ASQ Provider Instructions

Welcome to the ELC Pinellas Provider Portal

The process for completing will be easier to manage on the Provider Portal. The Provider Portal will allow you to record answers to the ASQ-3 questionnaire and save it to a child's profile to be retrieved at any time.

LOGIN

https://providers.elcpinellas.net Create Account: • Email address • Password must be at least 7characters long,

containing 1 capital letter and 1 number • Child Care License number or Phone number • Physical zip code of site

• Review children and ASQ-3 screening status, if missing a notification • Reminders and past due notices are displayed on the Dashboard • SR Children will be displayed in when an ASQ is needed • SR Children will be displayed in when an ASQ is Past Due • New Enrollment SR Children will be displayed in Black • Do screening for SR child • Use the Screening Form from the ASQ Master Kit • View a letter of the Results

• Review children and ASQ-3 screening status, if missing a notification on

the Dashboard

• ASQ Reminder Notice is displayed on the Dashboard • Click on this notification line

• SR Children will be displayed in when an ASQ is needed • Select “Do screening” for SR child

or

• ASQ Past Due Notice is displayed on the Dashboard • Click on this notification line

• SR Children will be displayed in when an ASQ is Past Due • Select “Do screening” for SR child

or

• Use the Screening Form from the ASQ Master Kit • Select “ACCEPT”

Note: if missing ASQ Master Kit, send email to providerasq@elcpinellas.net

• Fill out the answer form • Click on a pencil icon to enter any comments you may have • Select “COMPLETE”

• Select “YES” to Submit

• View a letter of the Results

Note: answer “Accept” to Pop-up Blocker notification, then repeat steps

• Results Letter will pop-up • Share with child’s parent(s) or guardian(s)
