AFO’sAn AFO (Ankle Foot Orthotic) is a special brace that is designed to help control the motion of the ankle jointand also to provide support to the foot. They can be custom made, or prefabricated.

A functional AFO should provide superb control to the foot and ankle while offering greater comfort andeasier shoe fit than other braces.

Carbon FibreA Carbon Fibre AFO is material that is used to give greater strength. Some AFO’s are entirely made out ofthis material and are strong as well as flexible.

Carbon fibre can also be included as material when producing other orthotics.

Ankle unilateralAn ankle unilateral AFO is designed to be one sided. This may be applied on the medial or lateral side ofone foot and provides greater control and support for that side.

Night splintA night splint is an AFO which is used at night, where it is necessary to hold and support the foot in aneutral position. Night splints can be used in the evening to improve calf muscle flexibility. They can also beused during sleep to prevent night time contracture of the calf muscle to ensure that the first steps takeneach morning are not so painful.

They are also useful to hold the foot in the correct position for those confined to bed rest for care. Thisreduces risk of deformity.

Foot drop liftA foot drop lift is an AFO used by patients with ‘dropped foot’. This prevents the foot from dragging andclawing as they walk. It stops the foot dropping and works to lift the foot to improve walking.

Leaf SpringA leaf spring is an AFO which has a retaining spring effect enclosed. This provides stability for the foot incertain positions. It reduces foot drop.

Suitable:• Foot drop

• Hemi paresis

• Charcot Marie Tooth (CMT)

Shoe fit:• Many shoes are available with removable inlays which will accommodate this type of orthoses

• Sport footwear

• Sandals with removable inlay

• Prefabricated or custom Medical Grade Footwear

Contra indication:Not suitable for significant varus or valgus deformity and for sensitive skin such as an insensate foot(Diabetic foot), unless significant padding is provided. In the latter case a shoe fitting with extra depth, widthand length or custom made medical grade footwear may need to be considered.

The sample below has additional elastic straps on the dorsal side of the foot.

Ankle enclosingAn ankle enclosing AFO surrounds the ankle, part of the lower limb, or foot. It provides side stability,reduces foot drop and can restrict painful movement.

Suitable:• Foot drop including those with varus or valgus foot alignment

• Hemi paresis with varus or valgus foot alignment

• Suitable for moderate to severe foot support needs

• Cerebral Palsy. Recommended in combination with senso control foot orthoses

• Charcot Marie Tooth (CMT)

• Severe foot support needs in combination with supportive footwear

• Flat feet

• PTT disorders in combination with supportive footwear

• Arthritis

• Various foot pathologies essentially those that require stability or restriction around the ankle and foot joints.

Shoe fit:• Many shoes have removable inlays and will accommodate this style orthoses

• Sport footwear

• Sandals with removable inlay

• Prefabricated or custom Medical Grade Footwear

Contra indication:Not suitable for sensitive skin in the insensate foot (e.g. diabetic foot) unless significantly padded. Toaccommodate the padding a shoe fitting with extra depth, width and length or custom made medical gradefootwear may need to be considered.

OrthosesOften called orthotics, these are also known as ‘shoe inserts’ or ‘arch supports’. Primarily designed to re-position your foot into a more functional position whilst you walk or run, they may be firm or soft and aremade from a variety of materials.

Orthoses will reduce pain levels and improve stability and co-ordination. They may be useful to those thathave a disability and/or symptoms in everyday life, who may be using an orthotic or special footwear already.

Orthoses may also be used by those that do not have any obvious deformity but may have pain orsymptoms during or after sport activity, whether for leisure or at a competitive level.

Sensomotoric® Orthoses (It’s a new concept)Exclusive to the Footpower clinic, sensomotoric® orthoses are orthotic devices which utilise theproprioceptive system in the body.

This orthotic concept was first published by Lothar Jahrling in Germany. We utilised our association and co-operation with him in providing our expertise for your benefit.

They aim to improve the foot function, instead of functional substitution and can help by providing balance.They minimise the stress forces that can ultimately cause further pain or deformity in the feet.

The sensomotoric® theory has been described by Dr. W. Laube, MD, Sports Physician and elite athlete in anumber of his books. It is well documented as a theory in rehabilitation, manual therapy and physiotherapy.The body has various receptors which constantly check body position, muscle tone including movement.Sensomotoric® orthoses use those principles to improve foot function and to guide you to better movementpathways.

