This report includes a summary of the many initiatives we have undertaken to enhance quality, improve access, encourage innovation and fulfill a passionate commitment to community service. As you will see, we are defining the future of healthcare in Omaha.

In 2013 Charles Drew served 10,451 unique patients. We facilitated 38,304 patient visits (a 12% increase over

2012). Patients 18 and under, represented 35% of our patient population. Although 47% of our patients were uninsured – none were denied health care services due to their inability to pay.

Charles Drew Health Center provided employment opportunities for 234 individuals, many residing in the Health Centers targeted service area, (east/northeast and northwest sector of Douglas County) with an overall direct/indirect economic impact of over $15 million.

Our goal is to provide quality, comprehensive affordable healthcare to all individuals in a comfortable and safe environment. Charles Drew Health Center strives to recruit the best medical, dental and behavioral health providers available while supporting them with qualified, culturally competent, compassionate staff. We believe the personal relationship between a patient and doctor is the key to quality health and dental care. It is our goal to treat all patients with the care and respect we all deserve.

One of the key factors that distinguishes a thriving community is its ability to deliver excellent medical care to every resident and community member in which it serves. The character of a community is defined by many factors, how well it answers the health care needs of its population is surely one of the most important. Our Community Profile Charles Drew Health Center (CDHC) provides quality affordable health care services for individuals and families living throughout Douglas County in Omaha, Nebraska. According to 2010 US Census data for Nebraska,

Our Target Population Profile The Charles Drew Health Center service area includes the east/northeast and northwest sectors of Douglas County in Omaha, Nebraska. The health center’s target population is the east/northeast sector of Douglas County. Population data from the 2010 US Census provides the following data:

• CDHC’s target population is comprised of 44,410 residents.

• CDHC’s target area includes 18 census tracts designated as medically underserved and a health professional shortage

area. All but one of the census tracts in the CDHC project area is designated as “areas of racial and ethnic minority

concentration.”

Community, Target and Patient Profiles

HEALTH CARE SERVICESMedical Care

Charles Drew Health Center served 10,451 patients and facilitated 38,304 patient encounters (visits) in 2013. Charles Drew Health Center provides care for the entire family. Services include family practice, internal medicine, pediatrics, OB/GYN, pharmacy, radiology, opthalmology, minor surgery, behavioral health, nutrition, cardiology and podiatry.

Homeless Health Care

Of the 10,451 patients served in 2013, 1,568 were homeless. The Health Center serves homeless patients at two locations: Catholic Charities Campus of Hope, located at 1490 North 16th Street and the Siena Francis House, located at 1111 North 17th Street.

Dental Care The CDHC Dental Program served 2,542 patients. This represents a 25% increase over 2012. The Dental program facilitated 6,291 patient visits in 2013. This represents a 28% increase over 2012.

The CDHC Dental Department provides oral exams, sealants, fluoride treatments, restorative services, oral surgery (extractions) emergency services and rehabilitative services.

New Baby Beginnings

The CDHC Baby Beginnings perinatal care program offers a comprehensive approach to ensuring healthy birth outcomes for infants and mothers. The program is staffed by one family practice physician/OB, a pediatrician and a perinatal nurse. In 2013, Charles Drew Health Center provided perinatal care services to 136 women. Of the 136 women served, there were 81 deliveries.

Mental Health Services

In 2013 Charles Drew Health Center served 630 patients and facilitated a total of 1,884 patient visits through its Integrated Primary Care Behavioral Health Program. The program is supported in part through grant dollars funded by the United Way of the Midlands and Region VI Behavioral Healthcare.

Pharmacy

During 2013 the CDHC pharmacy filled 27,185 prescriptions at an average cost of $ 14.57. When compared to the national average of $ 83.26 (AARP 2012 average price + a 3.6 % increase), patients at Charles Drew Health Center saved $1,867,123.

The average cost of a prescription at the Charles Drew pharmacy increased by 1.18 %. The pharmacy also filled 426 prescriptions at a cost of over $35,000 for uninsured patients with emergency medical needs.

Prescription Assistance Program In 2013, through the Medication Assistance Program, 814 prescriptions were filled at a cost to the patients of $20,350, at a value of $67,773.64. This represents a savings of $47,423.64 to CDHC Patients.

School-based Health Centers

Building Healthy Futures School-based Health Centers program partners (which includes Charles Drew Health Center) impacted 10,166 children during the 2013-2014 school year. The mission of the Building Healthy Futures School-based Health Centers is to improve the health for our community’s underserved children and youth through thoughtful collaboration and advocacy.

Charles Drew Health Center provides primary health care services at four of eight Omaha Public Schools School-based Health Centers. The total number of unique patients served was 1,328. Charles Drew Health Center receives funding support from Building Healthy Families to operate the following School-based Health Centers:

Public Housing Health Centers In 2013 CDHC served 250 patients residing in public housing. CDHC in partnership with Omaha Housing Authority operates and manages four (4) Public Housing Health Centers at the following OHA locations:

Chronic Disease Management

The CDHC Chronic Disease Management Program provides systems of care designed to improve patient health outcomes and reduce costs associated with long-term chronic illness. Currently CDHC operates five chronic disease management programs, which are diabetes, asthma,

cardiovascular disease, depression and obesity. Through these programs, CDHC case managed 541 diabetic patients, 1,033 patients with a diagnosis of hypertension and 75 asthmatic patients. Additionally, CDHC addressed weight management with 1,281 pediatric and 541 adult patients.

HIV/AIDS Testing and Counseling

In 2013, CDHC tested a total of 1,161 individuals. HIV/AIDS testing and counseling was provided at the main clinic and at both homeless clinic locations. Outreach and community health education initiatives also include free HIV testing and counseling.

STD Counseling, Testing and Partner Notification During the calendar year of 2013, CDHC provided STD testing, counseling, treatment and partner notification for 1,931 individuals. Of the 1,931 individuals tested, 19.6% (378) individuals tested positive. The average age of the population served was 26.

Of the 378 individuals that tested positive for an STD, 269 tested positive for Chlamydia (71.2%) and 109 (28.8%) tested positive for Gonorrhea.

In 2013 the Charles Drew WIC program facilitated 34,957 program participant visits.

Omaha Healthy Start (OHS) The mission of OHS is to reduce the rate of infant mortality in East/Northeast sector of Douglas County in Omaha, Nebraska by means of partnership with families, community members and organizations through awareness, training and education.

Through the Omaha Healthy Start HealthNET case management program, OHS staff provided the following services in 2013:

• One-on-one case management, screenings and health education and training for 56 pregnant women and 211 inter-conceptional women;

• There were 38 births among OHS program participants;

• OHS case managers completed 1,069 home/office visits;

• OHS provided 121 pregnancy tests at no cost to women;

• OHS provided 120 STD tests at no cost to women;

• OHS completed 623 program participant transportation requests; and

• Expanded its Health Education and Training component from 13 on-site classes for women only to 16 on-site classes for women and men.

FITNESS AND NUTRITIONHealthy Families

Healthy Families is aimed at reducing obesity among children with a body mass index (BMI) at or above the 85th percentile. Seven families were enrolled in the program in 2013. Children and families participating in this program are referred by their primary care physician. The program is sponsored by Alegent Health and is offered to families at no cost.

Employee and Community Wellness

Charles Drew Health Center has a commitment to improving not only the health and well-being of its patients, but its staff as well. In 2013 the CDHC Employee and Community Wellness program provided the following services:

• Employee Wellness Challenges for Weight Loss, Physical Activity, Better Nutrition, Stress Reduction. On an average, 50% of CDHC employees participated in each Wellness Challenge.

• Employee Zumba classes for employees and the community. Classes accommodate 25 participants.

• Earn-A-Bike program for children. Sixty children earned new bikes, new helmets and bike locks by completing volunteer services in the community. The children and their parents were required to participate in Wellness Workshops (Better Nutrition and The Importance of Physical Activity). They also participated in a Bike Safety Workshop. This program was made possible by Alegent Health and Mutual of Omaha.

OTHER ENABLING SERVICESHealth Insurance Marketplace Outreach/ Enrollment (O/E) Efforts In 2013 Charles Drew Health Center was among health centers that received funding designed to assist health center patients in establishing eligibility for and gaining access to Federal, State, and local programs that provide or financially support the provision of medical, social, housing, educational, or other related services.

These funds were invested in health centers to expand current outreach and enrollment assistance activities and facilitate enrollment of eligible health center patients and service area residents into affordable health insurance coverage through the Health Insurance Marketplace, Medicaid, or the Children’s Health Insurance Program.

Charles Drew Health Center provided the following Health Insurance Marketplace Outreach/Enrollment support services in 2013:

Interpreter Services

Charles Drew Health Center continues to provide on-site interpreter services for the medical, dental and WIC clinics and for the Omaha Healthy Start HealthNET case management program. In 2013 CDHC provided interpreter service encounters (appointments) for 4,532 patients. This represents a 6% increase over interpreter services provided in 2012.

Disabilities Assistance Program

In 2013 CDHC provided assistance to 33 medical and 25 dental patients through its Disabilities Assistance Program.

Fathers for a Lifetime (FFL)

Fathers for a Lifetime program provided the following services in 2013:

• Graduated 51 men from its 12-week evening educational sessions;

• Maintained a monthly case management and care coordination caseload of 21 men through the FFL Resource Office;

• Processed 2,227 contacts – 1,611 calls and 152 texts/emails for assistance (this represents a 1% decrease from 2012); and

• Served 464+ men through its FFL Resource Office on a walk-in basis.

Comparative Analysis

Charles Richard Drew attended Amherst College, where he became an all-American football player. Amherst College, at that time, was one of the few white colleges who accepted sizable numbers of black students.

In 1926, after graduation from Amherst, Drew took a job at Morgan College as a biology teacher and athletics director.

Earning his money coaching athletes, Drew then returned to college to receive his medical degree. He graduated from McGill University’s medical program, and took an internship and residency in Canada. Afterwards, he became a teacher of pathology at Howard College, and a resident at Freedman’s Hospital in Washington D.C. He then worked and taught in New York, at the Columbia Presbyterian Hospital, receiving his Doctorate from Columbia University.

His dissertation was on the condition of blood stored in blood banks, and the method of storing blood as plasma (sans white or red blood cells) to increase storage life. He then supervised the blood-plasma division of New York City’s Blood Transfusion Association, which was involved in collecting blood for the British Army.

When America went to war in 1941, Drew was named as director of the blood bank for the National Research Council, collecting blood for the US Army and Navy, and setting the groundwork for the Red Cross collecting and banking procedures. In 1942, Drew returned to Washington D.C., where he became head of Howard University’s Department of Surgery and Chief Surgeon at Freedman’s Hospital. In 1944, he was elevated to hospital Chief of Staff and Medical Director, a position he held until 1948. He was killed in an automobile accident in 1950, on the way to a medical conference.

The storage of blood in plasma form has saved untold lives since Drew brought the process forward in the 1930’s, and he created the model for blood and plasma storage that is used by the Red Cross even today. His importance to modern medicine cannot be overstated.

Dr. Drew was a brilliant African American physician, educator and surgeon, famous for his pioneering work

in blood preservation.
