PROFICIENCY TESTING When to use CAP PT and when other PT survey providers can be used The Centers for Medicare and Medicaid Services (CMS) regulates all human laboratory testing through the Clinical Laboratory Improvement Amendments (CLIA). CMS has a list of regulated analytes that require enrollment in a CMS-approved proficiency testing (PT) program. In hemostasis, these analytes are PT, APTT, and fibrinogen. New York Department of Health also requires a CMS-approved PT program for the same analytes. The College of American Pathologists (CAP) proficiency survey program is CMS approved. In addition, CAP has a few additional coagulation analytes that require a CAP-approved PT program for laboratories that are CAP-inspected: factor VIII activity, quantitative D- dimer, anticardiolipin IgG/M, and homocysteine. Analytes that require CAP-approved PT programs can be found on the CAP website or can be easily located on your CAP laboratory accreditation program Laboratory Activity Menu and are indicated by a “Y” in the PT Required Column. Laboratories that are not inspected by CAP can use NASCOLA alone for all proficiency except PT, PTT, and fibrinogen. PROFICIENCY TESTING

When to use CAP PT and when other PT survey providers can be used

EXECUTIVE DIRECTOR NOTES

Greetings! As we enter NASCOLA’s 17th year, there is much to celebrate

and much to anticipate. New NASCOLA officers are the first to assume a

3-year term as mandated by our updated bylaws. Once again, the

executive board consists of highly qualified individuals who have a

passion for the field of hemostasis and thrombosis. Short biographies of

the elected officers are the basis for this newsletter.

NASCOLA continues collaboration with THSNA to promote education and

research, and with ISLH to bring excellent and pertinent presentations to

members through the annual conference. The NASCOLA member base

continues to grow as laboratory leadership discovers the quality and

breadth of proficiency testing material supplied by our ECAT Foundation

partner.

New and longstanding members are encouraged to join a committee of

interest. Contact a NASCOLA committee chair or the executive director

PRESIDENT: Dorothy Adcock, MD

Dr. Adcock is the newly elected NASCOLA president. After completing her BS and MD from Michigan State University, she completed her residency in pathology at the University of Colorado Health Sciences Center. She is the medical director of coagulation at Esoterix Coagulation Englewood and has served in this role since 1990. Dr. Adcock has been involved in the field of hemostasis on a national and international basis for the past 20 years. She is chair of the Hematology and Pathology Device Panel for the FDA and is active on national committees for ASCP and CAP including the coagulation resource committee. Dr. Adcock is past chair of the Clinical Laboratory Standards Institute Area Committee on Hematology and has authored a number of CLSI guidelines. She has served as a member of the CAP Coagulation Resource Committee. In her support of NASCOLA, she has served as secretary, treasurer, and vice president. Dr. Adcock lectures often on a national level and has authored over 60 peer-reviewed articles in the area of hemostasis as well as a dozen book chapters.

VICE PRESIDENT: Kristi J. Smock, MD

Dr. Smock is an associate professor in the Department of Pathology at the University of Utah. She completed her residency and fellowship training at the University of Utah and is board certified in anatomic and clinical pathology and hematopathology. She serves as medical director of the Hemostasis/Thrombosis Laboratory at ARUP Laboratories where she enjoys being involved in all aspects of the laboratory. She is interested in education and serves as the Associate Residency Program Director for the pathology residency program at the University of Utah, director of the rotations in coagulation medicine for residents and fellows in the pathology department, and is actively involved in medical student teaching. Dr. Smock has been a part of the NASCOLA executive board for the past 2 years as a member-at-large. She has also been actively involved in several NASCOLA proficiency testing subcommittees, including serving as chair of the heparin-induced thrombocytopenia and protein S subcommittees and as a member of the factor assays, factor XIII, and activated protein c resistance subcommittees.

SECRETARY: Donna Castellone, MS, MASCP, MT (ASCP), SH

Donna D. Castellone is the QA manager for specialty testing and supervisor, special coagulation and special hematology at New York Presbyterian Hospital- Columbia, New York, New York. Donna has 33 years of clinical laboratory experience and 6 years industry clinical trial experience. She has nearly completed PhD work on standardization of laboratory testing (all but dissertation), and has authored several book chapters and publications, and presented many coagulation lectures. Donna is an adjunct professor at York College and Rutgers University. Most recently, she served on the NASCOLA executive committee as a member at large, and is on the publication and precurriculum committee for ASCP, and the expert committee for hematology for CLSI. Donna's professional memberships include ISTH, ISLH, and ASCP.

