The use of temporary mechanical access systems must be planned and conducted in accordance with the requirements for high risk activities. 1.3.4 Temporary suspended access equipment (boson’s chairs, cradles, gondolas, rope access, swing-stages) must only be used as a last resort where safer means of access are not available. All use of temporary suspended access equipment must be planned and conducted in accordance with the requirements for high risk activities. Appropriate secondary fall prevention must be in place to effectively protect persons using the equipment at all times from any failure of any part of the equipment and its suspension system. Effective rescue procedures must be prepared and be ready for implementation. 1.3.5 Guarding must be of suitable strength for any elevated working platforms, with harnesses secured to provide fall restraint, and tethers in use to prevent tools and materials falling if dropped. 1.3.6 All Mast Climbers, Swing Stage Scaffolds and Building Maintenance Units/BMUs must be installed, maintained and inspected by engineer/competent person(s), following the manufacturer’s specifications at all times – as a minimum details of the design, maintenance, inspections and manufacturer’s specifications must be provided. A thorough 3rd Party/Independent Examination following the manufacturer’s specifications (provided by either the manufacturer/supplier/regulator/authorised assessor) must be carried out by an engineer/competent person(s) and a certificate of examination issued when any of the following circumstances apply; Prior to first use, At maximum 6 monthly intervals, After substantial repair, or alteration, or each time a mast section is extended, or relocated, If the stability of the unit has been affected 1.3.7 The engineer/competent person(s) involved in installation, maintenance and

GMR Subsection: 1.3 Building Maintenance Units and Mechanical Access

1.3.1 Permanent Building Maintenance Units (BMU), e.g. mechanised cradle systems, should be designed into new operations and installed on existing operations where possible to provide safe access for cleaning and maintenance. They should be fixed to the operation’s structure and must have the safe working load clearly marked and have sufficient, clearly designated safety harness anchorage points designed to withstand the forces caused by a fall of any person located anywhere on the platform.

1.3.2 The use and maintenance of BMUs must be planned and be the subject of a regular

inspection and preventative maintenance regime. 1.3.3 Temporary mechanical access systems, e.g. boom lifts, should be employed where

permanent systems are not available. The use of temporary mechanical access systems must be planned and conducted in accordance with the requirements for high risk activities.

A thorough 3rd Party/Independent Examination following the manufacturer’s specifications (provided by either the manufacturer/supplier/regulator/authorised assessor) must be carried out by an engineer/competent person(s) and a certificate of examination issued when any of the following circumstances apply;

Prior to first use,

At maximum 6 monthly intervals,

After substantial repair, or alteration, or each time a mast section is extended, or relocated,

If the stability of the unit has been affected

inspections must provide proof of competence and must keep a record of inspection and actions with the device and/or attach tags detailing the last inspection and confirming that all structural anchorages have been installed and tested to manufacturers/design engineers’ instructions and where temporary anchorages include metal tubes as part of the tie arrangements, stop-ends must be provided so as to prevent ties from pulling off the end of the tubes.

1.3.8 Where temporary works/elements (including ties or fixings to structures, independent

lifelines, etc) are deployed, secondary devices (anti pull out) must be installed where there is any risk of ties pulling out.

1.3.9 All persons using Mast Climbers, Swing Stage Scaffolds and Building Maintenance

Units must use a suitable fall arrest harness and lanyards at all times. Emergency retrieval rescue procedures must be allowed for and all harnesses, lanyards and static lines must be installed and attached to one of, or a combination of the following elements;

A vertical line independent to the PVAE and specifically engineered for the purpose of withstanding the forces likely to be experienced in a fall situation, or;

An engineered anchor point or horizontal static line fabricated and certified by the manufacturer for that purpose, and capable of withstanding the forces likely to be experienced in a fall situation, or;

A transportable temporary independent anchor point engineered for that purpose, such as; a sling (choked or looped around suitable structures), an anchor strap (looped around suitable structures) or an eyebolt (fixed with a trigger catch mechanism for fixing through holes). In all cases these elements must be capable of withstanding the forces likely to be experienced in a fall situation.

1.3.10 A trained operator or other competent person must conduct and record daily checks of

the equipment following the manufacturer’s recommendations to ensure the platform is in good working order, free from visual defects, and any obstructions, e.g. open windows

1.3.11 More detailed inspections in line with the manufacturer ’s specifications must be

carried out and recorded by a trained operator or other competent person on a weekly basis, after any repair, or after any event that may have caused damage to the unit (i.e. high winds etc). The following items must be reviewed, tested and verified (in addition to the items listed within the Daily Checks above) before the equipment is put into operation:

Bolted connections are secure, e.g. platform connections, mast bolts and tie fixings to building structure

Some form of visual indication must be logged as to which ties and bolts have been checked during inspection (e.g. coloured paint, or a mark up on a drawing if access to bolts is restricted).

