2018 Health Insurance Opt Out
If you are currently opting out of medical insurance and receive an opt out allowance, you must submit your proof of other health insurance coverage to Human Resources on an annual basis. Acceptable proof of other health insurance coverage is a copy of your other health insurance plan’s identification card or a certificate of other health coverage. The proof of other insurance must include your name. In order to continue your medical opt out allowance for 2018, either one of these document copies must be submitted to the HR Department no later than January 8, 2018.

Deferred Compensation
Contribution Change Forms received by December 28, 2017 will be effective the first payday in January. Please indicate
January 2018 as the effective date on the form.
Age 50 Catch-Up – Allows participants who attain age 50 (or older) during the year to contribute an additional amount
over the regular deferral limit in effect for the year. Regardless of which month you turn 50, you may sign up for this
provision to begin in January 2018 if you so choose.
Complete the Contribution Change Form
Submit the completed form to H.R. or [email protected].
Pre-Retirement Catch-Up – Allows eligible participants to contribute an additional amount over the regular deferral limit in
effect for the year (up to double the regular limit), to make up for the years in which they did not contribute the maximum
amount they were eligible to contribute under the City’s plan.
Complete the Pre-Retirement Catch-Up Form
Submit the completed form to [email protected] or any Def. Comp. staff who will determine if you qualify.
NOTES:
1. Please note that the “Age 50” and “Pre-Retirement” catch-up provisions may not both be used in the same
calendar year.
2. You cannot make catch-up contributions in the year of your declared NRA.
3. You are not required to make catch-up contributions in all three years leading up to the year of your declared
NRA.
4. You are not required to retire in the year of your declared NRA.
5. The pre-retirement catch-up provision can only be used one time with your current employer.
*NRA – Normal Retirement Age can be any year during or after which you are eligible to receive unreduced benefits from
CalPERS, but cannot be later than the year that you attain age 70½.

Saving Forward: Symptoms of Financial Wellness
In the last issue of the Quarterly News, we asked readers what financial wellness
means to them. Nearly 20 percent said it means reducing or eliminating debt;
about 10 percent cited long-term saving in their retirement account; and 10
percent said living within or below their means. And more than 60 percent said
it means all of those things. Here are a few steps that can work together to help
you manage your money now and reach your financial goals.
Track your spending. Monitoring your spending for a month may make it surprisingly easy to see ways to cut
back. Doing so can free up money for other goals.
Minimize high-interest-rate debt. Spending a lot of money on interest each month can make it much more
difficult to reach the rest of your goals. Make paying down high-cost debt one of your top priorities.
Build your emergency fund. This is a key to avoid landing in debt to cover unexpected expenses, such as car
repairs or a leaky roof.
Check your credit report. A good credit record can help you save money on a mortgage, a car loan, and car
and home insurance. You can check yours for free every 12 months at www.annualcreditreport.com.
Assess your progress. Use the retirement-savings calculator at www.icmarc.org/ontrack to see if you’re on
track to reach your retirement goals.

2018 Annual Excess Sick Payoff Deferral Form
If you have over 175 hours of sick leave at the end of 2017, you will automatically be paid hours above 175 on payday January 5, 2018 unless you elect to defer this payment into your ICMA-RC Pre-tax or After-Tax account. Please submit this form to the Human Resources office by December 13, 2017 if you elect to participate. All forms (scanned or faxed) will not be accepted after 5:00 pm on December 13th to Human Resources: [email protected] or [email protected] or fax to (714) 765-5211.
2018 1st Quarter Deferral for Vacation & Paid Leave Form
Please submit this form to the Human Resources office by December 31, 2017. You may elect pre-tax for tax shelter purposes (now) or after tax ROTH (to obtain tax benefits at time of distribution). The default for those that do not make an election will be to your pre-tax account. To receive CASH payment of vacation/paid leave hours requires a form from the Payroll division.

Completing Your Retirement Application

This is an alternative to a one-on-one retirement counseling appointment for those within 90 days of retirement. Bring your application and a copy of your Retirement Estimate, that you can order online or by phone.
