Physiological Units

G. ThEWS

International system of units. In recent years various international societies and organizations have recommended that a new system of units be introduced to standardize the physical and chem­ical quantities used in physiology, as in other branches of science. Many countries have fol­lowed this recommendation, passing laws that require the use of the new system. This Interna­tional System of Units (SI, for Systeme Interna­tional d'Unites) is based on the seven units listed in Table 1.

These base units are defined as follows: Meter (m) - The meter is the length equal to 1,650,763.73 wavelengths in vacuum of the radiation corresponding to the transition between the levels 2plO and 5ds of the krypton-86 atom.

Kilogram (kg) - The kilogram is the unit of mass; it is equal to the mass of the international prototype of the kilogram.

Second (s) - The second is the duration of 9,192,631,770 periods of the radiation corresponding to the transition between the two hyperfine levels of the ground state of the cesium-133 atom.

Ampere (A) - The ampere is that constant current which, if maintained in two straight parallel conductors of infinite length, of negligible cross section, and placed one meter apart in vacuum, would produce between these conductors a force equal to 2 x 10-7 newton per meter of length.

Kelvin (K) - The kelvin, unit of thermodynamic tem­perature, is the fraction 1/273.16 of the thermodynamic temperature of the triple point of water.

Candela (cd) - The candela is the luminous intensity, in a

diation of frequency 540 x 1012 hertz and that has a radiant 'intensity in that direction of (1/683) watt per steradian.

Mole (mol) - The mole is the amount of substance of a system which contains as many elementary entities as there are atoms in 0.012 kilogram of carbon-12.

The units of all the other quantities can be de­rived from these base units. Some of them are listed in Table 2. The numerical values of the quantities in Tables 1 and 2 often contain powers of ten as factors. To simplify specification of numerical data, certain commonly used powers often have been assigned special prefixes (Table 3) which are combined with the names of the units in question. The conventional units listed in Table 4 can con­tinue to be used along with the SI units.

Conversion among units. Within the prOVISIOns of the SI system, concentrations can be given as amount of substance per unit volume (moljl, mmoljl, Jlmoljl) or as mass per unit volume (g/l, mg/l). It is recommended that the substance con­centration always be used for chemically uniform substances of known molecular weight (relative molecular mass). The mass concentration is a use­ful measure in the case of mixtures of dissolved substances - for example, total plasma protein. The conventional concentration units g% (g/ dl), mg% (mg/dl) and meq/l can be converted to SI units by reference to the relations summarized in Table 5. The new system will probably not be used con­sistently in medicine until a fairly long transition period has elapsed. Time is required not only for new equipment to become generally available, but also for the modification of unit-dependent norms to conform to the new system. Not until the most important norms in the new units have become as natural a part of the physician's store of knowledge as the old ones are now will the practicability of the proposed new system have been established. In particular, objections have been raised with regard to the replacement of the traditional unit of pressure, mm Hg, by the less

easily visualized pascal. On the other hand, the joule is being increasingly accepted as the unit of energy, in place of the conventional calorie. To help the reader become accustomed to the new units, in this book they are usually used together with the conventional units. Some of the frequently required conversions be-tween SI and earlier units are given in Table 6.
