S.C.T.I.M.S.T. is a tertiary level referral centre, catering to a population of 25 million, and it is declared as a University and an Institute of National Importance by an act of Indian Parliament. I joined this institute as a full time faculty member in February 1979 and worked there till September 1991. I was second in position in the hierarchy of neurology department.

Neurology services have 50 inpatient beds with an ICU dedicated exclusively for their patients. This is complemented by the Neurosurgery department having similar bed strength and ICU facilities, and supported by a Neuroradiology division with facilities for CT scanning, MRI scanning, Digital Subtraction Angiography and Myelography, a Neuropathology division having facilities for all types of histopathological, immunopathological and electron microscopic studies and a neurochemistry division offering various neurochemical assays. The Neurophysiology laboratory has facilities for digital EEG and brain mapping, 24-hour EEG video monitoring; multimodality evoked potential studies, EMG and nerve conduction studies.

The department conducts 5 neurology clinics and 3 special clinics for epilepsy, headache and neuromuscular disorders in a week and offered advanced treatment facilities for all types of neurological problems. Twenty-five new cases are scheduled in a single neurology clinic, and an average of 850-900 new cases are admitted annually to neurology wards. I conducted 2 general neurology clinics and one neuromuscular clinic in a week. In addition to the routine clinical neurological activities, I was in charge of Neurology ICU and 1065 patients were treated in NICU between 1st January 1981 and 31st December 1990. Plasmapheresis, critical care neurology, interventional neuroradiology for vascular malformations of brain and surgery for epilepsy and movement disorders, are some of the specialized clinical services offered by this centre. I was in charge of Plasmapheresis programme, which was offered to Acute GBS and in selected indications of Myasthenia patients. Our department was one of the early centres in India offering Thymectomy for Myasthenia Gravis and I was instrumental in starting this way back in 1982.

I have gained extensive experience in performing and reporting Neurophysiological investigations. On an average 1400-1500 EEGs, 300-350 NCS/EMG studies and 400-450 EP studies were performed annually in the Neurophysiology labs and 2 – 3 Neurologists shared this workload. Consultants took on call duty once in 3 days for most part of my stay and at times once in 2 days. There are weekly Neuroradiological conferences on every Saturday morning and Neuropathology meeting /brain cutting sessions every Saturday afternoon. I was member of the Library Committee and Junior Staff Selection Committee during varying periods of my tenure.

This is the only medical school in this country and the University hospital is the apex referral centre here. I joined the division of Neurology - Department of Medicine, from the very beginning of the department in September 1991,and was actively involved in the organization and development of neurology services and training in that country. The neurology division has all modern facilities and offers tertiary level service to the entire country. Neurophysiology Laboratory facilities include EMG and nerve conduction studies, digital EEG and video monitoring, 24-hour EEG recording, polysomnography and electronystagmography. Neuroradiology facilities include CT scanner, MRI, digital subtraction angiography and myelography. There were 3 Neurologists in the Unit.

The neurology unit conducts 3 outpatient clinics in a week and the average booking for a day was 25-30 patients. Neurology clinics were conducted at two regional hospitals in Nizwa and Sohar once a month. Annual neurological admissions ranged between 250-300 patients. I did on call rota with general physicians on a 1 in 6 scale and also gave on call cover for emergency neurology admissions once in 3 days. Neurology consultation service was provided to other departments on a regular basis. On an average 1200-400 EEGs were performed annually. Statistics for other Neurophysiological procedures are tabulated below.

The Unit of Neurosciences at the University Sains Malaysia is comprised of Neurology, Neurosurgery and allied supporting specialties like Neuroradiology, Neuropathology etc: and I am the Chief Neurologist in the Unit. This is a tertiary level referral centre for neurological disorders in the eastern part of peninsular Malaysia, equipped with all modern facilities. There is a 6-bedded dedicated ICU for Neurosciences. Neurophysiology Laboratory facilities include EMG and nerve conduction studies, digital EEG and video monitoring, 24-hour EEG recording, polysomnography and electronystagmography. Neuroradiology facilities include CT scanner, MRI, digital subtraction angiography and SPECT. There are 2 general neurology clinics in a week and the average booking per clinic is 40-50 patients. Yearly neurology admissions are in the region of 230-250 patients. Neurology consultation service is provided to other departments on a regular basis. The volumes of Neurophysiological testes are 900-1000 EEGs, 100-125 NCS/EMG and 150-200 EP’s annually. There are weekly neuroradiology conferences and monthly neuropathology meetings. I drew up the protocol for Thrombolysis in Stroke and this treatment is offered at this unit since 2003. I also drafted the Epilepsy Surgery Protocol and we have started Epilepsy surgery programme here since July 2004. In 2006 we started D.B.S. for movement disorders for the first time in HUSM.

