Introduction

Who are Caer Las?We are a charitable company operating in the third sector.

What do we hope to achieve?Our vision is to continually provide the highest possible quality of service that promotes social inclusion in Wales.

Who do we help?We work with people who encounter any form of social exclusion; for example, people whose lives can be traumatised by poverty, mental ill health or substance misuse.

What do we do?We support people to gain confidence and self-esteem in order to build the life they want. We earn respect by listening attentively to clients, funders, commissioners and peers. We develop creative solutions and provide excellent value for money.

What Outcomes do we achieve?We are proud to hear that so many people we work with make sustainable change to their lives, gaining independence and enhancing their well-being.

What are our values?• We believe that everyone has strengths and resources to create their own solutions and build a better life

• We regard everyone as having equal value and treat people with dignity and respect

• We are committed to absolute equity and fairness in everything we do

• We are committed to work with people who experience extreme vulnerability despite the challenges this may present.

It is eighteen months since I joined Caer Las Cymru as the Executive Director. I am really proud to lead an organisation that makes such a positive difference to some of the most disadvantaged, socially excluded people in Wales.Our mission is to promote social inclusion in Wales; or to put it more plainly to help and support some of Wales most excluded citizens to regain the confidence and self respect needed to engage in mainstream society. Social exclusion is one of those terms that can take on many meanings; at Caer Las we try to keep it simple: there are some aspects of life that most of us are privileged to take for granted, jobs, family, a secure home, good health and a sense of well-being, access to primary health care, control over our own money or even our liberty.

Most of us are lucky enough to be socially included; Caer Las works with those who are not.

Our work is targeted at tackling four major aspects of social exclusion:

• We support and house people who are trying to turn their life around after leaving prison• We support people who are struggling with a substance use problem• We support people to cope with, and flourish despite suffering mental ill-health• We help people who are homeless or are at risk of becoming homeless to keep or acquire appropriate accommodation• On many occasions we support people with all of the above

Our belief is that people don’t choose a life punctuated by these issues, they are at any given time doing their best. Some people are born into really difficult circumstances, other people experience them randomly. It is of no use to us, or our clients to make any judgement upon why someone is in the position they are. We accept people for what they are, respect them, and believe that everyone has strengths and resources that can lead them to the life they want. Our job is to work with people’s inherent resourcefulness and capability and support them to become as included in mainstream society as they want to be.

We give practical help to people in a tough spot, we work alongside a plethora of other agencies, and we talk to people. It is my belief that by undertaking quality, conversation based, personal development, work we can make a sustainable difference to peoples lives, helping to restore hope, confidence, self respect and dignity.

Message from the Chair of the Board of Trustees

It is with a great sense of pride that I tell people I am chair of the Caer Las board. That pride is inspired by our staff and the difference they make to people’s lives on a daily basis; the remarkable changes our clients make to their lives; the great relationships we have with our funders; and the culture of inclusion and respect we have built together.

This has been a challenging year for Caer Las and along with the rest of our sector we’ve faced budget and benefit cuts which have and will continue to have a significant impact. In anticipation, we have taken a range of steps to give ourselves the best possible chance of preserving our services and meeting our client’s needs. Many of those steps you will read about in this document.

As a board, we have also been doing work to strengthen our governance procedures and recruit new board members. We have looked for people with the skills and expertise to support Caer Las as we move forward through these challenging times, enabling us to make the most of any opportunities which may present themselves.

As an organisation we have developed a realistic and yet aspirational business plan which will lead us forward positively with a sense of determination.

Social Inclusion

Caer Las has been working hard to tackle the causes of social exclusion for over 35 years. We take a holistic approach to tackling the issues and try new and innovative approaches across all of our projects. Our support uses a person centred approach and delivers tailored support to the individual at their time of need.

Skills and Training Over the past 12 months we have looked to develop interworking between projects, so that clients can access different levels and types of support. Our training and skills programme has been adapted and improved and we now offer a wider range of OCN accredited courses which are being delivered at a wider range of sites led by demand from service users. We have also increased our offering of Financial Inclusion training and support due to the current economic climate and now have staff with specialist skills in this area to support increased demand.

Floating Support Our Floating Support team take pride in providing specialist generic support for people with complex needs. As well as day to day management of a wide range of clients who have difficulties managing tenancies, the teams also provide, benefits and

welfare rights advice and have a strong network of partner agencies in each location to support service users further.

There is an emphasis on helping clients to integrate into society and become further involved in their local communities, a regular news letter is distributed in Swansea, Neath Port Talbot work closely with employment initiatives and Cardiff have specialist language skills and BSL Sign Language support.

Young Persons Scheme In Neath and Port Talbot Caer Las operates a dedicated Young Persons support Scheme aimed at client aged 16-24 helping them to understand their rights and responsibilities as a tenant, the scheme provide flexible, floating support and a 24 hour on call service.

Clients tend to be inexperienced in holding a tenancy and the basic skills necessary to sustain independent living. The project provides support on a one to one basis to develop and maintain those skills, such as financial and debt advice, benefits and welfare rights information, health and social issues and provides guidance and signposting to education work and training opportunities.

As well as offering practical tenancy support, the project is keen to develop young people’s confidence and social skills. It offers a variety of client participation events and groups that nurture a holistic and progressive support service.

Getting Better

We run a number of projects aimed at improving mental and physical wellbeing, social inclusion, skills and learning and independent living.

Over the past 12 months Caer las has worked to introduce the Solution Focus Brief Therapy model of support in every area of the organisation. Solution Focus concentrates on what the client wants to achieve rather than the problems in their past. We find it creates a more positive environment with outcomes that are client driven.

Connect Connect is a membership based community resource centre in the centre of Swansea dedicated to working with people who experience learning difficulties and mental illness, some people will have had experience of homelessness, substance misuse and difficulties coping with independent living.

The focus of the connect project is to ensure that services are member led, meaningful and re-abling. Activities include arts and crafts, food, music, drama, sports and leisure and gardening all within a relaxed café style environment.

Connect has an ever increasing membership base, currently at 150 plus which is testament to the popularity of this service.

Willow Project Willow provides 24 hour supported accommodation for 7 people with severe mental health problems. It provides positive and restorative support to it’s users who have usually spent a large amount of the adult lives in and out of secure / low secure hospitals and have never experienced managing a tenancy or living in the community has been a negative experience for them.

Tenants live in their own high quality self contained flats with access to professional support at any time and are encouraged to develop skills such as budgeting, household management and improving social opportunities while reinforcing the positive aspects of living within a community.

The key to the success of the Willow Project is the co-ordination of clinical and specialist support whilst managing medication and routines in as normal an environment as possible.

New Mill New Mill consists of six units of accommodation with communal areas in each house and on site support. The project provides longer term low level support to individuals recovering from mental ill health.

Clients are encouraged and supported to access community services and facilities to enable them to pursue personal interests and hobbies and to develop the skills in order to move towards a more independent quality lifestyle.

Housing and HomelessnessAt Caer Las, we are a firm believer in the “housing first” model, a stance we took many years ago when we moved away from hostel accommodation in preference of helping people to maintain their own tenancy and take necessary steps to building their own sustainable and independent lives.This model has proved enormously successful and has the sympathy of the Welsh Government and with all likelihood will be implemented into the new “Welsh Housing Bill”

Caer Las owns and manages properties across South Wales and works closely with partner organisations to provide tailored support to people in their own home environment.

Shared Housing Our model of Shared Housing is becoming more popular with both service users and commissioners across South Wales. We currently provide 12 houses of shared accommodation over 4 counties housing over 50 vulnerable people within their own communities

We find that by providing an individually tailored package of support within a high quality shared house and working closely with partner agencies can make a significant difference in people’s lives and while not everyone wants to share communal facilities, those who chose this option can benefit greatly. It addresses the feelings of loneliness and isolation common amongst vulnerable people and is a positive experience of learning how to live with other people whilst managing a tenancy.

Outreach Outreach is Swansea based project that offers practical support and advice to people who are homeless or threatened with homelessness.

As the prevention of homelessness is far better than reacting to it, the service is also actively targeted at Hospital and Prison discharge processes, mental health and substance misuse agencies seeking to prevent vulnerable people from entering a chronic homelessness cycle.

As well as providing access to support for housing and benefits, clients can be supported with skills and training opportunities, solicitors, counsellors and specialist interpreters.

Outreach works alongside a number of other agencies in Swansea and plays a key role in the implementation of the new Individual Budgets programme.

Routes Routes is a highly visible drop in centre located in Port Talbot town centre providing advice and support for people who are homeless or are threatened with homelessness. It provides a valuable service to people who are in crisis for a variety of reasons.

Staff at Routes are highly knowledgeable of local services and by working closely with local partner agencies, local authorities and benefits agencies, Routes can offer a rapid response service for people experiencing social and economic deprivation and have built a highly successful network of local private landlords that assist with emergency housing needs.

Other services include access to welfare rights advice, counselling and legal advice.

Bridgend Housing Advice Caer Las Housing Advice scheme provides support to Probation Services to support offenders and ex offenders to reintegrate into their communities by working with local housing providers, benefits agencies, the local authority and the bond board to minimise the risk of clients becoming homeless therefore reducing reoffending rates within the county.

Caer Las is a community business and takes responsibility for the well being of its employees and service users. This is reflected in the level of reserves where Caer Las strives to ensure that these are at an adequate level to enable continuation of operations for a defined period of time if funds were to cease.

Mae 18 mis wedi mynd heibio ers i mi ymuno â Chaer Las fel y Cyfarwyddwr Gweithredol. Rwy’n wirioneddol falch fy mod yn arwain sefydliad sy’n gwneud y fath wahaniaeth cadarnhaol i rai o’r bobl fwyaf difreintiedig ac sy’n wynebu’r lefel fwyaf o allgau cymdeithasol yng Nghymru.Ein cenhadaeth yw hyrwyddo cynhwysiant cymdeithasol yng Nghymru; neu’n symlach helpu a chefnogi rhai o’r bobl sydd wedi’u hallgáu fwyaf yng Nghymru i adennill yr hyder a’r hunan-barch sydd eu hangen i gymryd rhan mewn cymdeithas brif ffrwd. Rwy’n ymfalchïo’n fawr yn y ffaith mai fi yw cadeirydd bwrdd Caer Las. Ysbrydolir y balchder hwnnw gan ein staff a’r gwahaniaeth y maent yn ei wneud i fywydau pobl bob dydd; y newidiadau rhyfeddol y mae ein cleientiaid yn eu gwneud i’w bywydau; y cydberthnasau gwych sydd gennym â’n harianwyr; a’r diwylliant o gynhwysiant a pharch rydym wedi’i greu gyda’n gilydd.

Bu’r flwyddyn hon yn un anodd i Gaer Las ac ynghyd â gweddill ein sector rydym wedi wynebu toriadau mewn cyllidebau a budd-daliadau sydd wedi cael effaith sylweddol ac a fydd yn parhau i wneud hynny. Rydym eisoes wedi cymryd nifer o gamau i sicrhau ein bod yn y sefyllfa orau posibl i ddiogelu ein gwasanaethau a diwallu anghenion ein cleientiaid. Byddwch yn darllen am lawer o’r camau hynny yn y ddogfen hon.

Fel bwrdd, rydym hefyd wedi bod yn gweithio i atgyfnerthu ein gweithdrefnau llywodraethu a recriwtio aelodau newydd i’r bwrdd. Rydym wedi chwilio am bobl â’r sgiliau a’r arbenigedd i gefnogi Caer Las wrth i ni symud ymlaen drwy’r cyfnod anodd hwn, gan ein galluogi i fanteisio i’r eithaf ar unrhyw gyfleoedd a all godi.

Fel sefydliad rydym wedi llunio cynllun busnes sy’n realistig ac eto’n ddyheadol a fydd yn ein harwain ymlaen mewn modd cadarnhaol a phenderfynol.

