1) Fill in the forms, there are 4 mandatory forms.

a) Fill in Application Form - IMM5257 (Download IMM 5257)

(Fill in online/electronically; use Internet Explorer to search the link) This main application form (IMM5257) needs to validated after you are done filling in the data. Just click the validate button on the first or last page. But remember to check your details carefully before validating. If you notice any error, please fix it and revalidate. You may refer Form Fill-in Guide. If your passport does not specify on Family or Given Name such as Malaysian Passport, your name is considered as a full name. Fill it into Family Name Column ONLY. LEAVE GIVEN NAME COLUMN BLANK. There’s an instruction on every column, please read it carefully. The information of employment/activity on page 3 must be filled with latest details despite not working, housewife, retired or studying. If retired, please mention so and follow with previous 10 years of employment.

b) Family Information IMM5645 c) Visa Office Instruction IMM5890 (Note for Passport Copy: include all the stamped page copy)

d) VFS Consent Form Family Information IMM5645, Visa Office Instruction IMM5890, VFS Consent Form not needs to be validated. You can also choose to hand write them. However, please write clearly and properly at all columns. The top box of the form has instructions on what to fill in, read and follow it. 2) Representative / Travel Agent

If you are a representative or agent, please fill in the Use of Representative Form IMM5476 to fill in your contact details. Do not place your email or phone number in of the IMM5257 form. Also, ask your client to sign the VFS Consent Form page 2 and 3 to give consent for submission on behalf. *If you wish ICE-Holidays process for your client’s visa application, please indicate as following details in Representative Form IMM5476,

3) Visa Fee Payment Visa fee must in Canadian Dollar (CAD 100/applicant). It cannot be paid in Ringgit Malaysia or cash at the counter. Purchase a Bank Draft at any Malaysia’s local bank except for Standard Chartered & Nova Scotia Bank. Make Payment to RECEIVER GENERAL FOR CANADA. You can pay the bank in Ringgit Malaysia, they will do the conversion. Before you leave the bank, please make sure the bank draft contains the banker’s signature, the receiver name is correct, the amount is correct and there’s Canadian bank address printed on it. Remember to write your name and passport number at the back of the draft. If you wish to apply the visa fee via credit card please provide us the receipt emailed to you later by CIC as proof of payment. 4) VFS Processing Fee VFS Processing Fee is only payable in cash and it is RM 115.35/applicant (Excludes ICE-Holidays processing fee RM 50.00 per applicant)

5) Prepare 2 passport sized photos (Visa Application Photograph Specifications) with natural face expression, white background and taken within months’ time. Write applicant name and date of birth at the back. Please make sure there is no stain on the photos.

6) Original passport and expired one if you have any. In a situation, you need to use your original passport while the processing is being done, make a copy if the passport. Remember, you still need to show the Centre’s officer your original passport.

If a minor child (Under age of 18) is travelling with one parent only Children under the age of 18 are considered minors in Canada. They must follow the same rules to enter Canada as any other visitor. Minors who try to enter Canada without the proper documents, or who are with adults other than their parents or legal guardian(s), will be checked more closely. Border services officers are on alert for children who need protection and check very carefully for missing or runaway children. They may question you about children who come with you to Canada or question a child who travels alone. Make sure you have the proper documents with you. The parent should present: The child’s passport A copy of the child’s birth certificate, and A letter of authorization, in English or French if possible, which is signed by the parent who is not travelling with them and lists:

the address and telephone number of the parent who is not travelling, and a photocopy of that parent’s signed passport or national identity card.

If the parents are separated or divorced, and share custody of the child: The parent travelling with the child should carry copies of the legal custody documents. It is also best to have a letter of authorization from the other parent who has custody to take the child on a trip out of the

country. If the parents are separated or divorced and one of them has sole custody of the child: The letter of authorization may be signed by that parent only and they should bring a copy of the custody decree. If one of the child’s parents is deceased: The travelling parent should bring a copy of the death certificate.