No override of emergency controls and all safety limits and brakes, including secondary brakes, are fully operational.

Controls including emergency stop buttons function correctly. Power supply and all electrical cables and connections are in good order.

Mechanical Aerial Lift Device 2- Point Suspended Platform

How to Comply with the Standard:

Definitions:

Building Maintenance Unit is an item that comprises a working platform suspended on wire ropes from a securely mounted overhead structure or carriage. It is designed and erected to provide access to parts of a permanent building and to serve only that building, although parts of the installation may be demountable for storage purposes. The platform is capable of being raised or lowered and may be able to move laterally across the face of the building. Usually it is powered.

A two point suspension scaffold is an item that comprises a working platform suspended on wire ropes from a securely mounted overhead structure. It is designed and erected to provide access to parts of a building. It may have been erected for an irregular task such as the painting of the facade or resealing of windows. The building may be permanent or it may still be under construction. A two point suspension scaffold may be powered or may be manually operated, or it may be a combination of both. The complete installation may readily be relocated to other locations or projects.

Note: The difference between building maintenance units and two point suspension scaffold should be understood. BMUs are permanent; two point suspended scaffolds are temporary.

utilize materials and products which do not require workers to perform at height.

2. The hierarchy of risk control:

a. Eliminate the need to work at height

b. Reduce the need to work at height.

c. Provide individual fall arrest as a last resort.

3. All Lend Lease Americas facilities designed and constructed five stories or greater will have a BMU system for maintenance, painting, and cleaning .

4. Any acquired asset which does not have a BMU system or two point suspension scaffold system will use mechanical aerial lift devices or an approved plan will be in place.

5. Install complete stairs, stair rails, and landings prior to sheathing 2nd floor deck.

6. Each project must maintain a log of mechanical access systems (i.e. MEWPs, scissor and aerial lifts) to include type of equipment, inspection dates, and operator qualifications. A safe work method statements (SWMS) must be in place prior to use.

7. Suspended access equipment (e.g. bosun’s chairs, cradles, gondolas, swing stages) must only be used where safer means of access are not practical. All persons working on suspended access equipment must wear and use appropriate fall prevention equipment to effectively protect them at all times from any failure of any part of the equipment. Effective rescue procedures must be prepared and be ready for implementation.

8. All Mast Climbers, Swing Stage Scaffolds and Building Maintenance Units/BMUs must be installed, maintained and inspected by engineer/competent person(s), following the manufacturer’s specifications at all times – as a minimum details of the design, maintenance, inspections and manufacturer’s specifications must be provided. A thorough 3rd Party/Independent Examination following the manufacturer’s specifications (provided by either the manufacturer/supplier/regulator/authorised assessor) must be carried out by an engineer/competent person(s) and a certificate of examination issued when any of the following circumstances apply;

9. All mechanical Access Systems must be installed, maintained, and inspected by a competent person and records

maintained on site.

10. Any worker using a PVAE, (Power Vertical Access Equipment) such as; mast climber, swing stage scaffold or building maintenance unit must utilize an independent vertical lifeline. When using PVAEs, if provided, the manufacturer’s anchor system must be used while ascending/ descending.

Safe Use of Mobile Elevated Work Platforms with

articulated and telescopic booms (excludes scissor lifts)

Mobile Elevated Work Platforms (MEWPs) fitted with articulated and telescopic booms commonly known as Boom Lifts or Cherry Pickers, provide an effective means of providing a safe method of access for work at heights. However, they can also cause serious incidents if poorly designed, used and/or maintained. Incidents commonly occur when these types of MEWPs are used outside of the manufacturers’ specifications, in unsafe conditions such as on excessive slopes or on unstable ground, or by inadequately trained operators. This document sets out the Lend Lease policy for controlling the risks associated with this type of Mobile Elevated Work Platforms (MEWPs), whether they be truck or trailer mounted. All Lend Lease businesses must implement effective measures to ensure that they comply with this policy and include it in their Means and Methods.

Lend Lease Global Policy for Safe Use of MEWPs � All MEWPs used on Lend Lease assets, premises or projects must be safe by design and

appropriate for the task � Works involving MEWPs must be carefully planned to ensure all significant hazards associated with

the task and environment are identified and effective control measures are implemented � All MEWP operators must be competent and briefed on the safe method of work to follow � MEWPs must only be used within their safe working parameters set by the manufacturer � All MEWPs are maintained in accordance with manufacturer’s instructions to ensure they are safe to use

Action Required:

I) Performance Standards

All locations where MEWPs are must immediately implement the following performance standards for their safe use:

1. All operations using MEWPs must be covered by a Safe Work Method Statements (or JSA), produced before works commence on site, which assess the control of all significant hazards associated with the task and work environment, including safe delivery and getting the MEWP into the work location, electrical hazards, ground conditions, heights to be reached, materials to be handled, obstructions, services, traffic, weather conditions, etc. Rescue procedures must be developed to deal with all likely emergency situations. The safe method of work and area of operation must be reassessed after any significant changes occur which could impact on safe operation eg weather conditions, work tasks, etc.

