 Dear Sir or Madam:  Thank you for using the Federal Communications Commission's (FCC) Tower Construction Notification System (TCNS). The purpose of this electronic mail message is to inform you that the following authorized persons were sent the information you provided through TCNS, which relates to your proposed antenna structure. The information was forwarded by the FCC to authorized TCNS users by electronic mail and/or regular mail (letter).  Persons who have received the information that you provided include leaders or their designees of federally‐recognized American Indian Tribes, including Alaska Native Villages (collectively "Tribal Nations"), Native Hawaiian Organizations (NHOs), and State Historic Preservation Officers (SHPOs). For your convenience in identifying the referenced Tribal Nations and NHOs and in making further contacts, the City and State of the Seat of Government for each Tribal Nation and NHO, as well as the designated contact person, is included in the listing below. We note that Tribal Nations may have Section 106 cultural interests in ancestral homelands or other locations that are far removed from their current Seat of Government.  Pursuant to the Commission's rules as set forth in the Nationwide Programmatic Agreement for Review of Effects on Historic Properties for Certain Undertakings Approved by the Federal Communications Commission (NPA), all Tribal Nations and NHOs listed below must be afforded a reasonable opportunity to respond to this notification, consistent with the procedures set forth below, unless the proposed construction falls within an exclusion designated by the Tribal Nation or NHO. (NPA, Section IV.F.4).  The information you provided was forwarded to the following Tribal Nations and NHOs.  If a Tribal Nation or NHO does not respond within a reasonable time, you should make a reasonable effort at follow‐up contact, unless the Tribal Nation or NHO has agreed to different procedures (NPA, Section IV.F.5). In the event a Tribal Nation or NHO does not respond to a follow‐up inquiry, or if a substantive or procedural disagreement arises between you and a Tribal Nation or NHO, you must seek guidance from the Commission (NPA, Section IV.G).  These procedures are further set forth in the FCC's Declaratory Ruling released on October 6, 2005 (FCC 05‐176).          1. THPO Emily Smith ‐ Winnebago Tribe of Nebraska ‐ Winnebago, NE ‐ electronic mail and regular mail Details: The Preservation Office of the Winnebago Tribe of Nebraska now charges a research and review fee in the amount of $125 for all TCNS matters.  Please make the check payable to "The Winnebago Tribe of Nebraska," and mention the TCNS number in the memo line.  Please mail to:  Emily Smith‐DeLeon, THPO Winnebago Tribe of Nebraska P.O. Box 687 Winnebago, Nebraska  68071

                16. Acting THPO Lisa C LaRue‐Baker ‐ United Keetoowah Band of Cherokee Indians ‐ Tahlequah, OK ‐ electronic mail and regular mail Details: BEGINNING WITH INVOICES DATED APRIL 1, 2014 OR AFTER: PREPAYMENT IS REQUIRED FOR REVIEWS. This includes both Section 106 reviews, and reviews to determine if properties are potentially eligible for the NRHP.  Due to the large number of accounts with latepayment or non payment, we are now adopting the policy of PREPAYMENT in order to conduct reviews. Invoices will be issued up receipt of notification from the FCC of TCNS filing, and represents a request for more information (Form 620, 621 or any other items required by either Program Comment, or request of the tribe or FCC). Reviews will be conducted within 30 days of receipt of payment, unless provisions for an expedited review have been made. We are sorry for any inconvenience this may cause any of ourclients.  NON PAYMENT: Any payments over 90 days late will be considered non‐payment and will result in termination of reviews for consultant/tower company (unless handled directly by the Federal Agency) and will be turned over to tribal court to be recommended for assignment to collection agency and notification to the Better Business Bureau. Future reviews for the consultant/tower company will require PREPAYMENT.  Our review fee is for ALL reviews, including colocations and resites, as well as making determinations if properties are potentially eligible for the NRHP. Cancelled projects are still subject to fee, as the UKB reviews the project in good faith, even if the site is later abandoned. REVIEWS AND INVOICES ARE BASED ON OFFICIAL REPORTS OF TOWERS SUBMITTED DELIVERED TO THE UKB BY THE FCC. If a site is cancelled or abandoned within the first week of listing on the TCNS system, PLEASE CONTACT THE UKB (Lisa Baker) at ukbthpo‐[email protected] to CANCEL to avoid review and invoicing. We respond within expected timeframes, we expect payment in 30 days, as well.  YOUR SECTION 106 obligations with our tribe are not fulfilled until you receive our written response indicating our determination.  Please do not hamper your client's ability to receive reviews and responses by becoming late with your payments!!  ALL CORRESPONDENCE COMING FROM TCNS AFTER YOUR RECEIVE YOUR INITIAL FILING NOTIFICATION ARE NOT AUTOMATED, BUT GENERATED INDIVIDUALLY AND AS NEEDED BY THE TRIBE, USING THE TCNS SYSTEM AS THE DELIVERY VEHICLE.

