Missouri Delta Physician Services and Patients Embrace Telemedicine

Telemedicinehere to stay

When Sikeston resident Amy Evans recently called to make a doctor’s appointment for her teenage son, she learned he could be seen through a virtual visit. “They offered the telehealth, and we were interested. We were trying to avoid going out as much as possible,” Evans said. Telehealth is defined as the delivery of healthcare services via electronic devices allowing for video and audio interactions between a patient and healthcare provider who are not in the same location. Evans said she made the telehealth appointment with Amy Folsom, PA with Missouri Delta Primary Care. “It was extremely easy and convenient,” she said. When the time came, Evans said she used her iPhone to connect with Folsom’s office after they provided her with a link. She and her son found a quiet place in their home to limit distractions during the appointment. Folsom connected with them through a live chat and assessed Evans’ son by asking about his symptoms and ultimately looking at his throat through their devices. “Our provider recommended a follow-up swab for strep throat,” Evans said. She and her son drove to the clinic, which provided curbside service. “We pulled up in the front, and the nurse came out and did the swab. Fifteen minutes later, we had the results, and he had tested positive for strep throat,” she said. Folsom then prescribed medication, which Evans retrieved from their local pharmacy.

“It was convenient, and it kept everyone safe, and we really appreciate they are offering the services,” Evans said, adding telehealth appointments and curbside testing limits exposure for all involved. Latina Hampton, director of operations for physician services at Missouri Delta Medical Center, said telehealth has been around, but there wasn’t really a push to offer the service to patients until the COVID-19 outbreak. Missouri Delta uses the platform, Doxy.me, to conduct its telehealth appointments. Hampton, who is a registered nurse with a master’s degree in nursing, said she conducted research and learned Doxy.me was simple to implement and user-friendly. Before offering it to patients, the

hospital’s IT team conducted trial runs and worked out connectivity issues, she said. On March 27, Missouri Delta conducted its first telehealth appointment, but it wasn’t until a few days later, around April 3, when all of its physician services and clinics were utilizing telehealth. When the COVID-19 outbreak hit the area, physicians soon began seeing less than half of the visits they normally would — and they knew patients were still in need of their physicians, Hampton said. “People who were sick weren’t coming to see their doctors, and they were getting even more sick.” Appointment volume is picking up again, Hampton said, adding the first day of telehealth use included one or two visits. Now some providers are doing 10 or more a day, she said. “We are still seeing patients in person,” Hampton said. However, Hampton said patients who are interested in the service should call their providers and ask about telehealth. Once a telehealth appointment has been made, the process is simple, she said. “Our nurses send them a link by email or text. Using the link, the patient checks in for the appointment from their device. The nurse will do patient intake and get information and updates on their medicines. Then they are put back in the virtual waiting room. The provider will open the video chat and do their part,” Hampton explained. Also, during the COVID-19 pandemic, virtual visits are now a covered benefit by Medicare and most insurance companies. Tips for patients to make virtual visits go smoothly include: find a quiet location where video works best; be patient; weigh themselves; if they have a blood pressure machine, check their blood pressure; and if diabetic, check blood sugars. The nurses will document the information, according to Hampton. “Our physicians and staff were very happy to start virtual visits,” Hampton said. “They all agree this is a positive option that reduces the risk for unnecessary contact.”

We had an eventful and productive 2019. As you know the hospital physical environment must be maintained to a level of readiness and safety for all occupants. The authorities having jurisdiction over the environment of care dictate that we meet conditions of participation with Centers for Medicare and Medicaid Services (CMS), Joint Commission and other requirements of federal, state and local authorities. Many of the work hours and projects that Plant Operations do over a fiscal year are directly mandated by these entities and a large numbers of hours are spent upgrading or maintaining existing conditions of the physical plant. In 2019, the Engineering and Biomedical Staff logged over 10,472 scheduled and unscheduled work-orders with 5,260 of those being corrective or repair type work orders. In addition, we have been very busy with large construction projects and other remodeling projects to ensure Missouri Delta Medical Center provides the best environment of care possible.

The Adolescent Behavioral Health project began in 2019 and we are happy to announce that this project has been completed and ready for occupancy as of May 13, 2020. The State Bureau of Hospital Standards approved the physical space for occupancy on May 1, 2020 after an online inspection that was completed by myself and the State. The on-line inspection and documentation/picture/video survey was done off-site due to the COVID-19 epidemic. There were no deficiencies found during the inspection. The Adolescent Behavioral Health project consisted of construction of over 7,000 square feet of patient space that supports patients in a ligature resistant psych-safe environment. The project also included replacement of antiquated HVAC/Air handler, plumbing, and electrical systems that had been in place since 1960. The Adolescent Behavioral Health Unit is the first of its kind in the entire region.

