CASE PLAN INDIVIDUAL CLIENT RESPONSIBILITIES

CASE PLAN SERVICE OBJECTIVES AND CLIENT RESPONSIBILITIES

Danger Statement: The Agency, Mike and Jenna are worried that Jenna may get drunk again while she is alone with Stephen, that Stephen could get into her pills, and could get sick or seriously hurt. Safety Goal: Jenna agrees to make a plan with Mike and Sharon to show everyone that she will always be sober when she is caring for Stephen. Mike, Sharon and Jenna’s best friend, Monica will work with Jenna to ensure she is following the plan. This plan will need to be in place and working continuously for six months for the CFS to consider closing the case.

Date 1. Stay free from alcohol and show your ability to live free from

alcohol dependency Objective: Jenna agrees to demonstrate that she can remain sober for the next 6 months and that she has followed a plan to maintain her sobriety.

1. Jenna agrees to work with her sponsor to develop a plan to maintain her sobriety. She agrees to have the plan developed in the next 60 days and will refine it for the following 4 months. She will share the plan with her network and social worker.

2. Jenna agrees to tell her network 3 things that have helped her stay sober in the past. Jenna and the network agree to develop ways to help Jenna continue to do the things that have worked.

3. Jenna agrees to tell her network what she does when she is thinking about using and agrees to develop ways for her network to help her during those times.

2. Stabilize Behavioral HealthObjective: Jenna agrees to develop 3 ways to manage and control herdepression and will demonstrate those skills to her therapist andnetwork for the next 6 months.

1. In the next 30 days, Jenna agrees to meet with a therapist anddevelop a plan together.

2. Monica or another network member agree to check in every dayand ask Jenna how the plan is going. If this does not work,Jenna will work with the social worker to develop another wayto meet her objective of managing her depression.

3. Jenna agrees to re-evalutate her medication in the next 60 daysand agrees to demonstrate that she can follow that plan for thenext 6 months.

4. Jenna agrees that if she is starting to feel depressed to the pointthat she does not want to do anything, she will call Monica,Sharon or her dad, and they will come over right away.

1. Jenna agrees to invite AA sponsor to join network in the nextmonth.

3. Jenna agrees to come up with a list of people who might besupports for her and activities that will help her build hernetwork.

4. Jenna agrees to invite one new person to her network meeting.

1. Monica, Mike and Sharon agree to take turns watching Stephen when Jenna goes to see her therapist. They will also help with transportation if there is a problem.

2. Jenna agrees to work with the social worker and her network on her plan to manage her depression and let her social worker know what is working for her

3. Jenna agrees to meet with a psychiatrist to evaluate the use of meds for her depression.

Drug and Alcohol Services 2. 06/04/2015

Description Jenna agrees to attend an AA meeting 4 times a week. She will let her network know when she needs help with child care or transportation.

1. Jenna agrees to work with sponsor to meet her goal of completing the 10 steps. 2. Jenna agrees to get random drug tests when requested by her social worker. The

network will let the social worker know if they think Jenna might be using.
