Theimportanceofthehumanimmunesysteminfightingcancerwasemphasizedin2011whentheprestigiousNobelPrizeinPhysiologywasawardedtothreerenownedimmunologists:BruceBeutler,MD,andJulesHoffmann,PhD,whodiscoveredhowtoactivateinnateimmunity,andRalphSteinman,MD,whodiscoveredthedendriticcellanditsroleinadaptiveimmunity.
Cellulartherapyofcancerremainsacornerstoneoftreatmentforpatientswithhematologicmalignanciessuchasleukemia,lymphomaandmultiplemyeloma.Sinceitsinceptionin1983,theBloodandMarrowTransplantProgramatBaylorUniversityMedicalCenteratDallashasbeen“wieldingtheimmunesword”inthefightagainstcancer.Withcontinuedrefinement,throughclinicalandbasicresearch,thebladehasbeensharpenedandpotentiallycurativetherapycannowbeofferedtoselectedpatientsintotheir70s(Sorror,etal, JAMA.2011;306(17):1874-1883).
Ifyoutreatadultswithacuteleukemia,itisessentialtoobtaingeneticandmolecularprognosticfactorsatdiagnosistoiden-tifyearlythosepatientswhowillclinicallybenefitfromsubsequentallogeneictransplantation. Modernmethods(FISHpanels,PCRdetectionofmutantgenes)obviatetheproblemswhichusedtoplaguestandardcytogenetictesting.Themostvaluablefeatureofthesemethodsisprobablytheirrobustness,assamplescanbe3–5daysold.Pendingtheresultsoftheseassays,treatmentcanbeginonweekendsandholidays.
Inaddition,westronglyrecommendpatientsundergoHLAdetermination(tissuetyping)andincludetentativedonoridentifi-cationintheirinitialtreatmentplan.Therationaleissimple:adultleukemiaisdifficulttocure,especiallyifaccompaniedbyadversemolecularorgeneticfeatures.HLAtypingandinformaltransplantationconsultationcanbeeasilyarrangedbycontactingourprogramandspeakingwithoneofthetransplantphysicians.
Autologoustransplantationhasalsomadegreatstridesinthepastthreedecades.Duetoitscurativepotential,itremainsthestandardofcarefortreatingrelapsedHodgkin’sandaggressivenon-Hodgkin’slymphoma.Itisimperativethatthephysicianofferingsalvagetherapycloselyworkswiththetransplantteamtointegratechemotherapycycleswithstemcellcollectionandtoavoiddamagingthestemcellcompartment.Consultationbeforestartingsalvagetreatmentprogramsisinthepatient’sbestinterest.
Multiplemyelomaremainsoneofthemostchallengingofdiseases.Whilemodern“targeted”therapywithagentssuchasbortezomib(Velcade)andlenalidomide(Revlimid)areeffective,theyalsoareexpensive,inconvenientandconfersometimespermanentsideeffectssuchasperipheralneuropathy.Treatmentwithhigh-dosemelphalanandautologousstemcellsupportremainsastandardofcare,bothasa“cap”toinductiontherapyandasasalvageprograminrelapseddisease. Generally,transplantationmaynownotonlybecost-effective,butmayactuallybelessexpensivethanprolongedtherapywith“targeted”drugs.Moreover,qualityoflifeandfreedomfromfrequentvisitstothedoctor’sofficeareamajorpotentialbenefittopatientssotreated.
Autologousimmunotherapy—usinggene-modifiedTeffectorcells—hasrecentlyemergedasanovelandtantalizingapproachtochronicBcelldiseasessuchasCLL(Porter,etal,NEJM.2011;365(8):725-733).Whilesuch“high-tech”approachesarenotyetavailableinthecommunitysetting,theyservetodemonstrateconvincinglythepoweroftheimmunesysteminfightingcancer.Extendingtheanalogyjustabitfurther,onecanimaginesimilarapproachesbeingusedtotreatchronicinfectionssuchasHIV,orevenautoimmunediseasessuchasSLE.