The Senior Lifestyles Unit remodel was completed in 2019. The unit underwent a heavy remodel by our in-house staff and

general contractor for the mechanical, plumbing and electric. The unit floors, walls, ceilings, windows and nursing station were all remodeled following guidelines for behavioral health physical environment. Some of the biggest improvements to patient safety are the Lexan window coverings, door alarm system, cameras, and new flooring. The space is safer, cleaner and more appealing to all.

Ongoing LED lighting conversions continue to take place in all areas of the hospital for cleaner, brighter lighting that costs 70% less to operate verses the older fluorescent lights. We are approximately 70% LED inside the hospital at this time and are currently working on the front main parking area, main hospital canopy, and pathway lighting upgrades. The Emergency Department project included LED conversion.

The Radiology Department was painted and the reception work station was modernized in 2019. There was over 8,000 square feet that was painted and the old film storage area in the center of the department was converted to a useful storage space. LED Lighting was installed.

The OB/Pediatric lobby was renovated this past year. LED lighting, paint, furnishings and flooring were all included in this project. The space is modern and can accommodate more families.

The ICU Waiting area was remodeled with flooring, paint and new volunteers desk and chairs. Modern LED lighting was installed.

Medical Air Compressor H-Basement was upgraded to meet NFPA standards and ensure patient safety and capacity for current and future use.

The Coleman Room was renovated and modernized with new lighting, paint and flooring. This was part of a Joint

Rural health and specialty clinics saw many improvements in 2019. The Portageville Clinic received a 1,500 square foot addition to help support an ever-growing patient population. The existing clinic was remodeled as well. The entire clinic looks modern and is much more functional. The renovation added 2 ADA bathrooms, one in the waiting area and the other by the exam rooms. The clinic now has 10 exam rooms and 4 offices to support operations. A nurse’s station was added to ensure

privacy and an efficient work space for staff. Additionally, the parking space was doubled and new asphalt and striping was added with additional lighting in the parking areas.

The Urology Clinic that is located at 1106 North Main, Sikeston was completely remodeled by the in-house engineering staff starting in December of 2019 and finishing April 2020. The remodel touched every space in the clinic and included flooring, lights, ceiling tile, walls, cabinetry, windows, and replacement of the roof. The biggest improvement was removing the old carpet to support a healthier environment and the addition

of 4 exam rooms, two nurse workstations, a staff break-room and updated bathrooms. The patients and staff are really pleased with the final results of this project.

The rural health clinic in New Madrid has undergone much renovation as well. The receptionist area has been updated and a nurses work station has been added along with complete paint and lighting upgrades. The bathrooms have been updated and one has been made ADA compliant. The area still needs space for storage as they continue upward in patient population.

Benton Community Care Center has seen improvements in the parking lot. The gravel lot was expanded and asphalted

to reduce risks of falls on the gravel. The parking space is now modern and useful with good flow.

Dr. Hunter’s office in Sikeston had renovations to the waiting area and clinic vestibule. The old worn flooring and trim was replaced and the walls were painted. The area now is modernized and serving the patients well.

The north entrance of the Jolly Building was fitted with an automatic door that made it safer for patients entering the building. Plans are to install automatic doors on the east entrance into the building in the near future. We are currently working on replacing all of the building glass that has been stained and broken.

Moving Forward Despite UncertaintyA Letter from The President, Jason Schrumpf

A lot has changed at Missouri Delta over the past year, and a significant level of uncertainty remains regarding our current environment. We started off the year with strong demand across most aspects of our healthcare system, abruptly slowed down with the COVID-19 pandemic and now are seeing volumes slowly come back. The challenges related to COVID-19 are immense, and we have worked hard to make the necessary changes to ensure the safety of our patients, employees, and community. Throughout the pandemic, we have taken a measured approach based upon guidance by the Centers for Disease Control (CDC). During these challenging and uncertain times, we remain focused on executing our strategy, so that we can weather this storm and emerge stronger than ever before. We are working not only to battle the current crisis, but to also make meaningful and necessary changes that will benefit Missouri Delta’s future.

A crisis of this magnitude highlights the need for organizations to be diversified. Although many areas of our hospital have been negatively impacted, demand for our behavioral health service lines have remained strong. We ended 2018 with the expansion of our 12 bed inpatient geriatric behavioral health unit with the addition of a new 11 bed unit. Additionally, we hired three mid-level providers that help manage behavioral health patients in our clinics, nursing homes, and hospital. Lastly, we opened our 15 bed inpatient adolescent behavioral health unit on May 13th and reached a census of 9 patients six days following the opening of the unit. For people throughout the region who faced mental health illnesses pre-COVID-19, these health issues are likely to persist post-quarantine to a greater extent. Those people who did not show signs of depression, anxiety, or substance abuse may be at a heightened risk of developing tendencies in this new COVID-19 environment. Therefore, an increased demand for behavioral health services in a previously underserved segment is expected to continue. We are very fortunate to now have the expertise and facilities to address mental illness for patients in our region.

