NHS Greater Huddersfield Clinical

Commissioning Group (CCG) is a group of family doctors and other

health professionals who have the job of looking after the health of

people in a local area.

One of the most important things

they do is to plan and buy health services for local people.

The CCG knows how many people

there are and what their needs are. This information is in written in a

Joint Strategic Needs Assessment (JSNA).

The CCG plans and buys:

the services provided by big hospitals.

ambulance services.

‘out of hours’ family doctor (GP) services.

services for people who have

learning disabilities and/or

mental health needs.

NHS Greater Huddersfield CCG wants to get the best health for the

people of Greater Huddersfield

2014-2019

The CCG is thinking about what it plans to do for the next 5 years. It

wants everyone to have the right

care at the right time in the right place.

We have to do things differently because people’s needs have

changed.

We are better at treating diseases

that used to kill people.

New treatments, drugs and

technology have changed how we look after people.

There are more people.

People are living longer.

People are more likely to be ill because of conditions like diabetes,

heart disease and mental health

problems.

Care can now be provided at home,

or near to where people live. This works really well and people like it.

The CCG have used information from the JSNA, the Health and Wellbeing

Strategy and what people have told them to make their plans.

They have also thought about what changes are needed to make sure

services are high quality and safe.

The CCG aims to

Help people look after their own

health better

Make the best use of technology and new ways of doing things

make sure all the services people use work together well.

Have services that are easy to use and treat people with dignity

and respect

We asked local people what they

thought.

People told us:

Information on health should be

easy to understand and find.

Page 8: ‘Right Care, Right Time, Right Place’ · Strategy and what people have told them to make their plans. They have also thought about what changes are needed to make sure ... Right

Make health services easier to use.

We want to know more about our

health.

Help us to look after ourselves better.

Spend more money in the local community.

Staff should talk to us and listen to

us.

We want more health checks.

We want more help when we leave

hospital.

Keep people safe.

We want more services near to our homes.
