Getting Started Logging in Go to https://app.calstates4.com/csuchico Click the Student/Faculty Log In button and use your campus username and password to log in to the system During the login process you may be asked to select your campus in order to be successfully redirected to your campus’ single-sign-on page

Once logged in your will see a page like the one below.

User Guide

You will be directed to a page that includes all of your current and previous courses by semester. Click on the course link or the “View Course” button to access the course

The course page lists your students and their placements.

Click the placement link to open a popup. Use the “View Placement #” link in the pop-up to access a student’s placement page

The placement page contains the required forms for a student’s field placement

1. Forms are separated by the role that completes them. Only that role can access the form

2. “Complete form” button means that a form has not been started 3. “Complete draft” means a form has been started, but not completed 4. “View submission” means the form is complete and can be viewed

no matter what role each user is 5. Forms that require a signature are noted on the right

Signing Forms To sign or check on what signatures have been completed, click on “View submission”

Once you are viewing the form, you can look in the upper right of the page to see which signatures have been submitted (in green) and which signatures are still needed (in red).

When both Signature 1 (main site supervisor) and Signature 2 (task supervisor or off-site agency field instructor) have been submitted and show in green, it is your turn to review and sign the form. Click the Field Liaison Signature link and scroll to the bottom to electronically sign and submit.

View Site Related Information From the placement page, click the site link to view site information

Click the “Site staff” link to view contact information for Field Instructors/Task Supervisors

Viewing Student Information From the course page, click on the student name to view student related information

You can view the student’s email address and current and past courses

You can also view current and past placements and all forms submitted by the student
