Your child has signed up to participate in the Apocalypse Lock-In at the library. This event celebrates the Mayan calendar ending on 12/21. Teens will be participating in a variety of activities such as trivia, making zombie survival plans and watching clips from various Apocalypse movies.

Adults from the Sellers Library staff will be supervising at all times. Once a participant arrives at 8:00 for the program, he/she will not under any circumstances be allowed to leave the library. If, however, a participant does not behave in a manner respectful of other students or staff, a parent or guardian will be required to come pick him/her up immediately. In case of an emergency only (not to check up on your child), library staff can be reached at 610-789-4442 during the program.

An adult must come to pick up the participants at 12:00 midnight. Only the adult designated on this slip will be allowed to pick up the participant. (One adult may pick up several participants, but that adult MUST be listed on this permission slip). No one will be allowed to leave with a person not listed on this slip. No one will be allowed to walk or bike home alone or in a group without an adult.

This program is limited to 60 participants. To be fair to all participants, registration is not final until this completed permission slip is returned. Because we care about your child and want to keep him/her safe, this information is required:
