How to begin?

• With any improvement project you will need to know what you are trying to do or improve upon.

• An improvement project will need to have a document for organizing your work.

• There are MANY types of documents [Charters] that can help organize your work.

AIM statements• Aim statements do the following:

• Define the problem

• The process you are trying to improve

• Articulate a clear message to the team

• The more specific the AIM the better chances the teams will be able to improve

AIM statement examplesThrough the implementation of an EMR our patients will get better.

The management of diabetes patients served at Make Believe Clinic will have their health outcomes improve. By May 2013 we will increase the percentage of patients meeting A1C and blood pressure targets from 25% to 70%.
