•Through December 2013 - enroll a diverse group of at least 300,000 men and women across the United States aged 30-65 years with no prior history of cancer

•At least 25% minority participation

CPS-3 Community Champions assist with promotional efforts to educate individuals

about CPS-3, including criteria for enrollment with an emphasis on the long-term

commitment to the study

The goal of each CPS-3 champion is to engage ten qualified individuals in the study

Role of the CPS-3 Participant

Who Can Enroll to be a CPS-3 Study Participant?

To be eligible to enroll in CPS-3, men and women should:•be between 30 and 65 years old•have no personal history of cancer (this does not include basal or squamous cell skin cancer)•be willing to make a long-term commitment to the study which simply involves completing periodic follow-up questionnaires.

What is Involved in Being a CPS-3 Study Participant?

Initial enrollment• Part One (at home electronic)

– Comprehensive baseline survey

• Part Two (on site during appointment)

– Consent and short survey

– Waist circumference

– Small blood sample

Long-term follow-up • Repeat surveys every few years