TREASURER: Robert Gosselin, CLS

Robert C. Gosselin, CLS is recently retired from his position as senior specialist in hematology/hemostasis at the University of California, Davis Health System in Sacramento, CA. Mr. Gosselin is a licensed Clinical Laboratory Technologist in California, and certified Specialist by the International Society of Clinical and Applied Hemostasis, Thrombosis and Vascular Medicine. Bob has authored or coauthored over 70 peer-reviewed articles related to coagulation, 2 books chapters, and participated in over 40 national and international speaking engagements. He is a member of the ISLH, NASCOLA, and recently served as member-at-large with NASCOLA. Bob serves as a member for the CLSI consensus committee on hematology, is also a member of the CLSI’s document development committee for von Willebrand assays, an advisor for CLSI’s document development committee for lupus anticoagulant testing, and serves as a reviewer for several journals including the Annals of Pharmacology, Journal of Thrombosis and Haemostasis, Thrombosis and Haemostasis, and the International Journal of Laboratory Hematology.

JOHN FRANCIS, PhD

John Francis, PhD is the Director of the Florida Hospital Center for Thrombosis Research. Originally from the United Kingdom, he joined Florida Hospital from Southampton University Medical School in 1994. Dr. Francis' main research and clinical interests are in the areas of anticoagulant therapy, heparin-induced thrombocytopenia, hemostasis and cancer, hypercoagulability, platelet function and novel approaches to the assessment of patients with hemostatic disorders. An experienced educator, Dr. Francis also holds the title of Professor of Medical Education at the University of Central Florida, College of Medicine.

Dr. Francis is a member of the International Society for Thrombosis and Hemostasis and The American Heart Association. He was the founding editor of the International Journal, Blood Coagulation & Fibrinolysis, served on the editorial board of Thrombosis Research and acts as a reviewer for more than 20 other journals in the field of hemostasis, thrombosis and cardiovascular medicine. He acted as chair of the Region I and II Thrombosis Study Group of the American Heart Association, and was a previous executive committee member NASCOLA.

Dr. Francis acts as a consultant for several pharmaceutical and diagnostics companies, and is a member of the FDA Hematology Devices Panel. He has given over 270 invited lectures, 200 research presentations, and published more than 120 papers in this field.

JANA GAUSMAN, MT(ASCP)

Jana Gausman, MT is the supervisor of Hematology, Immunology, and Coagulation Laboratories at the Oklahoma City Veteran Affairs Medical Center. She received her BS in medical technology from East Central University in Ada, Oklahoma, after which she did clinical laboratory work and research at the University of Oklahoma and the Oklahoma City Veteran Affairs Medical Center. For the past 16 years, Jana has been the supervisor of hematology, immunology, and coagulation laboratories at the Oklahoma City Veteran Affairs Medical Center.

Jana serves NASCOLA as the ECAT liaison and as the contact for members with questions about the proficiency testing program.

NICOLE ZANTEK, MD, PhD

Dr. Zantek is an Associate Professor in the Department of Laboratory Medicine and Pathology at the University of Minnesota, Minneapolis, MN where she also serves as Medical Director of the Special Coagulation Laboratory and is attending physician in the Division of Transfusion Medicine. Dr. Zantek received a Bachelor of Arts degree from Concordia College in Moorhead, MN and as a Howard Hughes Medical Institute predoctoral fellow, earned her PhD from Purdue University. She earned her medical degree from Georgetown University School of Medicine and remained for a residency in Anatomic Pathology and Clinical Pathology at Georgetown University Hospital. Dr. Zantek then traveled to the University of Minnesota and completed fellowship training in Transfusion Medicine/Blood Banking. Her research interests include quality in hemostasis testing, understanding the influence of extracorporeal circuits on hemostasis, and pediatric transfusion medicine.

Dr. Zantek believes that a critical area for NASCOLA to focus on is promoting collaborative research activities among its members. Development of a network of investigators and laboratories that have diverse areas of expertise and availability of test methods would expand the ability of researchers to connect and advance the field.
