Click on ‘Search for a doctor or hospital near you’ on the Beech Street website (located in the upper right corner)Follow the 3-step process:

1. Select “Primary” in the plan type drop-down box.

2. Choose how you want to search by selecting ONE of the options: a) By Location b) By Doctor Name c) By Hospital or Facility Fill in the blanks for your selection with the required information.3. Choose what type of provider you want to locate by selecting ONE of the options: a) Search for Doctors (and select a specialty, if desired) b) Search for Hospitals or Facilities (and select a type, if desired) c) Search All (to include all provider types)Click on ‘SEARCH’ to get your results or ‘CLEAR’ to start over.

Create a Customized Directory (not shown)Click on ‘Print a personalized directory’Follow the 4-step process:1. Choose your plan type by selecting from the available options: a) Complementary b) Consumer Card c) Limited Benefi t d) Primary e) Supplemental f) Workers Compensation2. Enter the address where you want to start the search (your home, offi ce, etc.). Indicate how many miles from that location you want to search by using the drop-down menu.3. Select the provider specialties and the facility types of interest to you. Note: Hold the ‘Ctrl’ key down to select multiple specialties and/or facility types.4. Create a title for your directory.

Click on ‘CREATE DIRECTORY’ to get your directory or ‘CLEAR’ to start over.

The directory contains all providers who are participating in the Beech Street PPO network, according to Beech Street’s records. Every effort is made to ensure the accuracy of the information; however, changes occur daily and may not yet be included in the directory. Therefore, it is possible that the provider you select is not currently participating in the PPO network. Please confi rm the provider’s participation by calling the provider directly. The provider you select may not be available to all Beech Street clients due to individual benefi t plan or network restrictions. Additionally, the provider may not offer the Beech Street contracted rate for all services. Please confi rm your benefi ts by calling your Health Plan Benefi t Administrator or Insurance Company. It is Beech Street’s policy to verify the contact and professional information for each provider participating in the Beech Street PPO network every three years.
