Our vision is to make Delhi 'Cataract Free'. To achieve this, we are engaged in examination of Eyes of Patients through six well equipped Hospitals. Besides the above, Free Eye and Health Check-up Camps are also held in different parts of Delhi NCR. The patients of Cataract detected at these Hospitals undergo Cataract Surgery free of cost at our three State of the Art Operation Theatres, one at Rajdhani Charitable Hospital, Nabi Karim, second at Kishwarna Charitable Hospital, Badarpur and third at Hauz Rani Saket Hospital, New Delhi. Nearly 21 Thousand Cataract Patients have undergone Operation with IOL implant so far. Our Operation Theatres compete in all respect with that of any Private Hospital. The patients undergoing Surgeries are the messengers of our Success. This has been possible with the support of well qualified team of Doctors & Para Medical Staff associated with us. So far we have provided services to more than 31 lac patients. We believe not in Quantity, but in Quality and to provide best quality services matching with any Private Hospital, we upgraded our super specialty hospital at Hauz Rani,Saket with ultra modern Glucoma Unit and State-of-the Art eye operation theatre for cataract surgeries with IOL Implants coupled with ultra modern 1.5 Tesla Whole Body MRI & CT Scan facility. In the series, we have undertaken renovation and upgradation of our Rajdhani Charitable Eye and Medical Center, Nabi Karim,New Delhi with installation of Ultra Modern and latest Technology. The work on which is slated to commence in August. Hospitals are the real Temples, Gurudwaras, Mosques and Churches of the Society where people get new life. I call upon all of you to join hands with your Heart, Body and Soul and associate with us in providing best of services. By the time this Magazine reaches you, you must have celebrated Independence Day, the most important National Festival on 15th August. It is memorable for every Indian on earth, Cultural programs and flag-hoisting ceremonies are the predominant affair of the day, while colourful kites fill the sky in the evening, to symbolize freedom. Festival season has started. The Raksha Bandhan- A festival symbolises the bond between brothers and sisters will be celebrated on 29th August and Janmashtami- A festival celebrating birth anniversary of Lord Krishna will be celebrated on 5th September. I express my good wishes for the upcoming festivals.

In India, we have an estimated 4.6 million people with corneal blindness that is curable through corneal transplantation made possible by eye donation. More than 90% of the corneal transplantation is carried out successfully and helps restore vision in people with corneal blindness. Corneal transplantation
can make a big difference to the lives of blind people. Mahavir International Delhi is doing its bit in the field of eye pledge and eye donation. During July, 2015, 4 eye donations were made, facilitated by our members Shri Rakesh Kumar, Shri Dinesh Kumar Jain and Shri Darshan Kumar Jain. Eyes of Mrs. Pushpa Jain w/o Shri Anurag Jain; Mrs Vimla Devi Agarwal, W/o of Mr. G.S. Agarwal; Mrs. Shail Jain, W/o Shri Naresh Jain, Shri Pushp Jain, H/o Mrs. Renka Jain were donated. A total of 176 eyes are donated through the efforts of MI Delhi so far and 33138 eye pledges have been made. We salute the spirit of donors and all those associated in making it happen.
We at Mahavir International Delhi have firmly spread our wings in the field of organizing Eye and Health check up camps under its flagship project Doctor at Door Step both in NCR and locations all across the country. We are already organizing camps at 24 locations outside NCR, sponsored by CONCOR, EIL and Safexpress. CONCOR has approved 12 new locations with 36 additional camps during the current financial year. This takes the CONCOR camp tally to 112. I am extremely grateful to the local centers of Mahavir International at these locations for playing a lead role in organization of these camps and providing logistical support. I also place on record my deep appreciation of the services being provided by Ms. Usha Kapoor, CEO MI Delhi in successful and efficient organization of these camps involving extensive travel.
We are always looking at the opportunity to serve the deprived section of society by expanding scope of our services. Recently we extended the timings of physiotherapy section of Badarpur hospital upto 5 pm from earlier closing time of 1 pm. It has been well received by the populace of the influence zone and we got very good response.
I am thankful to Vira Ms. Alka Jain, Chairperson Vira Group who is now devoting 3 days a week at Saket hospital and providing useful managerial inputs to improve the quality of service. We need more such support to improve the quality of camps also. I seek support from our members and members of Vira group.

