Student: ______________________________________________________________________ Last Name First M.I. Email Parent/Guardian: _______________________________________________________________ Name Email Address: ______________________________________________________________________ Home Phone #____________________________ Work Phone #_________________________ I have read the IB literature and commit my full support to my child’s participation. I understand that meeting the eligibility requirements does not ensure admission to the IB program; and, it is clear that admittance into the AYA Pre-IB program does not guarantee automatic acceptance into the IB program. I also understand that if there is a favorable decision for admittance into the IB program, my child’s success in the program is contingent upon his or her continued demonstration of excellence through graduation. The decision of the screening committee is final and cannot be contested. I will ensure that my child has uninterrupted study time daily and will supervise the use of the study time in the most productive way. I will also commit my support to the IB teachers by following their guidance regarding further study in order to ensure my child’s successful completion of the program. I understand that the International Baccalaureate curriculum is prescriptive and as such IB staff must teach what IB prescribes. I further understand that in addition to the courses and exams, in order to fulfill IB Diploma Program graduation requirements, my child must complete successfully CAS (Creativity, Action, and Service), Theory of Knowledge (TOK), and an Extended Essay (EE) prior to the end of his or her senior year; thus, it is critical to pay attention to due dates and deadlines. Please note that since CAS must be completed beyond the regular school day, it becomes the parents’/guardians’ and students’ responsibility to maintain open communication with the CAS mentor in the event of challenges preventing completion of the 18 months required. CAS hours will also help to meet the volunteer requirements for the National Honor Society. If a student decides to leave the program during grade 10 or 11, a request must be explained fully in writing, and submitted directly to the IB Coordinator followed by an exit interview. Further, I understand that should my child decide to leave the IB program, he or she must meet all graduation requirements as specified by American Youth Academy, which still includes high academic rigor. If my child is accepted either to the AY Pre-IB or IB program, I agree to support his/her participation. Further, I agree to attend all AYA Open Houses and mandatory Pre-IB/IB Parent meetings. ___________________________________________ __________________ Signature of Parent/Guardian Date