The theory behind sensomotoric® foot orthoses revolves around the natural response of the foot and bodyin relation to how the ground is felt by the feet.

The insoles are designed to alter how the foot ‘feels’ the ground in particular the timing of ground contact.The body has a preferred movement pattern whether it is the best pattern for you or not.

Body movement and functionality is not always perfect, in which bad habits can eventuate and becomerepetitive. In sport it is well accepted that you have a coach or trainer who points out the “wrong” movementtechniques whilst teaching the correct ones.

You then have to learn the improved movement pathways and repeat them until they become your newmovement pathways. Sensomotoric® orthoses are your personal trainer for your feet inside your shoes.

Thin Type Foot OrthosesEver wanted your foot orthoses to suit your different styles of footwear?

Thin Type Foot Orthoses may just be the right ones for you.

Suitable for conditions:• Mild to moderate foot support needs

• Flat feet mild to moderate

• Metatarsalgia or forefoot pain

• Pain under the ball of your foot

Shoe fit:Most footwear including a more elegant style, up to a medium heel height.

Not suitable for the following condition:• Sever foot problems

• PTT dysfunction

• Strong flat feet

• Neuropathy

These are thin type or Super Slim orthotics to fit into shoes and to some extent into sandals whichspecifically have minimal space.

You can choose the colour of the lining material to suit your footwear.

They are designed to help with common foot ailments, and to provide a broader choice of footwear. Theycan be full or short length depending on both taste and needs.

The thin type does not have a heel cup and as such can adjust to a varietyof heel heights. They typically have a firmer plastic element supporting yourfoot in both medial and lateral arches including transverse.

A metatarsal dome as well as limited padding or posting can be addeddepending on your needs. They may not be suitable for severe footconditions.

EVA BaseEVA is (Ethyl Vinyl Acetate) a light synthetic rubber, frequently used to make foot orthoses. EVA hasexcellent cushioning properties. We customise foot orthoses in a variety of hardness or softness dependingon individual requirements.

EVA orthoses are versatile with both a vast array of applications and material combinations. They can bemade accommodative such as TCI / TCO or with corrective elements.

Not suitable if a very rigid device is needed.

Your EVA base foot orthoses are made using a thermoplastic process or computer aided milling whichbrings the material into the desired shape.

The EVA is often reinforced with a thin layer of flexible stiffener to provide added strength. The archelements are made from soft but resilient foam to give your arch a strong but cushioning support. They arethen covered with a soft full length layer. The EVA base foot orthoses are extremely versatile. Depending onyour individual needs, material combination can vary from firm to super soft.

UCBL / Rigid Foot orthosesThe University of California Biomechanics Laboratory or UCBL foot orthoses are named after the laboratorywhere they were researched and developed.

UCBL orthoses are rigid with a deep shell like shape.

Suitable:• Suitable for severe foot support needs

• To hold the foot in a particular shape

• Reduce further deformity

Contra indication:Not suitable for sensitive feet

The UCBL foot orthoses are used to stabilize a flexible foot deformity. In most cases that applies to aflexible flat foot.

The UCBL is different from other foot orthoses in that it has a deep heel cup and shell like constructionwhich in turn holds the heel, or hind foot in a neutral vertical position. While correcting and holding the heelin a neutral position, the UCBL also controls the medial arch of the foot and the outside border of theforefoot. These three corrective forces keep the foot held in a neutral position.

It is very important that the foot must be flexible enough to be held in a neutral position comfortably. If thefoot deformity is rigid the UCBL will be very uncomfortable if used in an attempt to correct the deformity. Ifthe foot is rigid, the UCBL can be made to the shape of the foot and the goal is to prevent further deformity.

The UCBL is made out of a rigid material, usually plastic, that is moulded over a cast of the foot. The rigidmaterial usually ends at the ball of the foot to allow a smooth roll though process.

Total Contact Inlay – TCITotal Contact Orthoses – TCOA TCI or TCO is a moulded device which is designed to follow the plantar contours and tomography of thefoot. They reduce plantar pressure at points where they are higher than normal by offloading weight acrossa larger area of the plantar surface. Both TCI and TCO are usually made of soft material.