Cynhwysiant Cymdeithasol

Bu Caer Las yn gweithio’n galed i fynd i’r afael â’r rhesymau dros allgau cymdeithasol ers dros 35 mlynedd. Rydym yn ymdrin â’r problemau mewn ffordd gyfannol ac yn rhoi cynnig ar ddulliau gweithredu newydd ac arloesol ym mhob un o’n prosiectau. Mae’r cymorth rydym yn ei roi yn canolbwyntio ar yr unigolyn ac wedi’i deilwra ar ei gyfer pan fo’i angen arno.

Sgiliau a Hyfforddiant Yn ystod y 12 mis diwethaf rydym wedi ceisio datblygu rhyngweithio rhwng prosiectau, fel y gall cleientiaid gael gafael ar wahanol lefelau a mathau o gymorth. Mae ein rhaglen hyfforddiant a sgiliau wedi’i haddasu a’i gwella ac rydym bellach yn cynnig ystod ehangach o gyrsiau wedi’u hachredu gan Rwydwaith y Coleg Agored (OCN) sy’n cael eu darparu ar amrywiaeth ehangach o safleoedd a arweinir gan y galw o du defnyddwyr gwasanaethau. Rydym hefyd wedi cynyddu’r hyfforddiant a’r cymorth ym maes Cynhwysiant Ariannol a gynigir gennym o ganlyniad i’r hinsawdd economaidd bresennol ac erbyn hyn mae gennym staff â sgiliau arbenigol yn y maes hwn i helpu i ateb y galw cynyddol.

Cymorth Fel y Bo’r Angen Mae ein tîm Cymorth fel y Bo’r Angen yn ymfalchïo mewn rhoi cymorth cyffredinol arbenigol i bobl ag anghenion cymhleth. Yn ogystal â’r gwaith bob dydd o reoli amrywiaeth eang o gleientiaid sy’n ei chael

hi’n anodd rheoli tenantiaethau, mae’r timau hefyd yn rhoi cyngor ar fudd-daliadau a hawliau lles ac mae ganddynt rwydwaith cadarn o asiantaethau partner ym mhob lleoliad i roi cymorth pellach i ddefnyddwyr gwasanaethau.

Rhoddir pwyslais ar helpu cleientiaid i integreiddio mewn cymdeithas a chymryd mwy o ran yn eu cymunedau lleol. Dosberthir cylchlythyr rheolaidd yn Abertawe, mae Castell-nedd Port Talbot yn gweithio’n agos gyda mentrau cyflogaeth ac mae Caerdydd yn cynnig cymorth gyda sgiliau iaith arbenigol ac Iaith Arwyddion BSL.

Cynllun Pobl Ifanc Yng Nghastell-nedd Port Talbot mae Caer Las yn gweithredu Cynllun Cymorth penodol i Bobl Ifanc sydd wedi’i anelu at gleientiaid rhwng 16 a 24 oed gan eu helpu i ddeall eu hawliau a’u cyfrifoldebau fel tenantiaid. Mae’r cynllun yn cynnig cymorth hyblyg fel y bo’r angen a gwasanaeth ar alw 24 awr.

Mae cleientiaid yn tueddu i fod yn ddibrofiad o ran dal tenantiaeth a’r sgiliau sylfaenol sydd eu hangen i’w galluogi i fyw’n annibynnol. Mae’r prosiect yn rhoi cymorth un i un i ddatblygu a chynnal y sgiliau hynny, megis cyngor ar arian a dyledion, gwybodaeth am fudd-daliadau a hawliau lles, iechyd a materion cymdeithasol ac mae’n rhoi arweiniad ac yn cyfeirio pobl at gyfleoedd addysg, gwaith a hyfforddiant.

Yn ogystal â chynnig cymorth tenantiaeth ymarferol, mae’r prosiect yn awyddus i feithrin hyder a sgiliau pobl ifanc. Mae’n cynnig amrywiaeth o ddigwyddiadau a grwpiau cyfranogiad cleientiaid sy’n meithrin gwasanaeth cymorth cyfannol a blaengar.

Gwella

Rydym yn cynnal nifer o brosiectau i wella lles meddyliol a chorfforol, cynhwysiant cymdeithasol, sgiliau a hyfforddiant a byw’n annibynnol.

