Philosophy: We believe all members of the community deserve confidential access to education and medical testing regarding prevention, transmission, or treatment of sexually transmitted infections. Ensure information and education on the prevention of sexually transmitted diseases are available to all community members regardless of demographics, race, gender or ability to pay. (We believe that empowering individuals with the right knowledge and education can help to prevent the spread of sexually transmitted infections throughout the community.) 2. Ensure all confidential testing and education provided is done in a facility that can be easily accessed by community members. (Confidential access to education and medical testing regarding prevention transmission, or treatment of sexually transmitted infections.) 3. Ensure that all materials, equipment, supplies, and education available take into consideration the diverse cultures and ethnicities of the community to be served. (We believe that empowering individuals with the right knowledge and education can help to prevent the spread of sexually transmitted infections throughout the community.) 4. Ensure that confidential testing and services are available on a consistent schedule so that community members are able to access the services provided. (We seek to provide a safe haven for those who need assistance or want to improve their own sexual well-being and health.) Goals 5. Ensure that all services and education are one to one with a healthcare provider. (We seek to provide a safe haven for those who need assistance or want to improve their own sexual well-being and health.) 6.) Ensure that all care provided is done by health care professionals or individuals that have the necessary education and experience. (We will provide testing, education, and counseling on a one-on-one basis for each client.) 7.) Ensure overall improvement in the community STI infection rate with regards to our services. (We believe that empowering individuals with the right knowledge and education can help to prevent the spread of sexually transmitted infections throughout the community.) 8.) Ensure that more men and women are aware of their HIV status. (We will provide testing, education, and counseling on a one-on-one basis for each client.) Objectives To obtain a building centrally located in a community to house Aware by December 2015 To hand out 100 informational flyers per day for the first 3 weeks into the community to help draw awareness to our services. To complete training for all Aware staff members on STI testing and counseling by the first month of employment, demonstrating competent knowledge in these areas by completing all testing and obtaining an 80% or higher To demonstrate understanding of education received, clients will complete end of education questions getting at least 70% of answers correct. To see 300 STI tests completed on the public in the first year To double the clients served from the community in the first year within three years of Aware being opened. To record a 6% decrease in STIs noted in the community within two years of Aware opening To increase the number of men and women who are aware of their HIV infection status by 10% in the first 5 years. Organizational Job Chart Physician Director of Nursing Assiatant Director of Nursing Registered Nurses ClericalVolunteers Physician Responsible for medically managing the STI Clinic including: assessing, diagnosing and treatment of STIs. Ensure all medical documentation is: complete records are HIPAA compliant confidentiality protocols are maintained for patient record Perform: individualized assessment, diagnosis, and treatment services Physician (Cont) Utilize: laboratory test results and clinical assessment they will determine type and stage of STI appropriate protocols and guidelines as established by the Centers for Disease Control and Prevention the diagnosis and treatment of sexually transmitted infections Clinical tasks are completed by following established policies and protocols such as: Collects blood samples and other laboratory specimens as part of assessment and diagnosis process when required Counsels and administers hepatitis A and B vaccines to eligible clients based on hepatitis A and B laboratory screening results Performs one-on-one education specific to HIV/STI and other communicable disease to patients Provides education to clients regarding their infection and instructions regarding medication treatment and test-for-cure in 90 days Physician Qualifications Full registration and license to practice with the General Medical Council in the State of Michigan Completion of 4 hours post-graduate training Specialize training in sexual and reproductive health Experience or background working with and prevention of sexually transmitted diseases Director of Nursing (DON) Implement organizations goals and mission statement Follow evidence-based practices to support direction of education and services provided to staff and to patients Responsible for the following: Oversight of the finance/budgeting and marketing of the organization Fundraising, grants, and community development of this organization Development of the policies and procedures that help to direct the care of the patients Hiring/Firing of all employees and volunteers Oversee the standard of care and development of the organization Work as a part of the healthcare team to maintain HIPAA policy