At the onset of the COVID-19 pandemic and the subsequent passage of the CARES Act, the Centers for Medicare and Medicaid Services (CMS) expanded the scope of services covered by Medicare. As a result, we quickly implemented telehealth solutions and have found tremendous

value in connecting our providers and patients. Subsequently, the demand for telehealth solutions has grown exponentially, and it has served as a critical resource for Missouri Delta as a cost effective solution in expanding access to care while battling COVID-19. From behavioral health visits to primary care check-ups, virtual care is here to stay.

Recruitment and retention of staff have been pillars of our success, and we have made great strides this past year. Throughout this crisis, we have managed to retain our employees and expand our medical staff with key additions in Internal Medicine, Family Practice, Pediatrics, Psychiatry, General Surgery, Vascular Surgery, Radiology, Neurology, and Neurosurgery with Dr. Colle. Due to these efforts and the growing demand for space, we are anxiously awaiting the opening of the 15,000 square feet Medical Office Building South and are in the planning phase to renovate the second floor of the Jaycee Dialysis Building to create additional clinic space.

This past year, we discharged 4,017 patients from our hospital, saw 20,442 patients in the emergency department and treated 128,411 patients throughout our 21 clinics. We invested over $4.5 million in property and equipment to expand service offerings and improve our medical center. Our workforce of over 900 employees are able and ready to serve the community. Many operational processes are in place for us to safely perform elective surgeries and imaging procedures including screening mammograms, and provide face-to-face provider visits. We are pleased to welcome back our patients for any healthcare services needed at Missouri Delta Medical Center!

Our hospital has prospered for the past 72 years because of the support provided by the community. Your continued support is vital to our success, especially in these challenging times. With your caring generosity and loyalty, we will continue to pursue our strategies, so we can build a stronger medical center with a diverse and skilled workforce equipped to deliver the highest quality healthcare and provide the best experience possible for our patients.

I always look forward to publishing the annual report. It’s humbling to see all that you have helped Missouri Delta accomplish. A special thanks to our employees, friends, donors, volunteers, and grants. We were able to purchase items, improve patient areas, help those in need through scholarships and assistance and with money raised at our events and through donations we were able to help build the new adolescent behavioral health unit. The growth and enhancements at Missouri Delta are because of you.

Ringing in the new year and new decade at the Greatest Event on Earth was a fabulous way to kick start 2020. We entered into the new decade with enthusiasm of what is planned for 2020 and with anticipation of what is coming to Missouri Delta. Never in a million years did we ever foresee the Coronovirus/COVID-19 hitting with such a force. On a positive note, the lockdown-virus-Pandemic has brought families and communities closer together and allowed businesses to use their creativity to keep their patrons engaged through these challenging times.

The Foundation thanks our service communities, as well as vendors outside of Sikeston, and even the state of Missouri for the love and assistance through these unprecedented times. The Year of the Nurse and Midwife

When the World Health Organization designated 2020 as the “Year of the Nurse and Midwife,” marking 200 years since the birth of Florence Nightingale, it had no idea of the pivotal role nurses would play in leading a world population through an unforeseen pandemic.

The Year of the Nurse and Midwife and COVID-19 has demonstrated the value of nurses as heroes, caregivers and advocates. I am honored to be a nurse as I witness the compassion and bravery exhibited by nurses at Missouri Delta Medical Center and throughout the world during this pandemic. Nurses have the enormous privilege of carrying the burden of preserving human dignity and caring for others when they are at their most fragile, bringing hope on what may be the worst day of someone’s life.

Missouri Delta Medial Center takes great pride in knowing that our nurses and care teams have helped enabled fourteen and counting COVID-19 positive patients to return home to their families and communities. For those, who unfortunately, lost their battle with COVID-19, the care team helped them pass with dignity, and found innovative ways for families to safely connect with their loved ones at this critical time.

We are so thankful for the selfless care the nursing staff at Missouri Delta Medical Center has provided to COVID-19 patients.

Our population health department works with patients to coordinate care between the hospital and our clinics. The department is comprised of nurses and social workers who work with patients to help them manage chronic illness by providing education and information about resources that are available to them. Our goal is to establish relationships with patients, help to promote quality health care, lower costs, and reduce readmission rates. Care gaps are also monitored so patients can stay up to date and informed on preventative services available to them.