We have reviewed the provided SHPO response from the Illinois Historic Preservation Agency, the FCC Form 620, the Phase I Archaeological Survey, location map, site plans, photographs, aerial imagery, historical maps, engineering drawings, and topographic maps. We find after review of this information that we concur with the SHPO response and the findings of the Phase I Archaeological Survey for project clearance. Although project clearance is recommended, we remain interested in further communications regarding this project due to the location. The Shawnee people have a documented historical presence in Illinois. While there are no village sites within the project site or within a close proximity outside the project site, there still remains the potential of finding unknown sites in and surrounding the project location. It is further advised that if the area of potential effect changes or in the event of an inadvertent discovery of human remains or other cultural resources that we receive notification within 48 hours. As well, any advertent discovery of human remains or other cultural resources should remain in situ until consultation with interested tribes and agencies is undertaken. For direct effects, the findings of this National Historic Preservation Act (NHPA) Section 106 review for the proposed Martin Environmental Solutions project, TCNS 120653 is a determination that “No Properties” were identified within the Area of Potential Effect that are eligible or potentially eligible for the NHPA’s National Register of Historic Places. In accordance with the NHPA of 1966 (16 U.S.C. § 470-470w-6), federally funded, licensed, or permitted undertakings that are subject to the Section 106 review process must determine effects to significant historic properties. As clarified in Section 101(d)(6)(A-B), historic properties may have religious and/or cultural significance to Indian Tribes. Section 106 of NHPA requires Federal agencies to consider the effects of their actions on all significant historic properties (36 CFR Part 800) as does the National Environmental Policy Act of 1969 (43 U.S.C. § 4321-4347 and 40 CFR § 1501.7(a). The Miami Tribe of Oklahoma is satisfied with efforts conducted to be sure that no Miami historic properties orother Miami cultural resources will likely be adversely affected by construction and use of this tower. The MiamiTribe of Oklahoma is not currently aware of any specific Miami cultural or historic sites in this project area. Wetherefore offer no objection to the proposed construction.

Because this tower lies in the homelands of the Miami, in the event that archaeological materials, includinghuman remains, are discovered during construction or later ground-disturbing activities at this location, please re-open consultation at that time by contacting our THPO at [email protected] or (317) 625-1288.Similarly, if plans are considered to expand or modify this tower and cause additional ground disturbance in thefuture, please let us know. Please forward the Tribe's concerns regarding any such future work and unanticipateddiscoveries, as well as our contact information, to the appropriate parties.

The purpose of this email is to inform you that an authorized user of the TCNS has replied to a proposed tower construction notification that you had submitted through the TCNS. The following message has been sent to you from THPO Sherri Clemons of the Wyandotte Nation in reference to Notification ID #120653: Greetings from Wyandotte Nation. This is our affirmative act to indicate that we are interested in consulting on this tower, broadband, or PTC project. If this tower or PTC is a new build in a previously undisturbed area, please have an archaeological survey performed and submit the report to us as part of the information packet that we review. We have certain cultural information needs for raw land sites that can be satisfied only with an archaeological survey. While the information needs of SHPOs usually dictates that only one cell tower may be included in such a report, you may include multiple PTC components (poles or related facilities) in a single archaeological report for submission to us. Our archaeological procedures dated 9-9-2013 outline the level of information necessary for us to assess the cultural impact that construction will have. Please request a copy of these, if you do not already have them, and provide them to your archaeologists prior to field work. We will reject reports that do not provide the requested information and object to projects that fail to provide us with sufficient information to review such that we might assess project impact on our heritage resources. We will reject reports where the field archaeologist does not have at least a BA and two full years of field experience gained in the same or a similar geographic area. All procedures are available by e-mailing the cell tower program archaeologist, Rebecca Hawkins, at [email protected]. All further correspondence regarding this tower should also be directed to her at that email address. This is the e-mail address that we will use for all of our TCNS communications and final responses. With questions, please call her at 918-541-0782 or Lamont Laird at 918-533-2212. For PTCs, please provide us with clarification, via email, regarding which TCNSs, or which poles within an overarching TCNS, are and are not excluded poles. We will not respond further on excluded poles (or other facilities) unless we object to the exclusion. For non excluded poles, please treat our information review requirement the same as for cell towers proposed for disturbed areas and send us: proof of disturbance, maps, and selected photos. LISA HEISE NEPA Specialist