Suitable:• Arthritis

• Diabetes

• Sensitive feet

• Redistribution of plantar pressure

Shoe fit:• Sport footwear with extra depth

• Sandals with removable inlay and extra depth

• Prefabricated medical grade footwear

Contra indication:Not suitable if stronger support is needed or high volume usage. EVA base style orthoses are available insoft material. However our range includes styles that are both soft with added durability and support.

This is a total contact orthoses or custom moulded foot orthoses.

It is accommodative and made using soft tri laminate material(Plastazote PPT EVA) as well as EVA cork. It is suitable for sensitivefeet. The materials will self mould on the surface plantar to the footwhich will make it more comfortable after a little usage. The orthosesrequires extra depth inside footwear.

Hallux Valgus Night SplintA Hallux Valgus is most commonly treated and prevented with some simple adjustments to your shoes. Inorder to help alleviate pain from a Hallux Valgus, an orthotic device may be inserted to redistribute thepressure along the arch and ball of the foot. It should support the foot, and reduce the tendency towardHallux Valgus formation.

A night splint can be used to hold the big toe in a corrected position. This reduces the HV development.

Carbon InlayCarbon inlays provide more rigidity to shoes. They may be required where too much flexibility in a shoedoes not support the foot. This can be the case where inappropriate movement in the feet causes pain orincreases deformity.

The carbon inlay can be removable, or can be built into the footwear or orthoses and level of rigidity varied,depending on what is required.

Custom Orthopaedic Footwear / Medical Grade FootwearCustom made orthopaedic footwear is made specifically for each individual patient from a cast. Therequirement for each foot is completely individual; the Foot Power clinic prepares all custom orthopaedicfootwear on the premises.

COF is usually required when standard footwear is unsuitable, due to high risk, severity of problems orfinally, degree of deformation.

Foot conditions which may result in the requirement of custom orthopaedic footwear are:

Shoe ModificationsInternal shoe modifications are changes made to inside the shoe to relieve pressure or shear forces thatcan cause pain, irritation or trauma to the foot. External shoe modifications are made on the sole and orheel of the shoes.

Heel / Shoe raisesA heel raise is placed inside the shoe to balance leg lengths and align the pelvis. It can also beincorporated in an orthotic for other conditions.

Higher heel raises are built into the heel externally.

A shoe raise is placed externally to the heel and sole of the whole shoe. The relative distribution on theshoe raise depends on the foot requirements.

Rocker Soles / BarsRocker soles or bars can be added as a modification to regular footwear. They can also be incorporated into the construction of custom footwear.

There are several kinds:

The Rocker bottomThe rocker bottom applies to the entire shoe bottom, from heel to toe, which is rounded to help your foot rollthrough the process. However, typically there is some area of the shoe bottom made flat to provide stabilitywhen standing.

The Rocker soleThe rocker sole creates a roll through from the mid to the forefoot, depending on the placement of the apexline.

The heel RockerThe heel rocker applies to the heel only, reducing impact by delaying the heel impact and helps in a stiffrear foot.

Metatarsal Rocker SoleMetatarsal rocker soles are made with the apex line proximal or behind the metatarsal heads to reducepressure for the metatarsal heads.

The horseshoe rocker sole or butterfly rocker sole is a variation of the metatarsal rocker soles with extra

cavity and padding for specific metatarsal heads.

The Hallux rigidus / Limitus rocker sole has a specific cutout to compensate for the lack of mobility of the1st MPJ.

Metatarsal domesMetatarsal domes and platforms relieve pressure from specific metatarsal heads or the entire transversearch of the 2nd, 3rd or 4th metatarsal head. This modification is incorporated during the orthoticmanufacture.

It can also be an internal shoe modification.

Velcro conversionsA velcro conversion is a modification to a shoe whereby a velcro strip is attached where laces used to be.

WedgesA wedge is a shoe modification consisting of an insert incorporated into the heel of the orthotic. The wedgeacts to align the calcaneus and mid-foot into midline, thus decreasing the strain on joints, ligaments andtendons.

Well balanced wedges can reduce pain on a medial or lateral knee joint component.

Wedges can be internal or external, covering the heel only or full shoe length.

