Crisis DescriptionCrisis faced :• The 2nd oxygen tank in the C/S module exploded• The 1st oxygen tank was also damaged and started leaking• Not enough oxygen to support the 3 astronauts for journey

back to earth• Not enough oxygen to generate electricity for the spacecraft

Decision taken:• To shift to lunar module for the return journey and use it as a

lifeboat

Challenges of the decision:• LM had oxygen and supplies enough for only 2 people• Team had to learn to fly the thing all over again• Telephone Systems of LM were not working properly