GLAUCOMASun Tzu, the ancient Chinese military general, strategist and philosopher, said "If you know the enemy and know yourself you need not fear the results of a hundred battles". Glaucoma has for ages been an enemy and thief of sight, and our understanding of Glaucoma has come a long way from the earliest notions of the condition. The word glaucoma originally meant clouded in Greek; as such, it may have referred both to a mature cataract and to corneal oedema that might result from chronic elevated pressure. Now, glaucoma is a word that does not refer to a single disease entity, but rather, a group of diseases that differ in their clinical presentation, pathophysiology and treatment. The definitive characteristics of glaucoma are based on visual field loss or the appearance of the disc or retinal nerve fibre layer (RNFL). Thus, visual field defects form an integral part of the case definition of glaucoma.
Primary open-angle glaucoma is a progressive, chronic optic neuropathy in adults in which intraocular pressure (IOP) and other currently unknown factors contribute to damage and in which, in the
absence of other identifiable causes, there is a characteristic acquired atrophy of the optic nerve and loss of retinal ganglion cells and their axons, associated with an anterior chamber angle that is open by gonioscopic appearance. (1)
It is estimated that 45 million people in the world have open-angle glaucoma (OAG). (2) Glaucoma (both open-angle and angle-closure) is the second leading cause of blindness worldwide, with approximately 8.4 million people blind from glaucoma. There are approximately 11.2 million persons aged 40 years and older with glaucoma in India. (3) Primary open angle glaucoma is estimated to affect 6.48 million persons. (3)
Glaucomatous visual changes and defects are irreversible because of retinal ganglion nerve cell damage. Due to the irreversible nature of glaucomatous defects, current strategy aims at early detection through screening, and further detailed examination of 'at risk' suspects. Although not all patients with elevated intraocular pressure(IOP) develop glaucoma, reduction of elevated IOP is the only as yet proven approach to protect against visual field loss in patients with primary open-angle glaucoma (POAG).(4),(5) When you may develop glaucoma depends on the level of IOP your optic nerve may tolerate without getting damaged.
The changes of primary open angle glaucoma are noticed by the patient after significant disease progression due to a relative lack of attendant alerting symptoms. These factors make glaucoma the 'sneak thief' of vision. No symptoms. No pain. Normal vision. Without treat ment, people with glaucoma will slowly lose their peripheral vision. As glaucoma remains untreated, people may miss objects to the side and out of the corner of the eye and may seem to be looking through a tunnel. Over time central vision may decrease until no vision remains. Hence the need for early diagnosis, prompt treatment and lifelong compliance. Glaucoma is detected by means of a detailed eye examination including your visual acuity, visual field testing for peripheral vision, dilated eye examination for inspecting the optic nerve for any signs of damage, tonometry for measuring your IOP and pachymetry for measuring your central corneal thickness. A gonioscopy may rule out any occludable angles as the treatment for angle-closure glaucoma is different from that of open-angle glaucoma. Lastly,an optical coherence tomohraphy (OCT) for your retinal nerve fibre layer (RNFL) is included in your baseline investigations so that progress or control of glaucoma may be gauged at subsequent follow-up visits.
Among the identified suspects, the question arises of appropriate timing to initiate therapy. Therapy once initiated, is likely to continue for the rest of the patients' natural life, constituting a substantial burden on the patient, physically, mentally and financially.
In general, the overall goal to manage glaucoma patients is to preserve visual function while maintaining the best quality of life. This can be achieved by preventing or slowing the progression of glaucomatous damage by lowering IOP to a level at which further damage is minimal. For this, there are three management modalities:
1) Medical therapy 2) LASER therapy 3) SurgeryInitial medical therapy remains the standard for most of the newly diagnosed patients of glaucoma. This includes use of topical and
occasionally orally administered agents that lower IOP by enhancing aqueous flow or reducing production or both. Currently 6 different classes of glaucoma medications are available:
i. Prostaglandin – related agents ii. β –adrenergic receptor antagonistii. Adrenergic receptor agonist iv. Carbonic anhydrase inhibitorsiii. Cholinergic or miotic agents vi. Hyperosmotic agentsThe introduction of topical ß-blockers in the 1970s was a significant advance, and ß-blockers quickly became the most commonly
prescribed class of agents to lower intraocular pressure (IOP). They generally were the most effective medication to do so and were well tolerated by most patients.
The type of laser therapy and surgery will depend on the form of glaucoma and how severe it is. Selective laser trabeculoplasty (SLT), Argon laser trabeculoplasty (ALT), Micropulse laser trabeculoplasty (MLT) and Laser cyclophotocogulation are the various laser techniques used for POAG. Lasers produce a focussed collimated beam of light intended to produce drainage channel for aqueous. Being done in the outpatient setting, there is hardly any pain during or after the procedure so that patients can resume dsaily activities immediatelt after the procedure.
Patient compliance to medication has always occupied a central position in the course of ocular disease. Compliance becomes all the more important in the management of any of the conditions in the glaucoma 'spectrum', owing to the lifelong nature of treatment of an irreversible condition.
In light of inherent hurdles to therapy, the aim becomes to try, as far as possible, to keep the patient on as few medications as possible. The drugs mentioned above are all candidates for monotherapy, the exact choice depending on patients' tolerability, drug efficacy, side effect profile, and the clinician's experience with the drug.

Medical management is usually the first treatment option chosen for most types of open angle glaucoma. The decision to start treatment is one that must be made with complete involvement of the patient, as it is likely to continue for the patient's lifetime. Here arises the role for an agent with sufficient efficacy, minimal side effects, affordable cost, and ease of administration.
These factors are important in a country like India where many patients are from low socio-economic strata, and low levels of patient literacy and understanding add significant problems to ensuring patient compliance and follow up.Again, the decision to start treatment is many times not straightforward, and the need for an ideal agent is felt all the more, in light of the adding the burden of side effects and cost to the patient.
It is important to recognize that no single medication can be used in all patients in all circumstances. Each of the available drugs has its own unique advantages and disadvantages, and currently six separate classes of anti-glaucoma drugs are available. It is necessary to individualize each patient's treatment regimen to maximize the benefits and limit the undesirable effects. It is not realistic to anticipate using all the available types of drugs for any single patient. Rather, it is vital to select the best agent for each particular patient's needs.Wisdom lies in anticipating the possible course of glaucoma so that the best possible treatment can be offered to the patient.