FlaresFlares are shoe modifications which are commonly medial or lateral. Strips of material are applied to thewhole sole or heel only depending on the requirements.

Essentially flares are used to widen the shoe, both sole and heel and provide stability, and improve balanceetc.

RelastingRelasting is a process where a shoe bottom (heel & sole) is substantially widened or changed in shape.The existing shoe is made to a different last.

This is particularly useful to widen the mid foot to medial or lateral for wide flat feet or Vagus feet.

Gait AnalysisGait Analysis is an important first step to assessing where your feet may need support.

At the Foot Power clinic, a sophisticated computerised plantar pressure measurement system is employedto analyse your gait and help develop the optimum treatment plan.

Empiric gait analysisEmpiric gait analysis involves the Foot Power practitioner assessing your walking or running style. This isincluded in the basic consultation fee.

Computerised gait analysisThis form of analysis utilizes two cameras and a sophisticated software program which analyses yourpersonal running/walking style, ultimately generating a detailed report.

MBT Footwear RepairsAs the official repairer for MBT Footwear, we have implemented a few changes to the repair process. Theseinclude:

• A new format for repair forms – to improve turnaround time

• All repairs will be returned directly from Foot Power

Despite the metal calliper the foot and ankle twists with every step.

The right is the boot from Foot Power Pty. Ltd. You can see the foot and ankle is far more stable with ourboots.

There is no “twisting” in the ankle with our boots. We have reviewed this person over several years and theboot performs the same after using it for several years. However, some repair of worn heels and soles needto be done from time to time.

The left picture shows the plantar pressure values in a neutralshoe. The neutral shoe is simulating a “normal-standard”footwear without an specific feature and is our base referencerecording. Plantar pressure in the neutral shoe is very high asyou can see on the red color.

The middle picture shows the plantar pressure values asmeasured in the total contact cast that facilitated healing of theulcer. The right picture shows the plantar pressure measurement

in the custom made medical grade footwear. You can see clearly that the medical grade footwear providedby ShoeTech provided peak plantar pressure reduction better than the total contact cast and protected thefoot successfully from further ulcers for many years.

This chart shows the evaluation of the plantar forces forthe Charcot foot over several steps.

It compared the total forces in the neutral shoe, the totalcontact cast and the medical grade footwear provided byFoot Power. It compared 3 sections of the foot. On thevery left the entire force acting on the foot is displayed.That shows an increase of overall plantar forces with thetotal contact cast and the medical grade footwear by FootPower. This is a positive sign as it indicates that theperson is now walking with more confidence by placingmore overall load on this foot.

The middle graph identifies the plantar forces for theareas where the ulcer was. Here it is very clear that the forces have been reduced comprehensively by theFoot Power shoes. The right graph includes the area around the ulcer and again the Foot Power shoeidentifies the best result.

Initial ConsultationThe first appointment is a great opportunity to let us know what problems you have and how we might beable to help you.

What to expect at your first appointmentFirst consultations provide an opportunity to examine your feet in the context of your lifestyle. To helpassess your condition thoroughly and provide the best possible solution, it is a good idea to bring to yourfirst appointment:

• Samples of your current worn footwear

• Any doctor’s referrals, reports, scans or x-rays relating to your feet, ankle, leg or hips

• Any orthoses you may have previously been prescribed, or have worn

• A list of questions or issues you would particularly like to cover

At your first appointment a Foot Power practitioner will:

• Ask you about your lifestyle and the types of activity you undertake, to get an understanding of yourparticular needs and preferences

• Perform a range of assessments designed to analyse your individual requirements, including an emphiricgait analysis

• Ask about past problems with your feet or related pain

• Provide advice, options and expert recommendation as to the best solutions for your particular condition

• Discuss funding options that may be available to you

What to doAn appointment is needed to attend a Foot Power Clinic.

A fee is charged for all clinical services and is payable at the time of the consultation. Credit card facilitiesare available. Other acceptable forms of payment are by cheque or cash.

Studying current and past shoes can give important information to us about what works and what doesn’twork and why.

Such as if your foot swells a great deal, a measurement of when it is swollen, and when it is not is helpful. Further information can be given to you when you make the appointment.

Sometimes during a consultation people forget to mention special problems they have or features they want, and sometimes it is only after shoes are made that you remember. Thinking about this prior to your appointment, and providing us with your information can be very useful.

