BMMS Student Incident Reporting Form
This form is to be used to report incidents of harassment. If someone is harassing you, or if you witness someone being harassed, please provide the information requested below. You do not have to leave your name. However, know that if you do choose to leave your name, it will be kept strictly confidential and only be used if Mr. Owens or Ms. Bell-Jefferson have any questions.

• I-Ready (K-8 Universal Screener)
• Students performing 2 or more grade levels below cohort further screened via Reading and Math Diagnostics:
– ELA: Letter Word Identification, Word Attack, Fluency and Passage Comprehension,
– Math: Key Math 3 Diagnostic: Numeration, Addition/Subtraction, Multiplication/Division, Foundations of Problem Solving/Applied
• Students receive this AIS via 40’ classes everyday, no more than 6 students

• 7th grade:

• 90 or above Math average and or 3 or 4 on state assessment invited to perform aptitude assessments
• Strongest performing students offered Math 7 Honors
• Spring of 7th grade, students perform High School developed,
Algebra Readiness assessment to determine 8th grade math placement

• Wellness workshops• These activities are meant to be introductory, exploratory and fun. No prior
experience is necessary.
• We are open to suggestions from students for additional activities
• These activities are open to all students grades 6-8
• The Calendar of Events is posted outside the Counseling Center. Students can listen to the announcements and check the website for dates and events as well.
• Examples of activities include: Make natural beauty products, elementary school PE games, cooking, world drumming, mindfulness/meditation, recycle old tee-shirts, make stress balls, chess, jogging, Ultimate Frisbee, dodge ball.
Grade Reporting
• Numeric grading for all classes
• Report cards are available on the parent portal at the end of each quarter.
• Progress reports are available on the parent portal at quarter midpoint.
