The Hands of ExperienceWith 30 years experience under his belt Dato’ Dr. A. Jalil Bin Jidon, Consultant Plastic and Reconstructive Surgeon, makes his move to theBeverly Wilshire Medical Centre.

Consultant Plastic and Reconstructive Surgeon, Dato’ Dr. A. Jalil Bin Jidon, hasbeen practising medicine for 30 years, 25 of it as a plastic surgeon. This is a watershed year for the Ferrari and rockmusic loving (“I have 5000 heavy metalsongs on my iPod”) surgeon. After 13 yearsin government service followed by 17years in a hospital in Ampang, Dr. Jalil willbe moving to the all­new Beverly WilshireMedical Centre, a centre that will see himalmost entirely in an aesthetics practicefor the first time.

What many people don’t know is thatduring his housemanship all those yearsago, Dr. Jalil wanted to specialise in paediatrics. “But when I became a MedicalOfficer, we did a rotation of the differenttypes of surgery. I was drawn to plasticsurgery. I was told that not everyone canbe a plastic surgeon. You have to be patient and have the talent to imagine

what the end result will be. A general surgeon will do five stitches when sewingup a large wound whereas a plastic surgeon might do 50,” he explains. Dr. Jalilclearly did – and does – have the patienceand skill for this specialty.

“Over the past 10 years, 70 percent ofmy workload has been in cosmetic surgery,”Dr. Jalil says, explaining that the reconstructivework was part of being in a full­fledgedhospital setting. The move doesn’t meanthat he will be turning his back on reconstructive work entirely. He envisagesthat he will still do it “upon request”. Buthe enjoys cosmetic surgery.

“I do cosmetic surgery from Monday toFriday, usually two to three cases a day,”he adds. “I currently get two or three foreignpatients a week, and they tend to travel

here for multiple procedures so I cansometimes be operating for up to 10 hourson one patient. In terms of safety, thenumber of hours that I operate on a patientisn’t what matters so much as the conditionand health of the patient to start with. Mostof the patients who come to me are fit.”

In fact, Dr. Jalil’s primary concern is thesafety of the patient. “At consultation, thefirst thing I do is to send the patient for afull check up. If the patient is under 35, wewill do a blood test as a minimum. Patientsover 35 will get a full check up – bloodtests, blood pressure, a stress test or ECG ifnecessary. If the patient has a specific medicalproblem, he/she will be assessed by the appropriate doctor and the anaesthetistfirst to ensure that the condition is wellcontrolled before surgery.”

silicone implants were the popular option for these procedures but nowadays dermalinjectable fillers are proving to be the preferred method for body enhancement.

Breast Augmentation With FillersAlways refining his skills, Dr. Liow regularlyattends clinical workshops and trainingaround the world to fine­tune his techniques.His most popular treatment is breast augmentation with fillers. He finds thathealthy women in their late 30s to early 40sor those still of child­bearing age are suitablecandidates for this procedure.

“Many mothers opt for breast feedingnowadays. However, following that they tendto get breast engorgement as well as saggingbreasts,” reveals Dr. Liow. “Whether theywish to add fullness or to slightly elevate their

breasts, hyaluronic acid fillers can be utilisedto suit each individual’s needs as the fillersoffer the volume and shape that patients desire.” He adds that the procedure is short(about one to one­and­a­half hours) and involves no downtime so patients can leavethe clinic immediately after the procedure.

Dr. Liow says that patients are alertthroughout the whole procedure and able toassess the results once the procedure is complete. Immediately after, he advises patients to wear a support bra to support thenew size and shape of the breasts. “For thefirst two weeks, to avoid tissue swelling,

patients cannot do vigorous exercise or go toa hot sauna, spa or jacuzzi. After that period,they can return to their normal activities,”notes Dr. Liow, adding that results last for upto a year as hyaluronic acid dissipates andneeds to be topped up. Hyaluronic acid is asafe substance that is used extensively forface rejuvenation, enhancement and sculpting.It is a substance that dissipates naturally withtime.

Besides the breasts, Dr. Liow also usestemporary (hyaluronic acid) or semi­permanent(hydrogel) fillers to sculpt the whole face.“Using the fillers, I shape the chin, nose, eyebrows and cheeks to provide more profileand sharper features. The fillers can also addfullness to the lips and soften nasolabialfolds,” he explains. Dr. Liow says fillers areparticularly useful in shaping the nose. “Thenose is usually a very sensitive area, so I injecthyaluronic acid filler mixed with lidocaine(anaesthetic solution) using a cannula to prevent patients from feeling pain. It is acomfortable procedure and patients experienceno bruising afterwards,” shares Dr. Liow, statingthat results can last from 12 to 18 months.

