HOSPITAL CARESArkansas Heart Hospital recently
received the Medical Partners International 2009 C.A.R.E.S. Designation. The acronym stands for Compassion, Attitude, Respect, Environment and Service. The hospital is the only healthcare organization recipient in Arkansas. The designation takes into consideration nurse care, doctor care, dietician care and environmental service group care. The University of Arkansas for Medical Sciences (UAMS) received a $19.9 million grant — the Clinical and Translational Science Award (CTSA) — from the National Institutes of Health (NIH) and became one of only 60 institutions that will form a consortium with the “aim of translating basic science discoveries into speedier treatments and cures for patients.”
The grant will, in part, establish new projects and benefit several of UAMS’ ongoing projects, such as telemedicine and the UAMS Center for Clinical and Translational Research. Dr. Debra Fiser is dean of UAMS College of Medicine; she said the goal of the CTSA is to speed the application of research.
“Historically, it’s taken a long time to translate science to medical practice. Our primary focus is to speed the process from lab to use, from bench to bedside.” Their goals include, according to Fiser: “speeding up the regulatory process; increasing the training of scientists; enhancing collaborations and the health of our community, state and nation through partnerships; and to encourage T1 research.” T1 research seeks to move basic discovery into candidate health application.

UAMS RECIEVES $20M
NEW HEART NETWORKNorthwest Health System recently es-
tablished The Heart Hospital Network. The network will use virtual technology to inte-grate heart-health services at the system’s Springdale and Bentonville facilities as well as the Siloam Springs Memorial Hospital.
Under The Heart Hospital Network, doctors will use a team approach unifying the various departments, such as cardiac catheterization labs, surgery and surgery re-covery departments and the coronary care and intensive care units, needed to care for cardiovascular patients from Arkansas, Oklahoma and Missouri.

St. Bernards Healthcare in Jonesboro, Ark., has been reaching out to women in surrounding communities since the initiation of their mobile mammography program in 1998. The program is one of only two mobile units in the state that offers women the choice of digital mammography, which is considered superior to traditional film mammography in many ways.
“The digital mammogram is more sophisticated, much like comparing the images of a digital camera with a film camera. The digital images can be manipulated, and dark areas can be zoomed in on for closer inspection,” said Marti Whitehead, RN and service nurse educator at St. Bernards.
The mobile unit serves 17 counties in northeast Arkansas and travels to sites as close as a 10-minute drive and as far as two hours. “We mostly visit industrial sites, rural physician’s offices and community screenings,” Whitehead said. “Many of our patients would never get a mammogram screening if it wasn’t for us. I think we do the most good at industrial sites. The women don’t have to take time off from their job — they just step off the line to get the screening. Most of them don’t get a lot of vacation time or don’t have a means to travel to a doctor’s office."
The mobile mammography unit’s primary function is mammograms, but they offer osteoporosis screenings and a heart advantage program as well. The mobile unit’s team performs approximately 1,200 mammograms each year and travels two to three days a week.
MAMMOGRAMS TO GO
Arkansas Children’s Hospital (ACH) has long been a forerunner among the state’s medical facilities, and programs, such as their PULSE Center, illustrate why.
The PULSE Center — Pediatric Understanding and Learning through Simulation Education — offers state-of-the-art training for medical professionals with the use of life-like figures. There are several manikins including SimBaby, SimNewbie and SimTeen; the simulators “perform” a number of human-like actions, such as breathing and heart sounds and can receive fluids via IV, be intubated, shocked with defibrillators and much more.
Doctors, nurses, respiratory therapists and others receive instruction for situations including coding, scenarios such as drug and allergic reactions and other medical circumstances. According to statistics from the Centers for Disease Control and Prevention, Arkansas ranks a jaw-dropping third highest among all states in stroke deaths, with 61 per 100,000 residents. Through the newly-established Arkansas SAVES (Stroke Assistance Through Virtual Emergency Support) program, which was made possible by partnerships between the University of Arkansas for Medical Sciences (UAMS) Center for Distance Health; the state Department of Human Services Division of Medical Services; and Sparks Health System in Fort Smith, physicians hope to see our stroke statistics take a turn for the better.
The SAVES program, which is administered by the UAMS Center for Distance Health, began in November 2008, and currently links nine rural hospitals to stroke specialists at UAMS and Sparks Health System.
Stroke specialists are able to connect with a patient via two-way, real-time video. They confer with physicians, examine the patients through the nurses on hand and can examine CT scans before rendering an opinion.
The biggest asset for a patient to participate in the Arkansas SAVES program is that they have the possibility of being in-touch with a stroke specialist who can prescribe the use of the powerful clot-busting drug T-PA.
T-PA must be administered within the first three hours of a stroke. “T-PA makes a huge difference in the outcome of a patient; there is a 20 to 25 percent chance the patient will greatly improve after taking the drug. T-PA can melt away the blood clot so irreversible damage may not happen; however, there are very powerful side effects, such as brain hemorrhaging, that can occur in a small percentage so many physicians are very reluctant to use it,” said Salah Keyrouz, M.D., stroke neurologist at UAMS.
“Statistically, patients are more likely to do well with the drug, and I want to be able to give it to more patients.”
Nine hospitals currently participate in the SAVES program including: Baptist Health Medical Center – Arkadelphia; White River Medical Center in Batesville; Baxter Regional Medical Center in Mountain Home; Booneville Community Hospital; DeWitt Hospital; Johnson Regional Medical Center in Clarksville; McGehee-Desha County Hospital; Helena Regional Medical Center; and Mena Regional Health System. Program administrators hope to add nine more by June 2010.