Currently we have over 1,200 patients enrolled in our health home program and almost 3,000 attributed patients through the Accountable Care Organization (ACO), which is a large collaborative ACO through Caravan Health. These services are also provided to all managed Medicaid patients, managed Medicare patients, as well as UHC and Anthem patients.

Population Health obtained NCQA recognition for Missouri Delta Medical Center in 2017 as a primary care medical home. In 2019, Missouri Delta was awarded a Gold Award for performance through Health Care Quality Improvement Awards. Our population health department has also had the privilege to speak at several online events for Caravan Health to promote best practices for quality measures and our high performance in chronic care management.

Missouri’s Medicaid program currently does not cover most adults without children, and it’s income eligibility threshold for parents is one of the lowest in the nation at about one-fifth of the poverty level. In the 2018 Community Investment Report, Missouri Delta Medical Center reported $2,630,696 in uncompensated care. Based on a range of experiences in expansion states reported in the literature, uncompensated care may decrease between 25% and 47% when Medicaid expansion is fully implemented. Based on these ranges, uncompensated care at Missouri Delta Medical Center could be reduced by $657,674 to $1,236,427. The ballot proposal would expand eligibility under the terms of the 2010 federal health care law, Affordable Care Act (ACA). That law provides a higher-than-usual federal funding share for states that expand Medicaid coverage to adults earning up to 138% of the federal poverty level, about $17,600 for an individual or $30,000 for a family of three. For 2020 and subsequent years, the federal government is set to cover 90 percent of the costs.

Thirty-six states have adopted Medicaid expansion measures. Medicaid expansion will bring more than $1 billion of Missouri tax dollars back from Washington every year. Supporters estimate 230,000 additional adults would enroll in Missouri’s Medicaid program, if voters approve the expansion. That will help our hospitals financial position, protect our jobs, and ensure all Southeast Missourians have access to health care. Additionally, Medicaid expansion can help reduce high-cost emergency health interventions that could be managed with regular primary care, including access to rescue medications or chronic disease management. Avoiding unnecessary emergency department care will reduce costs for patients and the healthcare system, and increase the efficient use of health resources in a more preventative setting. Hospital closures in rural communities have increased the distance to lifesaving care for Missourians suffering from traumatic injuries, stroke and heart attack. Minutes count in medical emergencies. Medicaid expansion will help maintain access to emergency care in rural Missouri — benefiting those gaining coverage and all rural residents.

We were excited to add an event in Mississippi County this past fall. I would like to thank the following for all of their help, creativity and energy: Martha Oliver, Heather Collier, Whitney Montgomery, Anna Ferrell, Sarah Hicks, Alan Mills, Helen Renaud, Morgan Goodin, Leigh Tyler Oliver, Lisa Minner, Brooke Silman and Bailey Emerson and Coach Minner’s Class for their strength, lay out of the tables and assistance. Proceeds from the Tacky Prom event went towards the new Adolscent Behavioral Health Unit which serves the surrounding communities.

The Annual Breast Cancer Luncheon was held in October at the First Baptist Church. Dr. Paul Montany was the 2019 speaker and gave the doctor’s point of view of breast cancer. We served approximately 370 meals both dine-in and carry-out. We couldn’t have a successful luncheon without the help of the volunteers: Elizabeth Alcorn, Cay Sikes, Alice Nunnelee, Kathy Mitchell, Mary Carolyn Adams, Carolyn Sindle, Pat Glass, Sharon Urhahn, Morgan VanPool, Whitney Montgomery, Pat Thurman, Debbie Nichols, Jane Eaves, Sherry Welker, Jan Barkett, Marilyn Boyce, Toni Kellett and Sikeston High School/SCTC Skills USA Students. Thank you to each of the businesses that donated items for raffles and door prizes. We raised approximately $11,000! This money is used for Project Mammogram which helps underinsured and the uninsured patients receive mammograms.

The 2019 Misssouri Delta Foundation Ball featured Shades of Soul. The $80,000 raised at this event went toward the new Adolscent Behavioral Health Unit which opened May 2020.

We couldn’t have a successful ball without the help of the committee members: Amy Alcorn, Blake Rosen Bain, Sheila Bizzell, Heather Bohannon, Angela Gilmore, Susie Lawrence, Meredyth McKelvey, Katie Merideth, Rhonda Mitchell, Whitney Montgomery, Micah Nesselrodt, Catherine Schrumpf, Megan Straton, Sharon Urhahn, Heather VanPool and Morgan VanPool.

With your dedication and support to our community hospital, we’ve been able to accomplish great things! From supplying our new Adolescent Behavioral Health Unit with games, yoga mats, music and headphones to our Respiratory Department with BiPap and vital sign machines for our off campus clinics. Because of you, our Emergency Department is now equipped with an automatic lifesaving device that will save time and staff when minutes count.