Modern Dental TechnologyAccording to Dr. How, cheaper costs, doctors’expertise and skills and their meticulous attention to detail and services are luringmore international patients to obtain dentaltreatments in Malaysia. He believes the availability of high­tech equipment and facilities is also an important factor in this rising attraction. “Our investment in the latest dental technology and facilities supports us in performing a very complex,high level of work. A recent addition to the centre is thePlanmeca ProMax Cone Beam 3D, a revolutionary digital system that capturesthree­dimensional, high quality images ofthe oral anatomy, face and jawbone. The system uses very low radiation (about onetenth of the normal dose used in conventionalX­ray machines) ensuring patients’ safety

during diagnosis. “With this system, we canselect the areas we want to capture, therebyhaving more control on radiation. So far, noother imaging system is capable of that,” reveals Dr. How.

Effective and Safe TreatmentsDr. How always employs the newest andsafest techniques to guarantee patients’comfort during treatments. He regularlyutilises the Waterlase iPlus from Biolase, theadvanced all­tissue dental­laser technology,to perform bloodless gum shaping and recently integrated a new sinus lift techniquedeveloped in South Korea.

“Unlike traditional sinus lift procedures,this latest sinus lift kit allows us to lift thefragile sinus membrane safely, leaving patients with minimal swelling and ready forimplant placement. This takes 30 minutes tocomplete, compared with conventionalmethods which take two hours,” notes Dr.How, who also employs an atraumatic extraction system from Germany which preserves the gum and socket during toothextraction, without causing pain and swellingto patients.

Additionally, the centre’s operating theatre is equipped with an autofocus camerathat records procedures as they are beingdone, which is convenient for educationalpurposes. Thanks to this equipment, 30 orthodontists recently watched Dr. How performing surgery via a live feed on a screenin a separate room.

According to Dr. How, women’s teethare sharper, smaller and more defined thanmen’s, whose teeth are usually rounder andbroader. Women also display more of theirupper incisors (the first four front teeth)than men. He takes these aspects into account when designing teeth based on thepatient’s gender. Dr. How says patients canalso customise their teeth to complementthe shape of their body.

“We design patients’ teeth in such away so they are proportional to the patient’sface and body. Tiny­sized teeth on skinnyfemales will make them look smaller, so wemake the teeth slightly broader to balancetheir appearance,” he elaborates, sayingthat he also considers the patient’s eyes,nose, cheeks, chin and neck when creatinga smile. “We are meticulously detailed inour work to tailor to a patients’ appearance.”

Furthermore, Dr. How reveals thatmany European patients come to the centre requesting to look like Asians. “Europeans generally have teeth that aretoo straight and upright. They have flat lipsand always display dark buccal corridors(the dark space between the corners of themouth and upper teeth). Asian teeth areusually tautly angled and they have morecontoured profiles. We try to take these elements and implement them in Europeanpatients by modifying their incisor display,reshaping their teeth, or repositioning theirjaws if necessary,” shares Dr. How.

For patients with uneven gums, Dr.How often sculpts the gum line to enhancetheir smiles using the Waterlase iPlus, theall­tissue dental laser. Unlike a knife, thelaser cuts in smooth curves without causingany bleeding. “Cutting gums with a knifedamages a lot of cells and causes massivebleeding, making it difficult to see duringtreatment. With the laser, the procedure isbloodless and results are more beautifuland predictable. The cost is cheaper too, asit shortens the treatment time,” notes Dr.How.

For patients who are undergoing smile

reconstruction, Dr. How provides temporaryteeth, made from composite materialswhich look like real teeth, that patients cantry out for a week. Patients can requestchanges.

Dr. How combines patients’ wishes withwhat he thinks is ideal and unique for them,further refining the temporary teeth. Thisprocess continues until patients are fullysatisfied with the results and are committedto a final look.

“We take pictures of patients before,during and after treatment each time theycome here. By the end of the final treatment,200 to 300 photos have been taken andgiven to patients for free, so they can appreciate the step­by­step progress towards getting their new smiles,” says Dr.How.