Yn ystod y 12 mis diwethaf mae Caer Las wedi gweithio i gyflwyno’r model cymorth Therapi Byr sy’n Canolbwyntio ar Atebion ym mhob rhan o’r sefydliad. Mae Canolbwyntio ar Atebion yn edrych yn benodol ar yr hyn y mae’r cleient am ei gyflawni yn hytrach na’r problemau yn ei orffennol. Yn ein profiad ni mae hyn yn creu amgylchedd mwy cadarnhaol gyda chanlyniadau sy’n cael eu llywio gan y cleient.

Connect Mae Connect yn ganolfan adnoddau cymunedol i aelodau yng nghanol Abertawe a sefydlwyd yn benodol i weithio gyda phobl ag anawsterau dysgu a salwch meddwl. Bydd rhai pobl wedi cael problemau o ran bod yn ddigartref, camddefnyddio sylweddau ac ymdopi i fyw’n annibynnol.

Mae prosiect Connect yn canolbwyntio ar sicrhau bod gwasanaethau yn cael eu harwain gan yr aelodau, eu bod yn ystyrlon a’u bod yn ail-alluogi. Ymhlith y gweithgareddau mae celf a chrefft, bwyd, cerddoriaeth, drama, chwaraeon a hamdden a garddio a’r cyfan mewn amgylchedd ymlaciedig sy’n debyg i gaffi.

Mae aelodaeth Connect yn cynyddu’n gyson. Ar hyn o bryd mae ganddi fwy na 150 o aelodau sy’n tystio i boblogrwydd y gwasanaeth hwn.

Prosiect Willow Mae Willow yn darparu llety â chymorth 24 awr i saith unigolyn â phroblemau iechyd meddwl difrifol. Mae’n rhoi cymorth cadarnhaol ac adferol i’w ddefnyddwyr sydd fel arfer wedi treulio cryn dipyn o’u bywydau fel oedolion i mewn ac allan o ysbytai diogel / diogelwch isel ac nad ydynt erioed wedi cael profiad o reoli tenantiaeth neu y mae byw yn y gymuned wedi bod yn brofiad negyddol iddynt.

Mae’r tenantiaid yn byw yn eu fflatiau hunangynhwysol eu hunain sydd o ansawdd uchel. Mae cymorth proffesiynol ar gael iddynt unrhyw bryd ac fe’u hanogir i feithrin sgiliau megis rheoli eu harian, rheoli’r cartref a gwella cyfleoedd cymdeithasol tra’n atgyfnerthu’r agweddau cadarnhaol ar fyw mewn cymuned.

Cydgysylltu cymorth clinigol ac arbenigol tra’n rheoli meddyginiaeth ac arferion mewn amgylchedd mor normal â phosibl sy’n allweddol i lwyddiant Prosiect Willow.

New Mill Mae New Mill yn cynnwys chwe uned o lety â mannau cyffredin ym mhob tŷ a chymorth ar y safle. Mae’r prosiect yn rhoi cymorth lefel isel tymor hwy i unigolion sy’n gwella ar ôl cael salwch meddwl.

Caiff cleientiaid eu hannog a’u helpu i fanteisio ar wasanaethau a chyfleusterau cymunedol i’w galluogi i fwynhau diddordebau personol a hobïau a datblygu’r sgiliau er mwyn symud tuag at ffordd o fyw fwy annibynnol o ansawdd.

Tai a Digartrefedd Yng Nghaer Las rydym yn credu’n gryf yn y model “tai yn gyntaf”, safbwynt a gymerwyd gennym flynyddoedd lawer yn ôl pan wnaethom roi’r gorau i lety hostel o blaid helpu pobl i gynnal eu tenantiaeth eu hunain a chymryd y camau angenrheidiol i greu eu bywydau cynaliadwy ac annibynnol eu hunain.

Bu’r model hwn yn hynod lwyddiannus. Mae ganddo gefnogaeth Llywodraeth Cymru ac yn ôl pob tebyg caiff ei weithredu ym Mil Tai newydd Cymru.

