Store Demo Test Agreement

This Test Agreement is entered into this ___day of _________2011 between Aqua-zone Systems LLC and ___________________

Aquazone shall place an Aqua-zone S-10 demo in store___________________________________________________for a test period beginning___________and ending______________at no charge to the store. This unit is owned by Aqua-zone, who retains its ownership throughout the test period, and can be removed at will by Aqua-zone at any time upon written notice to the store management.

Aqua-zone will install and service this unit and report to both parties any significant sanitation results of the test.

This Test Agreement is approved by both parties as signed and dated below.