TEACHING-EXPERIENCE

SRI CHITRA TIRUNAL INSTITUTE FOR MEDICAL SCIENCE AND TECHNOLOGY. INDIA (http://www.sctimst.ac.in) February 1979 – September 1991.

The department conducts Neurology Residency Program of 3 years duration, leading to Doctorate in Medicine (D.M.) in Neurology. I was actively involved with the organization and running of this residency program from the very beginning. The entry requirement is Masters degree in Internal Medicine, Psychiatry or Paediatrics. Candidates were selected based on their performance at the entrance examination and interview.

The residents undertook supervised clinical responsibilities with more autonomy in decision making as they progressed through their training. There are dedicated teaching activities in the form of bedside teaching rounds, PG case discussions, Journal clubs, Seminars /Tutorials, Clinical Neurophysiology training sessions, weekly Neuroradiology and Neuropathology conferences and few didactic lectures on basic neuroscience topics. The residents also gained experience in clinical research when they undertook at least one research project during their training period. I regularly conducted all of the teaching activities listed above and supervised resident’s research projects.

There are periodic internal assessments as well as two examinations-one at the end of year 1 for basic neurosciences and the other at the end of year 3, consisting of written, clinical and viva components. 22 neurology residents were trained during my tenure. Neurology training was also offered to Internal Medicine, Paediatrics and Psychiatry residents who rotated through our department as part of their residency program. Continuing Medical Education Courses in Neurology were also conducted periodically, targeting the general practitioners and internists in the state.

The division of neurology is actively involved in both undergraduate and postgraduate training programs. Undergraduate training is mainly in the form of bedside clinical teaching, problem-based teaching of major neurological problems and integrated lecture series covering all the neurological topics. As the Coordinator of

On an average 20 hours were spent in teaching per week. I was also an Examiner for various in course assessments as well as for the final exit examination

Neuroscience unit is responsible for providing Neurology input to both undergraduate and postgraduate training programme. Undergraduate programme at U.S.M follows the Problem Oriented Learning module. Postgraduate programme offers 4-year M.Med degree. At present, residents from Medicine, Emergency Medicine, Psychiatry, Family Medicine and Ophthalmology have their rotation in Neurology. Our unit has started the M.Med Neurosurgery course in 2001, the only centre to offer this programme in Malaysia. Similarly, for the first time in Malaysia, Advanced Masters Degree course in Neurology was started in our department in June 2007. I was responsible for drafting the course objectives and curriculum and piloting it through various university bodies.

Research work undertaken./ ongoing

1. Immune mediated Polyneuropathies.

Most of my early research work was centred on Guillain Barre Syndrome and Chronic Inflammatory Demyelinating Polyneuropathies. It involved prospective study of the natural history, outcome prediction, role of Plasmapheresis, Clinicopathological and Immunological studies and cost effective studies. Most of these works are published.

2. Myasthenia gravis. Large experience with MG and Thymectomy. Results of Thymectomy in Asian population and the Experiences in Oman are already published.

3. Multiple Sclerosis. Clinical epidemiological studies in MS patients.

4. Stroke Studies on the Clinical radiological correlations in acute stroke, Role of Mitral Valve Prolapse Syndrome in Young stroke

1. Valproate in Partial Epilepsy. Principal investigator of this multinational clinical trial at University Sains Malaysia. The trial worth US$ 6,000 was successfully completed in

July 2004. Results are pending analysis

2. PROFESS- Principal investigator at USM for this multinational clinical trial evaluating secondary stroke prevention. This trial brings in revenue of US$ 220,000.00 to the University. Trial will be completed in March 2008

3. PERFORM- Principal investigator for this International Multicentric clinical trial. Evaluating secondary stroke Prophylaxis. Revenue of Euro.120,000.to the University. Trial is expected to be completed in 2010.

4. ENOS. Principal investigator for this investigator initiated multicentric study evaluating the role of B.P. management in acute stroke.
