Police Check Vulnerable Sector Screening:

Deliver vss to Board prior to placement. Must be current within 30 days.

You will be required to submit a photocopy the first week of classes (further instructions for where to submit will be sent in August).

Health and Safety Training Requirements:

Complete the "Training for Workers - Worker Health and Safety Awareness in 4 steps":

• Ensure that pop ups are enabled; the certificate opens in a new window

• Print and save the certificate once complete.

• Provide to placement school principal (advisable to make several copies for potential future use - dependent upon next board's process)

• You will be required to submit a photocopy the first week of classes (further instructions for where to submit will be sent in August).

Police Check Vulnerable Sector Screening: You will be required to submit a

Complete the DSBN Health and Safety Orientation Package, available at

https://drive.google.com/file/d/1rgF5zQX3ekxoMWalAx__BPXnwoci4Px2/view?usp=sharing (you must first be logged in to your LU email in order to access the google folder).

• Once the teacher candidate has reviewed the above package, have your faculty advisor sign page 1 acknowledgment letter (along with teacher candidate signature)

• On the first day of placement teacher candidate completes Appendix A Checklist with the associate teacher

• Principal/Vice-Principal signs Appendix A Checklist

• Package is returned to the teacher candidate at the end of placement

The Checklist cannot be re-used - it must be completed for every placement (not the entire training package; just the Checklist).

• Complete all training modules indicated on the Confirmation of Training Form

• Student must sign and date the Confirmation of Training Form

Peel District School Board Police Check Vulnerable Sector Screening: You will be required to submit a photocopy the first week of

classes (further instructions for where to submit will be sent in August). Request two originals when applying for your VSS check with your police service. If both fall and winter session placements are with Peel then the vss check is valid for the school year. If only the winter session is with Peel then you must reapply for new vss check.

All new employees, volunteers, service providers, individuals requiring a placement for professional certification and others (the “applicant”) are required to provide a Criminal Record Check that is satisfactory to the board prior to the commencement of any duties. The Criminal Record Check must meet all of the conditions outlined below:

• Must have been completed by a police service within the preceding 6 months

• Must include a Vulnerable Sector Screening (VSS) including a search of the pardoned sex offender database

• Must be an original

It is the responsibility of the applicant to ensure that the above conditions are met. Any cost related to obtaining the Criminal Record Check is the sole responsibility of the applicant.

The original copy of the Criminal Record Check, and any additional information requested, will become the property of the Peel District School Board, and will not be returned, copied or forwarded to any other institution or individual. The original Criminal Record Check should be hand delivered, if possible, to Human Resources Support Services, 2nd floor, 5650 Hurontario Street, Mississauga, ON L5R 1C6, in a sealed envelope addressed to Employee Relations Officer, Criminal Record Checks/Professional Certification, The envelope should be marked confidential, include the applicant’s full name and nature o f duties (employment, volunteer, placement, etc.), and the words “Criminal Record Check”. This is an important document. You should make a copy of your Criminal Record Check for your own records prior to submitting.

The Board’s recommendation to select an individual for duties is conditional upon the receipt and review of an original Criminal Record Check. The applicant must obtain a Criminal Record Check through a local police service in the area in which they reside. Police Services usually require the applicant to complete a form and provide one or two pieces of photo identification before they access the CPIC (Canadian Police Information Centre) database electronically. Searches completed by any private company or organization other than a police service will not be accepted.

Please be advised of the following:

1) Processing times with police services can be quite lengthy. Some searches can take in excess of 12 weeks to complete.

2) Some police services include information regarding police contact under the Mental Health Act as part of the VSS process; others do not. The Peel District School Board treats all information it receives through the Criminal Record Check and VSS process consistent with its obligations under the Ontario Human Rights Code. Unless and until the Board policy is changed, the applicant is not specifically required by the Board to disclose information regarding police contact under the Mental Health Act.

3) Many police services can provide the applicant with multiple originals if requested at the time of application (if required e.g. student placements/service providers).

Should the applicant submit a Criminal Record Check showing evidence of a police record, confirmation of his/her duties will be postponed pending a review of the information. A police record will not necessarily disqualify the applicant. The Peel District School Board reserves the right to request the applicant to provide additional information with regard to his/her application for duties. The final decision will be made by the Director of Human Resources Support Services in consultation with the “Criminal Record Check Management Review Committee”.

• WHMIS

• Workplace Violence and Harassment

• Ladder Safety (applicable to all staff who may be required to use a ladder)

• Sabrina's Law

• Standard Precautions/Bloodborne Pathogens (required for all staff who may come into contact with bodily fluids)

• Accessibility Note: There are different websites for Accessibility. No special ID or password is required, just enter the requested information (use 00000 for the employee number) and hit submit to begin the course.

Waterloo Region District School Board

Police Check Vulnerable Sector Screening: You will be required to submit a

photocopy the first week of classes (further instructions for where to submit will be sent in August). If both fall and winter session placements are with Waterloo Region then the vss check is valid for the school year. If only the winter session is with Waterloo Region then you must reapply for new vss check. Keep your original in a safe place and present it to your placement school.

Health and Safety Training Requirements:

For the first site visit, students should bring the following completed documents: • HR-16-101 – Terms and Conditions Form for UPSP.pdf, or equivalent, • Original Police Vulnerable Sector Check, dated within the previous six months, • A copy of HR-16-100 – Orientation Checklist for UPSP.pdf with sections A and B completed. Candidates can expect to meet with a school principal, or designate, upon arrival to provide the aforementioned documents and complete the remainder of HR -16-100 – Orientation Checklist for UPSP.pdf to finalize compliance expectations.
