ANSWERS TO CHAPTER CASES
The case studies in Chapter 15 require the completion of twenty claims. Notes:
• The signature on file date on the patient information form may or may not be the same as the date shown on the patient encounter form. The date on the encounter form is the date of the current visit. The signature on file date on the patient information form is the date the patient first filled in the patient information form or last updated it. At Valley Associates, PC, returning patients are generally asked to confirm/update their information at the beginning of each year.
• For TRICARE claims, the sponsor’s date of birth is listed in the case study text along with the patient information.
• In the claim case answer keys that follow, the carrier information block in the upper right corner of the claim form has intentionally been left blank as carrier information is not included as part of the case study text.