Arkansas is preparing to enter a new arena in healthcare after legislation passed in early July to allow budgeting for a statewide trauma system. The trauma system will be funded by state revenue from an approved increase in tobacco tax. The system, which is much overdue — Arkansas was one of only three states in the country without a trauma system when the legislation passed — will include not only the funding to implement trauma centers in hospitals throughout the state and surrounding states that treat Arkansans, but many other necessary components as well.
“The trauma system has many pieces besides the trauma cen-ters such as a call center for routing, support for Emergency Medical Services to be more efficient and a trauma registry to develop the quality and track improvements,” said Donnie Smith, director of the Arkansas Department of Health Center for Health Protection.
Thus far, 68 hospitals have submitted applications for trauma center funding. Trauma centers range in capability on a scale of level 1 through 4, level 1 providing the highest level of care and abiding by the strictest requirements.
The purpose of a trauma center is to provide immediate care after an incident such as a gunshot wound, car accident or other highly traumatic injury.
“National studies show that when severe injuries are treated in a trauma center, survival rates increase by 25 percent,” Smith said. He hopes each hospital that stated a desire for a trauma center will receive funding to attain a status appropriate to their size and loca-tion. “The legislation for this only passed in July. We are still hoping to get in more applications and will begin to work on putting the dif-ferent pieces for a trauma system in order. We hope that within two to three years, we will have a mature system in place.”

KANSAS HOSPITAL ASSOCIATION: The American Hospital Association's agreement to forego the $155 billion in payments is between the Federation of American Hospitals, the Cath-olic Health Association and the American Hospital Association [which includes the Ar-kansas Hospital Association].
The impact on Arkansas hospitals should, in the long run, be positive. The bill as it is should provide about 95 percent of the popu-lation with medical coverage, which we des-perately need because the current system of the number of uninsured patients is not sus-tainable.
And IF there is a public option in the Sen-ate Finance Committee bill, the public option would not pay Medicaid and Medicare rates. The Arkansas Hospital Association does not want a public option that pays these rates; they are not sufficient; but we can live with the ne-gotiated rate.
[On July 15,] another bill came out of the Senate, so understand that what we see today may be not the final product … we are a long way from getting where we want to be.
In regards to your question, was the agree-ment to forego the payments an attempt to avoid additional and/or wide-sweeping reduc-tions? The answer is yes, absolutely. That’s the way legislation is. If the number of insured individuals continues to grow, rates will keep going up and companies will drop insurance coverage and this exacerbates the number of insured.
Private health insurers also have a stake in this issue, thus we asked our state’s largest insurer, Arkansas Blue Cross Blue Shield to weigh in on the issue.

