The draft will be based on this outline, which will be presented to the HC for their approval. Special thanks to Dan for providing the cover art for both this presentation and the SFDPH Annual Report. Dan is an SF artist who participated in the visioning process. We selected this artwork as it serves as a good representation of the work that has been done in integrating input from SF residents, communities, and other stakeholders into a cohesive vision of health for the entire city. Special thanks also to the SFDPH staff who contributed their time and information to make this report possible. Annual Report represents a collective effort, hopefully one that makes us proud of our accomplishments and proud to represent SFDPH. Also, the colors presented both in this presentation and in the SFDPH Annual Report are consistent with those on the SFDPH website and represent our effort at branding.

This section will focus on updates regarding the Health Commission’s identified priorities: Integrated Delivery planning outcomes, public health accreditation, and financial and operational efficiency. These priorities mirror the “triple aim” of improving the experience of care, improving the health of populations, and reducing per capita costs of health care.

5. This section will target the accomplishments of each SFDPH division during FY 11/12. Division leadership will determine which accomplishments to highlight. For the purposes of the Annual Report, leadership will be asked to limit their submissions to three to five accomplishments/highlights per division.

6. This section will present numeric data relevant to the Department’s work throughout FY 11/12. Similar to that included in previous annual reports, data will present DPH patient demographics and budget information. This section will include visuals (charts, graphs, tables) to make the data more understandable and user-friendly. Service providers include: - SFGH - HIV Prevention - LHH- HIV Health Services - CBHS- Jail Health Services - COPC- SF City Clinic - HEALTHY SF 7. This section will present to the reader with a visual summary of DPH’s structure. This section will also depict the locations of all DPH service sites as well as those of DPH affiliated partners. Finally, this section will provide a list of all entities with whom DPH contracted in FY 11/12.