2. All MEWPs shall be checked to ensure MEWPs are not allowed to work on site unless they are of the type specified, in good condition, supplied with manufacturers’ instructions and maintenance records demonstrating compliance with the manufacturers’ instructions.

3. Before works commence a competent person shall confirm that all surfaces where MEWPs will operate have been

assessed to ensure they provide a safe environment for operation, including checks to ensure they are able to support the weight of the MEWP and associated loads, that they are level and smooth enough to enable the specific MEWP to operate well within the safe working limits of the machine, and barriers (e.g. Jersey Barriers) or other forms of suitable protection are provided to hazards such as excavations, holes, perimeter edges, services, slopes, underground structures, etc.

4. The competence of all MEWP operators to safely use the specific equipment they are assigned to operate must be assessed before being they are allowed to commence work. Proof of competence includes relevant industry standard qualifications, training provided by the manufacturer and/or practical testing.

5. All persons working in MEWPs must wear a full body harness attached to a manufactured lanyard anchorage point (and a life jacket for works over water where the risk of drowning) and stay within the carrier at all times whilst working.

6. Exclusion zones must be established by cordoning-off areas in which MEWPs operate to prevent collisions with persons or vehicles.

7. MEWPs must not be moved across sloping ground with the carrier occupied and in the raised or extended position. MEWPs may be moved whilst the carrier is occupied and in the raised or extended position only if the machine is design for such operations, the surface is firm and flat, and the activity has been assessed as safe and agreed as part of the SWMS.

8. MEWPs must be operated in accordance with the manufacturer’s instructions and must not be used: � in locations where the conditions are unsuitable for the MEWP (such as adjacent to unprotected edges and

excavations, in confined locations, on excessive slopes, on rough terrain, over unprotected services or, on unstable ground , etc.)

� beyond their Safe Working Load or for lifting materials liable to fall from the carrier � as an unsafe means of access � in ways liable to make them unstable, eg travelling with the boom raised and with the counter-weight on the down

hill side of a slope.

9. Daily pre-start briefings must be provided to MEWP operators on the safe method of work paying particular attention to ensuring MEWP stability by examining ground conditions, deploying outriggers (where fitted), and travelling with the boom lowered; establishing exclusion zones to prevent collision with vehicles or persons; preventing entrapment against fixed objects; remaining within the carrier whilst working at height and never standing or placing boards on the guard rails of the carrier or using ladders within it; and safe parking at end of work task with keys removed.

10. Tool box talk talks shall be provided on a regular basis on the safe use of MEWPs, emphasising the fact that they are only to be used according to the manufacturers’ specifications and the requirements of this GSA.

MEWPs must be checked and maintained to ensure they are kept in a safe condition by conducting daily safety checks before commencing work and weekly inspections, with associated fault reporting and remedy procedures. Thorough examinations and servicing should be undertaken by a competent person at least every 6 months.

II) Management Systems

Each business must immediately adopt the above performance standards which are regarded as the Lend Lease minimum performance standards and businesses are free to expand upon them to improve the control of risks associated with MEWP operations. The following issues shall also be addressed when planning works at height which involve the use of MEWPs and be included in business management systems and Means and Methods for planning works at height:

1. When considering the use of MEWPs as appropriate methods for safely conducting works at height careful consideration must be given to the nature of task and work environment, access, electrical hazards, ground conditions, heights to be reached, loads including numbers of workers needed tools and materials, obstructions, services, traffic, weather conditions, etc.

2. Construction works need to be planned and scheduled to ensure that a safe environment is created for MEWPs

to operate in, e.g. hard stand/subgrade is provided to give a firm level surface to operate on and work areas are able to be cordoned off from other activities.

Where the use of MEWPs is confirmed as a safe method of work, the type of MEWP specified must be appropriate for the task and specific work environment. They should also be safe by design by ensuring they comply with relevant national standards, are fit for purpose and fitted with robust means of fall prevention and alarms or cut out devices to prevent operation beyond safe working limits particularly those relating to stability due to slope angles, over-extension or speed of movement, have lanyard anchorage points fitted to facilitate the use of harnesses, and lifting points if they need to be craned into position.
