APPENDIX ‘B’ FORM OF PROPOSAL (Fillable Form)

The City is providing Appendix ‘B’ as a fillable form in Microsoft word format, for ease of completion. The tables below will be expandable. Please provide your response in the areas which are not highlighted.

The Form of Proposal must be signed by a person authorized to sign on behalf of the Respondent and to bind the Respondent to statements made in response to this RFP and include the following:(a) If the Respondent is a corporation then the full name of the corporation should be included,

together with the names of authorized signatories. The Proposal should be executed by all of the authorized signatories or by one or more of them provided that a copy of the corporate resolution authorizing those persons to execute the Proposal on behalf of the corporation is submitted;

(b) If the Respondent is a partnership or joint venture then the name of the partnership or joint venture and the name of each partner or joint venture should be included, and each partner or joint venture should sign personally (or, if one or more person(s) have signing authority for the partnership or joint venture, the partnership or joint venture should provide evidence to the satisfaction of the City that the person(s) signing have signing authority for the partnership or joint venture). If a partner or joint venture is a corporation then such corporation should sign as indicated in subsection (a) above; or

(c) The Respondent is an individual, including a sole proprietorship; the name of the individual should be included.

In the case of joint submissions, one party must assume overall responsibility, for communications within the RFP process and be identified accordingly in the Proposal, but all parties must be jointly and severally responsible for completion of any resulting Contract.

SECTION 2: PRICING FOR CHAIRS

Respondents are to provide their price for the chairs, including delivery. If two (2) makes/models are provided, complete a table for each make/model.

(a) All unit pricing is in Canadian Dollars;(b) Applicable taxes are shown as a separate line item;(c) All unit pricing is an all inclusive lump sum price and includes but not limited duties,

insurance in transportation, customs clearances.(d) Freight charges F.O.B. Destination, freight Prepaid and allowed to Bowen Park Complex

located at 500 Bowen Road, Nanaimo, British Columbia V9R 1Z7 and includes delivery inside the building at a designated area;

(e) Any Respondent providing chairs from outside Canada shall provide such chairs INCO terms DDP destination;

(f) All pricing is firm;(g) If the Respondent believes that any other cost element(s) have been missed and/or should

be considered, please add the total cost under miscellaneous Extended Total

In the case of discrepancy between the unit price and its extension, the unit price will be deemed correct. Calculation errors will be corrected to reflect the accurate Net Total Cost figure based on the unit prices submitted.

SECTION 4: TERMS OF PAYMENT