Mae Caer Las yn berchen ar eiddo ac yn ei reoli ledled De Cymru ac mae’n gweithio’n agos gyda sefydliadau partner i oi cymorth wedi’i deilwra i bobl yn eu cartrefi eu hunain.

Tai a Rennir Mae ein model o Dai a Rennir yn dod yn fwyfwy poblogaidd gyda defnyddwyr a chomisiynwyr gwasanaethau ledled De Cymru. Ar hyn o bryd rydym yn darparu llety a rennir ar ffurf 12 tŷ i dros 50 o bobl agored i niwed yn eu cymunedau mewn pedair sir.

Yn ein profiad ni, drwy ddarparu pecyn cymorth sydd wedi’i deilwra i’r unigolyn mewn tŷ a rennir o ansawdd uchel a gweithio’n agos gydag asiantaethau partner, gallwn wneud gwahaniaeth mawr i fywydau pobl ac, er nad yw pawb am rannu cyfleusterau cymunedol, gall y rhai sy’n dewis yr opsiwn hwn gael budd mawr. Mae’n mynd i’r afael â’r teimladau o unigrwydd ac ynysu sy’n gyffredin ymhlith pobl sy’n agored i niwed ac mae’n brofiad cadarnhaol o ddysgu sut i fyw gyda phobl eraill tra’n rheoli tenantiaeth.

Outreach Mae prosiect Outreach wedi’i leoli yn Abertawe ac mae’n cynnig cymorth a chyngor ymarferol i bobl sy’n ddigartref neu sydd mewn perygl o fod yn ddigartref.

Am ei bod yn llawer gwell atal digartrefedd nag ymateb iddo, mae’r gwasanaeth hefyd yn targedu prosesau rhyddhau Ysbytai a Charchardai, asiantaethau iechyd meddwl a chamddefnyddio sylweddau sy’n ceisio atal pobl sy’n agored i niwed rhag cael eu dal mewn cylch cronig o ddigartrefedd.

Yn ogystal â helpu cleientiaid i gael cymorth o ran tai a budd-daliadau, gelant hefyd gael help i fanteisio ar gyfleoedd i feithrin sgiliau a chael hyfforddiant a chael gafael ar gyfieithwyr, cynghorwyr a dehonglwyr arbenigol.

Mae Outreach yn gweithio ochr yn ochr â nifer o asiantaethau eraill yn Abertawe ac mae’n chwarae rôl allweddol o ran gweithredu’r rhaglen Cyllidebau Unigol newydd.

Routes Mae Routes yn ganolfan galw heibio amlwg iawn yng nghanol tref Port Talbot sy’n rhoi cyngor a chymorth i bobl sy’n ddigartref neu sydd mewn perygl o fod yn ddigartref. Mae’n darparu gwasanaeth gwerthfawr i bobl sydd mewn argyfwng am nifer o resymau.

Mae gan staff Routes gryn dipyn o wybodaeth am wasanaethau lleol a, thrwy weithio’n agos gydag asiantaethau partner lleol, awdurdodau lleol ac asiantaethau budd-daliadau, gall Routes gynnig gwasanaeth ymateb cyflym i bobl sy’n wynebu amddifadedd cymdeithasol ac economaidd ac mae wedi creu rhwydwaith tra llwyddiannus o landlordiaid preifat lleol sy’n helpu i ddiwallu anghenion tai brys.

Mae gwasanaethau eraill yn cynnwys mynediad at gyngor ar hawliau lles, cwnsela a chyngor cyfreithiol.

Cyngor ar Dai Pen-y-bont ar Ogwr Mae cynllun Cyngor ar Dai Caer Las yn helpu’r Gwasanaeth Prawf i helpu troseddwyr a chyn-droseddwyr i ailintegreiddio yn eu cymunedau drwy weithio gyda darparwyr tai lleol, asiantaethau budd-daliadau, yr awdurdod lleol a’r bwrdd bondiau i leihau’r risg y caiff cleientiaid eu gwneud yn ddigartref tra’n lleihau cyfraddau aildroseddu yn y sir.
