the CPT updates yet – I’ll be listening to an audio conference on them soon, which I registered for by renewing my AAPC membership early. As for the OPPS updates… well… Let’s put it this way: the final changes are in a document that is 1,969 pages that includes at least eight addendums and a dozen tables and an analysis of 141 million claims in hospital outpatient settings. Yes, that’s right: 141 million claims. These were whittled down some but geez! As soon as I read this I’ll let you know.

In the meantime I’ll write about what everyone writes about in the December magazines: how not to gain weight while at the same time how to make delicious desserts for Christmas. Since this is a coding newsletter I’ll relate this to coding.

First, make something with cinnamon. According to the magazines cinnamon is good for keeping your cholesterol down. Hypercholesterolemia.

Second, make something with chocolate. Some studies have shown that chocolate helps lower blood pressure and boosts your immune system. Hypertension. Unspecified disorder of immune system.

Third, you’ll probably have to use some kind of fat like butter, margarine, shortening, etc. Think overweight and BMI over 40 or BMI, pediatric, greater than 95th percentile.

Fourth, you might as well use some nuts too. Nuts have healthy fats, protein, and fiber which are good for heart health. Prevent CAD and CHF.

Finally, now that you’ve made several desserts that are loaded with health benefits but calorie rich, you need a way to keep from gaining weight. Abnormal weight gain. One solution is to do a cardiac stress test on a treadmill but the best solution is to give it to your co-workers. Sharing is caring. Besides, if they gain weight it will only make you look that much better in comparison.

Coding Humor: Erin Goodwin, CPC came across this predicament and sent it along for your reading pleasure!

A pt called today wanting an ultrasound so her parents can see the baby while they are in town for the holidays.  Did anybody else just gasp?  

I thought, this is elective and yes, we can do it, but the patient has to pay cash.  She informed me her insurance company states, she is allowed 2 ultrasounds per preg and she has only had one. Yes, I understand but this is an elective ultrasound, with no indication and that is not covered..  

Pt called her insurance company and they call me, 3 way call.  I am non-compliant with our contract by denying this pt the us.  I am non-compliant, what?  I asked them what the dx would be for this us and they told me, “it is not their job to code my services”.  

Any thoughts on how I might code something like this?  Is there a code for ultrasound done for entertainment purposes?  Am I missing something?  The insurance company asked how I billed the first ultrasound and I stated screening, she told me to bill this one the same as the first one.  This is not possible, we are not doing the screening this time.  Ugh!
