We were inspired by the extent of humble caring volunteering that is occur- ring, and the huge degree of collecve goodwill going into the lives of many socially isolated older residents. Hello Volunteers & Friends

Hello everyone, We have had a busy couple of months here at “the

office”. In June we put together a submission to contin-

ue CVS in our current service region from 2019-21 and in

July our annual performance report was due to the De-

partment of Health. These busy times came on the back

of our mid-year coffee catch ups with visitors and there

was the usual business of setting up new matches for

many visitors and residents. Our apologies if we haven’t

responded as quickly to queries as we would have liked.

Whilst these and the usual seasonal paperwork matters

has kept us somewhat office bound, Shari and I greatly

enjoy connecting with you and hearing how you’re going

with your community visiting role. We were inspired by

the extent of humble caring volunteering that is occur-

ring, and the huge degree of collective goodwill going

into the lives of many socially isolated older residents.

Changes to Police Checks

I had just put FAQ about police checks in the last newsletter and they went and changed how they’re done. My bad luck. So, disregard the information in the last newsletter because it’s changed! As of the 1st of July 2018 there are some changes to how police checks are lodged. The consent form has changed greatly in format but requires basically the same infor-mation. The changes that will most affect your lodgement are the ID requirements. ID no longer needs to be certified (a normal copy, scan or photo will do), but there needs to be a mini-mum of 4 documents submitted. Specifically there needs to be: One Commencement Document e.g. Full Australian Birth Certificate (not an extract

or birth card), Current Australian Passport, Australian Visa etc. One Primary Document e.g. Current Driver’s License, Australian Marriage Certifi-

cate, current proof of age card etc. Two Secondary Documents e.g. Medicare Card, Credit Card, Bank Card I have included a copy of the Documents Required page of the Police Check consent form on the next page which contains the full list of acceptable documents. There are also other new rules affecting ID Either the commencement or primary document need to include a photo of you. If

they don’t you need to submit a certified passport-style photo. Your ID documents need to include your full name and your date of birth. If your ID is under a former name you will need to provide evidence of your name

change, either a change of name certificate or marriage certificate (the official one from the state or territory).

We (CVS) will need to be able to confirm that the ID actually belongs to you. For re-

newals this will be easy as we have the previous police checks and have met you. For new visitor checks, to confirm the ID belongs to the right person, we’ll need to either see you and the ID or get a photo of you holding the ID (so we can read it) as well.

Note: The ‘evidence of right to a government benefit (Centrelink or Veterans Affairs)’ listed in the secondary documents can be a Centrelink or Veterans Affairs card.

DOCUMENTS REQUIRED

HOW TO SUBMIT THIS FORM

Minimum identity requirements

You must provide four documents with your completed

form to confirm your identity:

›› one commencement document to confirm your birth in

Australia or arrival in Australia

›› one primary and two secondary documents to show the

use of your identity in the community.

The Legal Entity Customer listed on page 5 will use these

documents to verify your identity against the personal in-

formation you have provided on this form. The personal

information contained in the identity documents you pro-

vide will be used to conduct a nationally coordinated crimi-

nal history check, as you consent to in Section C.

The documentation you provide must include evidence of

your full name and date of birth and a photograph of you.

The commencement and primary documents provided

must include a photo of you. If your commencement and

primary documents do not include a photograph you must

submit a passport-style photograph certified by a person

listed in Schedule 2 of the Statutory Declarations Regula-

tions 1993 (Cth).

Commencement documents

a. full Australian birth certificate (not an extract or birth

card)

b. current Australian passport (not expired)

c. Australian Visa current at time of entry to Australia as

resident or tourist

d. ImmiCard issued by the Department of Immigration and

Border Protection that enables the cardholder to prove

their visa and/or migration status and enroll in services

e. certificate of identity issued by the Department of For-

eign Affairs and Trade to refugees and non-Australian

citizens for entry to Australia

f. document of identity issued by the Department of For-

eign Affairs and Trade to Australian citizens or persons

who possess the nationality of a Commonwealth coun-

try, for travel purposes

g. certificate of evidence of resident status.

Primary documents

a. current Australian driver licence, learner permit or pro-

visional licence issued by a state or territory, showing

signature and/or photo and the same name as claimed

b. Australian marriage certificate issued by a state or terri-

tory (church or celebrant-issued certificates are not

accepted)

c. current passport issued by a country other than Aus-

tralia with a valid entry stamp or visa

d. current proof of age or photo identity card issued by an

Australian government agency in your name with photo

and signature

e. current shooter or firearm licence showing signature

and photo (not minor or junior permit or licence)

f. for persons aged under 18 with no other Primary Use in

Community Documents, a current student identification

card with photo or signature.

Secondary documents

a. certificate of identity issued by the Department of For-

eign Affairs and Trade

b. document of identity issued by the Department of For-

eign Affairs and Trade

c. convention travel document secondary (United Na-

tions) issued by the Department of Foreign Affairs and

Trade

d. foreign government issued documents (for example

drivers licence)

e. Medicare card

f. enrolment with the Australian Electoral Commission

g. security guard or crowd control photo licence

h. evidence of right to a government benefit (Centrelink or

Veterans Affairs)

i. consular photo identity card issued by the Department

of Foreign Affairs and Trade

j. photo identity card issued to an officer by a police force

k. photo identity card issued by the Australian Defence

Force

l. photo identity card issued by the Australian Govern-

ment or a state or territory government

m. Aviation Security Identification Card

n. Maritime Security Identification card

o. credit reference check

p. Australian tertiary student photo identity document

q. Australian secondary student photo identity document

r. certified academic transcript from an Australian univer-

sity

s. trusted referees report

t. bank card

u. credit card

Change of name

If you provide identity documents using a former name,

you must provide evidence of your name change. This

means providing a change of name certificate issued by

the Australian Registry of Births, Deaths and Marriages or

an Australian marriage certificate issued by a state or terri-

tory, in addition to your four identity documents. Church or

celebrant-issued certificates are not accepted.

Special provisions for proof of identity

The ACIC recognises that in exceptional circumstances

you may not be able to meet the minimum proof of identity

requirements.

Please contact the organisation that supplied you with this

form to assess your ability to meet the requirements and

determine the most suitable method to confirm your identi-

ty.

Directions

1. Sift the flour and custard powder into a large bowl and then stir in sugar.

2. Whisk yoghurt, oil, eggs and essence together and fold into the dry ingredients, taking care not to overmix.

3. Spread the mixture over the base of a baking paper lined 28cm x 18 cm slice pan. Scatter over peaches and dot with chunks of cream cheese.

4. Bake at 180°C for 35 minutes or until golden and cooked through.

5. Cool for 15 minutes in pan before turning onto a wire rack to cool completely. Cut into 16 squares and serve at room temperature. Store refrigerated in an airtight container.

Peaches and Cream Slice

Ingredients: 1 cup self-raising flour 1/4 cup custard powder 1/2 cup caster sugar 1 cup reduced fat vanilla yoghurt 2 tablespoons vegetable oil 2 eggs, lightly beaten 1 teaspoon coconut essence 440g peach slices in natural juice,

drained and cut in half 200g reduced fat cream cheese, broken

into 1 1/2cm chunks