Private Health FundsWe are registered as providers with most major Australian health funds, providing a quality service topatients, and working to the highest medical grade footwear industry standards.

A referral is usually recommended with claims from health funds.

Private Health Funds cover the products we provide.

Workers CompensationWe provide a service for patients using workers compensation.

We work with your insurance company throughout the process.

You will need to have a referral for our services. Furthermore, you will need to provide us with contactdetails of your insurance, claim number and case manager. Please advise us at the time of booking yourappointment that you will be covered by workers compensation.

Third Party Accident InsuranceWe work with your insurance company. You will need to have a referral for our services. Furthermore youwill need to provide us with contact details of your insurance, including the claim number and case manager.

Please advise us at the time of booking your appointment that you will be covered by Third Party AccidentInsurance.

PADP / Enable NSWPADP is the Program of Appliances for Disabled People.

Enable NSW is the state governing body that controls PADP.

We as a service provider work with that program. We are eligible prescribers and suppliers under NSWhealth guidelines.

The program does not cover consultation fees. The program is means tested for persons over the age of16. Children are exempt from a means test.

Below are some questions commonly asked by Foot Power clients. If your question is not answered, contact the Foot Power clinic for assistance.

Are Foot Power consultations covered by Medicare?Foot Power consultations and products cannot be claimed on Medicare. However, most private healthfunds will cover the costs of Foot Power products. Unfortunately the Foot Power consultation cannot beclaimed.

The level of private health insurance cover will determine exactly what can be claimed by each patient. Notall health funds require a referral but it may be a good idea to have one on file for potential claims.

Are referrals necessary?A referral is not required for a Foot Power consultation. However in cases of complex problems, or whereprevious treatment or surgery has taken place, it is beneficial for your Foot Power practitioner to know themedical history of diagnosis. Referrals are also useful with health fund claims.

Are home visits available?Foot Power practitioners are available for home visits, in cases where patients are unable to attend theclinic. These are charged on a case by case basis, as the cost allows for travel time. To enquire further,contact the Foot Power clinic.

I have Private Health Insurance, can I claim anythingback from them?We are registered as providers with most major Australian health funds, providing a quality service topatients, and working to the highest medical grade footwear industry standards. A referral is usuallyrecommended in relation to claiming back from health funds.

Our products are refundable by Private health funds.

I have a Workers Compensation / Third Party Claim?Yes we work with your insurance provider.

You will need to have a referral for our services. Furthermore, you will need to provide us with contactdetails of your insurance, claim number and case manager.

Is Foot Power endorsed by the Department of VeteranAffairs?Foot Power is an approved provider for the Australian Department of Veterans Affairs. However a limitedpart of our services is covered by DVA.

All DVA patients require a referral from an orthopaedic surgeon, rheumatologist or podiatrist. Please adviseat the time of booking that you would like to utilise the DVA cover.

How long do Foot Power orthotics take to customise?Foot Power orthotics take approximately one week to customise.

At the time of the consultation we arrange a fitting appointment which is usually within seven days.

What follow up appointments are necessary?In the weeks following the initial supply of the orthotics a full after sales service including a review of fittingand adjustments is provided. Training and advice is also provided to the patient’s primary carer ifnecessary, to ensure proper use of the product.

What product guarantees does Foot Power offer?Foot Power offers the highest quality in service, productivity and workmanship. We guarantee that ourservice is of the highest standard and we offer free follow-up service on custom made footwear andorthotics up to and inclusive of three months.

What are the Foot Power clinic opening hours?The Dee Why clinic is open Monday – Friday, 9am – 5.30pm plus one Saturday per calendar month 9am –1pm. Foot Power also has a clinic in the Sydney suburb of Concord, open every second Wednesday.

What public transport options are available to get to theDee Why clinic?From Wynyard station

• The following buses travel to the Northern Beaches from Wynyard station via Warringah Mall and thencontinue to Dee Why: 178, 180, 185, L80, L88 and L90

From Manly Wharf

• If you are catching public transport to Dee Why via Manly Wharf, buses depart throughout the day fromthe bus stop just to the left outside the wharf entrance. The following buses travel from Manly wharf directto Dee Why: 155, 156 and 169.
