What is transition planning?

• Transition (noun) 1. movement, passage, or change from one position, state, stage, subject, concept, etc., to another; 2. change: the transition from adolescence to adulthood.

• Transition Planning - Making and carrying out a very intentional plan for the change to your adult life in the areas of living, working and post–high school learning.

What is Transition Planning in High School?

• Outcome-oriented process focusing on what is needed for adult life functioning.

• Includes a coordinated set of activities, provided to the student by the school and sometimes other agencies, to promote a successful transition from high school to postsecondary education or employment, and independent living.

An Ideal High School Outcome: You eventually run your own IEP Meetings with your teachers, parents and agencies as your partners. You know what you need to meet your career / life goals and how to get what you need to be successful.

adult learning and independent living• Find out your level of those skills • Find out how you can build skills and make

a good match of your skills and life options in the areas of earning, learning and living.
