Behavior Incident Report (BIR) Instructions 1. If a child begins exhibit challenging behavior in the classroom, teachers should complete a Behavior Incident Report (BIR) immediately after the incident occurred and is under control. 2. Please capture the “key” point of the incident. Following the completion of each BIR teaches must consult with their Center Director (CD) within 24 hours and give a copy of the BIR to them. 3. If a child has three episodes of challenging behavior in a month, and three BIR are completely filled out and have been discussed with the CD, the CD should contact the Early Learning Disability Specialist (ELDS) or DMHC (Clark County only) to determine who will be the lead coach for the classroom and conduct a classroom observation. Always complete a BIR when a child engages in the following kinds of behavior: Aggression to another child or adult that results in physical pain or harm to that person (includes kicking, hitting, biting, scratching) Running out of the classroom, off playground, of from group without responding to the calls of the adult Intentionally injuring self in manner that may cause serious harm (serve head banging, biting self) Also complete a BIR when a child continues to engage in problem behavior despite efforts to redirect to use alternative skills. On these occasions, complete the form for children who are persistent in problem behavior and their problem behavior appears to be unresponsive to the child guidance produces you use in your classroom. The form will not be completed if the behavior has not occurred before or if the behavior may be developmentally expected.
