What will consumers be able to do using the app?

Once they’ve downloaded Health4Me, consumers will be able to easily find the information including:

• Register for myuhc.com, enabling full use of all of the app’s functionality

• Download and view their health plan ID card, and be able to email or fax it

• Quickly and easily find urgent care and emergency room locations nearest them

• Search for network physicians and facilities, call them, share them via email or text, view their location on a map, get driving directions and save their favorite physicians to their phone with notes

• Call NurseLine (if offered) to speak with a nurse about health concerns

• Use a brand new feature called EasyConnect (if offered) that enables them to quickly navigate the phone prompts before calling, then let UnitedHealthcare know to call them back about the specific issue they’ve submitted

• Check their network and out-of-network deductible and out-of-pocket benefits and see how much they’ve accumulated year-to-date

• Download their claims and view in an easy-to-understand format

• Check on their financial accounts associated with their UnitedHealthcare plan including viewing their OptumHealth Bank HSA balance

The UnitedHealthcare app provides secure, on-the-go access to personalized health plan information, helping members better understand and use their health plan effectively. Members are able to use the app to:•Find care and costs, including virtual care.•Review plan information, including deductibles and copays/coinsurance.•Access Optum Bank® financial accounts.•Check on the status of claims and pay bills.•Access pharmacy information (coming Q4 2018).•Contact UnitedHealthcare with questions (including 24/7 nurse support).

The new app will align all technologies to create a common look, feel and experience.Personal information is protected by HealthSafe ID®, and members receive customized information (such as provider search results and account balances) based on their specific location and plan benefits. The goal is to help members build their health ownership and reduce their health care costs.

Care24 connects employees to help when they need it.

The NurseLine component provides access to nurses 24/7 via telephone or online chat.

Nurses address a caller’s questions and also can provide information on providers, medications, and refer them to programs to help them achieve a health lifestyle.

• Treatment: Symptom support, self-care information, referrals to health and wellness programs, condition management and treatment information.

• Provider: Identifies UnitedHealth Premium® Program providers (Advocates can schedule appointments).

• Medication: Coaching on medication adherence, education on drug interactions or medication alternatives.

• Lifestyle: Preventive care information, healthy lifestyle coaching, and referrals to wellness coaching and behavioral health.

The EAP component supports both individual employees and the overall workplace. Employees get confidential* support with:

• Stress, depression and anxiety.

• Personal financial and legal advice (free 60-minute consultation with a financial professional; free initial legal consultation with certified attorneys and mediators).

• Parenting and family issues, including domestic violence.

• Substance use and recovery.

National network of providers to support members in their behavioral health needs. Members will talk to an “Intake Coordinator” who will help to triage them to the best provider. Private and confidential and most effective way of finding a behavioral health specialist.

Provider types:

Masters Level Clinician

Masters Level Clinicians have a Master's degree in one of the following Mental Health disciplines: Social Work, Nursing, Professional Counseling or Marriage and Family Therapy. These individuals are independently licensed in the state in which they practice. They provide various services including assessments and psychotherapeutic interventions.

Master's Level Clinicians provide therapy for many basic needs; Master's clinicians work with patients to address concerns and root causes. They can see you alone, as part of a couple, or with your entire family. In some cases, they may recommend that you consult with a psychiatrist or nurse practitioner to evaluate whether medication could contribute to achieving the best outcome. (Most therapists can suggest trusted psychiatrists or nurse practitioners to you for this purpose.)

Psychologist

Psychologists generally have doctoral degrees in the area of educational, child, clinical or counseling psychology. Psychologists provide a variety of services including assessments, psychological testing, and most offer psychotherapeutic interventions. Some psychologists have specialties in the area of treating children and adolescents.

Psychologists offer testing that can help answer questions about what may be causing certain symptoms; often, a Masters clinician or Psychiatrist will refer to a psychologist for such testing if they believe it is right for a particular patient. Psychologists can also perform therapy.

Psychiatrists

Psychiatrists are MDs that specialize in psychiatry. They provide psychiatric evaluations, prescribe psychotropic medications and may offer psychotherapeutic interventions. They are licensed by the state in which they practice and most are Board-Certified in their specialty. Some psychiatrists have sub-specialties in the area of child psychiatry, adolescent psychiatry, addictions or geriatrics. If you are interested in a psychiatrist who evaluates and treats children be sure to inquire as to whether or not they have been trained in child psychiatry.

Psychiatrists are medical doctors with a specialty in psychiatry. Most often they conduct a thorough evaluation and prescribe medications, then monitor the effects of medications. Some psychiatrists also conduct therapy.

Levels of Care

Inpatient: An acute inpatient unit is a secured and structured hospital-based service that provides 24-hour nursing care and monitoring, assessment and diagnostic services, treatment, and specialty medical consultation services with an urgency that is commensurate with the member’s current clinical need.

Partial Hospitalization: A partial hospital/day treatment program is a freestanding or hospital based program that maintains hours of service for at least 20 hours per

week, and may also include half-day programs that provide services for less than 4 hours per day.

Residential Treatment: Residential treatment services are delivered in a facility or a freestanding Residential Treatment Center that provides overnight mental health

services to members who do not require 24-hour nursing care and monitoring offered in an acute inpatient setting but who do require 24-hour structure.

IOP( Intensive outpatient therapy): An intensive outpatient program (IOP) is a freestanding or hospital-based program that maintains hours of service for at least 3 hours per day, 2 or more days per week.

Outpatient: Outpatient care consists of visits provided in an ambulatory setting for the purpose of assessing and treating a mental health condition.
