Affordability is critical, too

For employer groups of 2-50, Anthem dental offers fl exible plan designs that fi t into our Value, Classic and Enhanced categories -- so you can choose the network and level of benefi ts that fi t your needs and budget. We also offer Affordable Care Act (ACA) dental plans including dental pediatric EHB plans, with the option of child, adult or family.

Employer groups with 51+ employees have additional fl exibility by choosing network, coinsurance, deductible, annual maximum and orthodontic coverage options. You can customize a plan that works best for you.

In addition, we also offer voluntary dental plans, which allow you to offer dental coverage to your employees without adding to your expenses. And, our voluntary plans only require that fi ve or more employees enroll.

Dental networks that work hard for you Anthem Dental Prime and Complete members have access to large local and national networks. In fact, with Dental Complete, members have access to more than 95,000 unique dentists across the country, with more than 220,000 access points, through the national Dental GRID.1

The national Dental GRID links dental networks, including the dental networks of many of the nation’s Blue plans, and includes dentists in all 50 states2 — so your employees can fi nd a dentist wherever they live, work or visit.

When employees see a network dentist they can count on:

Savings: We agree on pricing with our network dentists and then pass that lower price on to your employees.

Freedom from paperwork: Network dentists fi le your employees’ claims with us.

Freedom from balance billing: If our network dentists charge more than the agreed upon price, your employees would only be responsible for the applicable deductible or coinsurance.

And for members working or traveling outside the U.S., we automatically include care for international emergency dental services through a worldwide program of English-speaking dentists — including 24/7 assistance fi nding a network dentist.3

Participation Guidelines

Employer-paid plans:– For groups with two to four eligible employees, 100% of eligible employees (a minimum of two employees), not covered under another dental plan, must enroll. – For groups with five or more eligible employees, a minimum of 60% of eligible employees (a minimum of two employees,) not covered under another dental plan, must enroll (exception: employers located in CT, require a minimum of 60% eligible employee, a minimum of two employees, not covered under another dental plan, must enroll).

Voluntary plans:– Minimum of five enrolled employees, with no participation-percentage requirement.

Orthodontia coverage: – Minimum of 10 enrolled employees (exception: employers located in CO and NV need a minimum of five enrolled employees).

As of March 2014, the Dental Complete network has no contracted dentists in the following counties: Crowley, Dolores, Gilpin, Grand, Hinsdale, Jackson, Kiowa, Mineral, Ouray, Phillips, Pitkin, Rio Blanco, Saguache, San Juan, and Sedgwick. This policy does not include pediatric dental services as required under the federal Patient Protection and Affordable Care Act. This coverage is available in the insurance market and can be purchased as a stand-alone product. Please contact your insurance carrier, agent, or the Federally Facilitated Exchange if you wish to purchase pediatric dental coverage or a stand-alone dental services product.

Out-of-network providers will bill you for amounts over what your plan pays, up to their usual charge.