Fields of Specialization Ms. Heise currently specializes in the preparation of National Environmental Policy Act (NEPA) documentation. She has prepared numerous informal biological assessments and completed consultations with Indian Tribes, State Historic Preservation Offices (SHPOs), and wildlife offices throughout the United States and the U.S. Virgin Islands. Ms. Heise concentrates on telecommunications facility projects at this time. Experience Ms. Heise has gained significant experience conducting consultations with over 200 Federally-recognized Indian Tribes through the Tower Construction Notification System (TCNS). Through her working relationships with these Tribes, Ms. Heise acts as a liaison between clients, Tribes, and the Federal Communications Commission (FCC) ensuring that the best interests of all parties are protected. Ms. Heise has managed consultations with numerous Federal, State, and County Wildlife Offices, including the preparation of informal biological assessments and threatened and endangered species assessments. Additionally, Ms. Heise conducts consultations with SHPOs. As part of these consultations, she solicits comments from local historic preservation societies, governments, and citizens. Ms. Heise has conducted numerous nest observations for Osprey (Pandion haliaetus) to determine the status of the nests. Additionally, she has performed Osprey monitoring during construction activities to ensure the Osprey are not distressed by the construction. Ms. Heise has experience in biological sample collection and processing, water filtration, GPS navigation, bird mapping, and DNA analysis of bivalves. THADDEUS NICHOLLS Field Scientist, NEPA Specialist

Fields of Specialization Mr. Nicholls has over 10 years experience in the field of environmental consulting, including performing biological surveys, field data collection and analysis, and wildlife monitoring. His specific skills are within the realm of coastal and coral reef ecology, and Living Shoreline habitat restoration and enhancement practices. Experience Mr. Nicholls has significant experience monitoring various endangered and protected wildlife species such as bald eagles, golden eagles, Swainson’s hawk, red-tailed hawk, osprey, and live-trapping/ handling of the endangered species the Preble’s meadow jumping mouse. He has a great deal of experience collecting field data, including underwater data collection using SCUBA and underwater photography. Mr. Nicholls' skills also include performing various parametric and non-parametric statistical analyses on collected field data. His Living Shoreline work involved habitat restoration and enhancement of eastern oyster (Crassotrea virginica) and Spartina alterniflora tidal grass habitats.

In addition to performing field work, Mr. Nicholls also writes and prepares materials required for SHPO submissions and NEPA reports, and performs numerous Phase I and Phase II Environmental Site Assessments.

Fields of Specialization Ms. Leman has over 25 years of diversified experience in the environmental field, including, but not limited to, Phase I and Phase II Environmental Site Assessments, National Environmental Policy Act (NEPA) reviews, subsurface investigations, regulatory compliance and permitting (air quality, wetlands, generators, and tanks), hazardous waste management, environmental microbiology studies, bio-remediation studies and implementation plans, environmental health risk assessment and management, indoor air quality studies, industrial hygiene, radon studies, lead-based paint studies, asbestos identification and remediation/abatement, quality assurance/ quality control procedures, and sample collection and analysis of water, wastewater, soils, and air. She also has provided testimony as an expert witness in environmental contamination cases. Ms. Leman specializes in telecommunications facilities at this time. Experience Prior to joining the firm, Ms. Leman was responsible for regulatory compliance with applicable environmental regulations for a national company within the telecommunications industry. She has significant experience in conducting and managing Phase I and Phase II Environmental Site Assessments and NEPA projects for national companies, as well as the implementation of SARA Tier I and Tier II reporting, hazardous waste management, and hazardous substance storage programs.