BLUE SHIELD: Certainly we could have a gov-ernment plan that exists alongside private plans. We have those today. We hope that any government plan under consideration in Washington will have to meet the same re-quirements as private plans. Anything less
could undermine real reform and significantly change coverage options. At Arkansas Blue Cross, we strongly support meaningful health care reform. To be effective, reform must re-duce the cost of coverage for all Americans. We believe that is best achieved by building on the current system and providing a safety net for those Americans who are unable to provide for themselves. It is important that our leaders in Washington recognize that they cannot solve the access issue without first re-solving the cost issues. Our focus must be on eliminating waste, paying only for health care that is proven effective, revamping how (not how much) we reimburse healthcare provid-ers, improving how the health care system operates through technology and encouraging healthy lifestyles. As an organization, we are prepared to make whatever changes are neces-sary to our business model in support of these types of initiatives because we believe them to be in the best interest of all Americans.
As the debate in the Senate is being waged along party lines, we asked Arkansas’ party lead-ers to voice their opinions.
DOYLE WEBB, CHAIRMAN OF THE REPUBLICAN
PARTY OF ARKANSAS: The healthcare reform bill is certainly a complicated and emotional topic that everyone agrees needs to be ad-dressed. As you would assume, both sides of the isle have their own ideals how to make this government-run insurance option feasible. The main concern with Republicans is how to fund this kind of program without raising tax-es and without increasing the federal deficit. The Obama administration and Democratic majority in Congress have proposed funding the program a number of ways, but it primar-ily involves a $550 billion tax hike, and cutting Medicare payments to medical providers. I personally fear a centralized government program that has too much control over medi-cal decisions will lead to even more red tape and inefficiencies within the system. I find it difficult to comprehend a program that would require the creation of yet another federal de-
partment and be so complex and expensive to operate, that it could be done without causing delays in medical treatment, without losing money, and sacrificing quality of care. The last thing this country needs is a European style health care system where medical delays are common and expensive.I only hope that during the ongoing discus-sions in Washington and Congress that the good of the public will be the deciding factor in any program that is approved, and not just what’s good for the respective parties.

PARTY OF ARKANSAS: Too many people in our state and nation can’t get the health care they need because they can’t afford it. Even more families must choose between basic necessities and the skyrocketing costs of health care cov-erage. When people without insurance have to be treated in an emergency room, everyone else ends up paying for it. It is a vicious and expensive cycle. This isn’t a problem that de-veloped overnight, and it’s not going to be easy to fix. The recent decision by hospitals to fore-go future Medicare and Medicaid payments illustrates their belief, as major stakeholders in the health care industry, that we are in dire need of reform and now is the time to begin. To be successful, major health care reform must lower costs, preserve patient choice and expand access to quality, affordable care to all Americans. It should be a deficit neutral plan that may include a public insurance option and the creation of a health insurance exchange. To preserve what’s best about our system, we have to fix what’s broken and build on what works. In some cases, there’s broad agreement — most people agree that we should use elec-tronic medical records to reduce errors, save lives, save money, while still ensuring privacy, and we should invest in prevention and well-ness programs. I doubt these reforms would be paid for by taxing benefits workers receive from their employers. I believe it is very un-likely at this point to be part of the final re-form package.
When Pres. Obama took office this January, one of the first hurdles he faced was healthcare reform. According to statistics from U.S. Census Bureau, 47 million Americans do not have health insurance. While most Americans agree, in principle, that every child and adult should have health care, debate ensues when one considers how to provide the care and how to pay for it. Reports in July that the American Hospital As-sociation agreed to forego $155 billion in Medicaid and Medicare payments to help offset the cost of an administration-backed plan along with reports of the Democratic and Republican parties being split led us to ask several state leaders their opinion of what has taken place thus far.
